Infant Anemia and Micronutrient Status : Studies of Early Determinants in Rural BangladeshEneroth, Hanna January 2011 (has links)
Anemia and micronutrient deficiencies in infancy are common in low-income settings. These are partly due to maternal malnutrition and may impair child health and development. We studied the impact of maternal food and micronutrient supplementation, duration of exclusive breastfeeding (EBF), growth and infection on infant anemia and micronutrient status. In the MINIMat trial in Matlab, Bangladesh, pregnant women were randomized to Early or Usual promotion of enrolment in a food supplementation program and to one of three daily micronutrient supplements. Capsules containing 400µg folic acid and (a) 30 mg iron (Fe30Fol), (b) 60 mg iron (Fe60Fol), (c) 30 mg iron and other micronutrients (MMS) were provided from week 14 of gestation. Capsule intake was assessed with the eDEM device recording supplement container openings. Blood samples (n=2377) from women at week 14 and 30 were analyzed for hemoglobin (Hb). Duration of EBF and infant morbidity was based on monthly maternal recalls. Infants were weighed and measured monthly. Blood samples (n=1066) from 6-months-old infants were analyzed for Hb and plasma ferritin, zinc, retinol, vitamin B12 and folate. In women, Hb increase per capsule reached a plateau at 60 Fe60Fol capsules, indicating that nine weeks of daily supplementation produced maximum Hb response. Anemia was common (36%) at capsule intakes >60 indicating other causes of anemia than iron deficiency. In infants, vitamin B12 deficiency prevalence was lower in the MMS (26.1%) than in the Fe30Fol group (36.5%), (p=0.003) and zinc deficiency prevalence was lower in the Usual than in the Early group. There were no other differential effects of food or micronutrient supplementation on infant anemia or micronutrient status. Infants exclusively breast-fed for 4-6 months had a higher mean plasma zinc concentration (9.9±2.3 µmol/L) than infants exclusively breast-fed for <4 months (9.5±2.0 µmol/L), (p< 0.01). No other differences in anemia, iron or zinc status were observed between EBF categories. Infection, low birth weight and iron deficiency were independent risk factors for infant anemia. Regardless of studied interventions, prevalence of anemia (43%), deficiency of zinc (56%), vitamin B12, vitamin A (19%) and iron (22%) in infancy was high and further preventive strategies are needed. / MINIMat

Recognition of Diversity: Charles Taylor's Educational ThoughtPalma, Anthony 13 August 2014 (has links)
This study focuses on Charles Taylor’s educational thought with a view to understanding his contributions to the discipline of Philosophy of Education. No comprehensive study of Charles Taylor’s educational thought has been attempted. There is a single dissertation and a dozen or so published periodical articles that do take Taylor’s educational views into consideration, to be sure. Yet these studies, which limit themselves to Taylor’s account of the recognition and/or non-recognition of identity in multicultural societies, are insufficient on five accounts: i) they are indifferent to the historical nature of Taylor’s scholarly work; ii) they neglect the philosophical sources of his educational thought; iii) they fail to highlight the interconnections between the key educational themes he takes up; iv) they disregard his major critics and the dialectical tensions raised by these critics; and v) they are somewhat dated in that they do not consider his more recent scholarship. My dissertation seeks to fill these scholarly gaps. My thesis is that an inner logic is implicit in Charles Taylor’s educational thought. I argue that Taylor’s views on the modern condition, (i.e. in his readings of Descartes, Kant, Herder, and Hegel), are closely interwoven with his views on modern education, and that interconnected currents in the modern history of ideas elucidated by Taylor, (i.e. scientific rationality, exclusive humanism, and the ethics of authenticity), have contributed to the rise of, and the sensitivity toward, both the theory and the practice of the politics of recognition in contemporary educational institutions. I conclude that an education for culturally diverse minds and hearts, anchored in human, historical, and epistemological recognition, and democratically open to both immanence and transcendence, is the true calling of Taylor’s educational thought.

Epigenetic regulation of clonally variant gene expression in Plasmodium falciparumCrowley, Valerie Margarita 02 November 2011 (has links)
Clonally variant gene expression (CVGE) is a common survival strategy used by many pathogens, including the malaria parasite Plasmodium falciparum. Among the genes that show CVGE are several members of the clag and eba families. The active or repressed state of clag3.1, clag3.2 and eba‐140 is transmitted epigenetically and is controlled by the euchromatic or heterochromatic state of bistable chromatin domains, characterised by H3K9ac and H3K9me respectively. Both of these modifications are maintained throughout the asexual cycle to transmit epigenetic memory. We have characterised CVGE on a genome‐wide level and found that it is widespread. Using clag3.2 in proof‐of‐principle experiments, we demonstrate that CVGE for genes other than var can be reproduced in an episomal system and provide preliminary evidence that CVGE is regulated by the spontaneous formation of facultative heterochromatin. The mutually exclusive expression of clag3.1 and clag3.2 is also addressed, including the characterisation of a neighbouring noncoding RNA. / La variació clonal en l'expressió gènica (CVGE) és una estratègia de supervivència comú utilitzada per molts patògens, incloent el paràsit de la malària Plasmodium falciparum. Entre els gens que mostren CVGE hi ha diversos membres de les famílies clag i eba. L'estat actiu o inactiu de clag3.1, clag3.2 i eba‐140 es transmet epigenèticament i és controlat per l'estat eucromatínic o heterocromatínic de dominis de cromatina biestables, caracteritzats respectivament per H3K9ac i H3K9me. Ambdues modificacions es mantenen durant tot el cicle asexual per transmetre la informació epigenètica. També hem caracteritzat la CVGE a nivell de tot el genoma, i hem vist que afecta un gran nombre de gens. Experiments preliminars amb clag3.2 ens demostren que la CVGE de gens diferents que var es pot reproduir en un sistema episomal I proporcionen evidència preliminar de què la CVGE està regulada per la formació espontània d’heterocromatina facultativa. També s’ha estudiat l'expressió mútuament exclusiva de clag3.1 i clag3.2, incloent la caracterització d’un ARN no‐codificant veí.

La fausse qualification de «moyen de défense» attribuée par tradition à l'exception de procédure a conduit le législateur à la soumettre à un régime totalement incohérent et inadapté. L'illustration en est donné par l'échec constaté de l'exclusivité de compétence attribuée au juge de la mise état sur les exceptions de procédure, échec se matérialisant par les nombreuses dérogations que ne cesse d'apporter la jurisprudence à leur régime. Elle se traduit aussi par la quasi-impossibilité pour le demandeur à l'action principale de s'en prévaloir, ce en totale contradiction avec l'esprit des textes. Cette incohérence est illustrée enfin par les confusions opportunistes entre exception de procédure, fin de non-recevoir, incident et défense au fond. L'exception de procédure n'est pas un moyen de défense mais une demande incidente relative à la marche de la procédure qui en termes d'ordre logique doit être examinée par préalable au fond. Elle n'intéresse donc que les rapports procéduraux c'est-à-dire l'instance dont l'ouverture et la conduite aux termes des articles 1 et 2 du Code de procédure civile appartiennent aux parties. La classification et le régime des exceptions de procédure doivent en tenir compte. Il faut donc distinguer les exceptions de procédure opposées à l'ouverture de l'instance de celles qui sont opposées à sa continuation. Seules les premières doivent relever de la compétence exclusive du juge de la mise en état de qui on doit réussir à faire un véritable juge de l'introduction de l'instance. Les exceptions de procédure opposées à la continuation, par contre, doivent pouvoir être proposées au fur et à mesure de leur survenance ou de leur révélation sauf la possibilité pour le juge de les écarter ou de prononcer des condamnations pécuniaires à l'encontre de la partie qui se serait abstenue dans une intention dilatoire ou abusive de les soulever plus tôt. / The false qualification of "defense" awarded by tradition to the procedural objection led the legislator to submit it to a totally inconsistent and unsuitable regime. The illustration is given by the failure noticed by the exclusivity of competence awarded to the judge of the enabled on the procedural objections, failure materializing by the numerous dispensations which the jurisprudence continues bringing to their regime. It is also translated by the quasi-impossibility for the applicant in the main action to claim it in whole contradiction with the spirit of Law. This incoherence is finally illustrated by the opportunist confusions between procedural objection, refusal of the action's receving, incident and defense in fact. The procedural objection is not a defense but an incidental request relative to the step of the procedure which in term of logical order must be examined by prerequisite in fact. It thus interests only the procedural relationships that is to say the instance which the opening and the conduct (driving) in compliance with the articles 1 and 2 of the Code of civil procedure belong to the parties. The classification and the regime of the procedural objections have to take it into account. It is thus necessary to distinguish the procedural objections on the contrary to the opening of the authority of those who are set against its continuation. Only the first ones have to be a matter of the exclusive competence of the judge of the enabled of whom we have to manage to make a real judge of the introduction of the instance. The procedural objections opposed to the continuation, on the other hand, must be able to be suggested according to their emergence or to their revelation except the possibility for the judge ruling out them or pronouncing pecuniary condemnations against the party which would have abstained in a delaying or unfair intention to raise them earlier.

Previous issue date: 2007 / The research contemplates the dilemma faced by small rural communities producers¿ in north-eastern Brazil maintaining last longing relationships. Due to difficult social conditions in distant regions, small producers utilize practices of commercial exclusivity to obtain sustainability. In the Lago do Junco community case, located in the state of Maranhão, the babaçu oil exports¿, the social environmental appeal guarantee the cluster participants sustainability and the representation of women in society. But social problems related to the lack of formal education e the absence of relationship with local aid organizations causes the non compliance with market demands, leading the cluster to a non-innovative trap. In the honey community case in Simplício Mendes, located in the state of Piauí, the closest relationship with the external aid agents increased the conversion of market demands in reality. The analysis reveal possibilities on obtaining economic sustainability for small producers thru the conscientious utilization of nature resources guaranteeing the harmony between the environmental preservation and the development of both activities, apiculture in Piauí and babaçu oil processing in Maranhão. / A dissertação Arranjos Produtivos Locais e Melhorias: Lições de Dois Casos Nordestinos retrata a dificuldade enfrentada por comunidades agrícolas de pequemos produtores no Nordeste brasileiro na manutenção de relações comerciais duradouras e na implementação de melhorias. Devido às condições sociais desfavoráveis de regiões ermas, pequenos produtores se utilizam de práticas de exclusividade comercial para atingir a subsistência. No caso da comunidade do Lago do Junco, no Maranhão, a exportação de óleo de babaçu, sob o apelo socioambiental, garante a subsistência dos integrantes do arranjo e a inserção da mulher na sociedade. Mas, problemas relacionados ao baixo nível de escolaridade e a ausência de relacionamento com organizações de apoio impedem a conversão de demandas de mercado em realidade, colocando o arranjo em armadilha não-inovadora. No caso da comunidade apícola de Simplício Mendes, no Piauí, o melhor aproveitamento de relacionamentos com órgãos de apoio local proporciona maior facilidade na conversão de demandas de mercado em inovações. A análise, com foco em arranjos produtivos locais, revela também que existe possibilidade para a obtenção de meios de sustentabilidade econômica de pequenas comunidades produtivas, através da utilização consciente dos recursos oferecidos pela natureza, garantindo a harmonia entre a preservação do meio ambiente e o desenvolvimento de ambas as atividades, da apicultura no Piauí e da extração e processamento de coco babaçu no Maranhão.

Exclusive breastfeeding is one of the infant feeding methods recommended by the World
Health Organisation (WHO) in the prevention of mother-to-child transmission of Human
Immunodeficiency Virus (HIV), especially to HIV positive women of low socio economic
status. The purpose of the study was to determine and describe the HIV positive
mothers’ perceptions towards exclusive breastfeeding in postnatal ward at a particular
hospital in Limpopo Province, South Africa.
Qualitative exploratory research design was used to explore and describe the HIV positive
mothers’ perceptions towards exclusive breastfeeding. A grand tour question in an
unstructured interview was used to collect data, followed by probing questions according
to participants’ responses. Data were collected from 15 HIV positive mothers aged 18
years and above. They opted for exclusive breastfeeding, and voice recorder and field
notes were used to capture data. The seven steps of data analysis, adapted from Colaizzi
were used. The findings: Although the benefits of exclusive breastfeeding was known by
mothers, some participants had limited knowledge on the importance of exclusive
breastfeeding. Some health care providers lack up-to-date information on exclusive
breastfeeding. Religious and cultural practices interfere with exclusive breastfeeding.

Der vorliegende Beitrag untersucht das Dreiecksverhältnis zwischen ökologischer Nachhaltigkeit, wirtschaftlichem Wachstum und liberaler Demokratie und stellt fest, dass dieses sich in den westlichen Konsumgesellschaften im Angesicht der zunehmenden lebensweltlichen Manifestation der "Grenzen des Wachstums" grundlegend neu konfiguriert. Dabei zeichnen sich tatsächlich die Konturen einer "demokratischen Postwachstumsgesellschaft" ab - allerdings unter gänzlich anderen Vorzeichen, als es der überwiegende Teil der Transformationsforschung ersehnt. In der Absicht, einen Beitrag zur Rückkopplung der normativen und sich als transformativ verstehenden Teile der Nachhaltigkeitsforschung an die sozialwissenschaftliche Analyse moderner Gesellschaften zu leisten, zeigt der Beitrag zunächst, wie im Bereich umweltpolitischer und demokratiebezogener Diskurse zentrale Narrative, die über Jahrzehnte die Debatte bestimmt haben, heute ihre Glaubwürdigkeit verlieren und sich damit das Feld für eine Neujustierung der Dreiecksbeziehung eröffnet. Anschließend wird aus gesellschaftstheoretischer Perspektive ausgeleuchtet, wie die Demokratie im Zeichen moderner Subjektivitätsverständnisse und bestenfalls noch moderater Wachstumsraten zunehmend zu einem Mittel der "nachhaltigen" Verteidigung nicht-nachhaltiger Lebensstile wird. To determine the relationship between infant-feeding practices and diarrhoeal
infections, a descriptive survey was conducted to infants between six to 12 months
of age.
A guided interview was conducted to 105 mothers of infants who attended the health
facilities of Mbabane, Swaziland.
The results show that breast-feeding is routinely practiced by the majority of mothers
and exclusive breast-feeding is very low, but supplementary feed in the form of
formula or solids are introduced by the majority of respondents within the first three
months of life. Infants who were given colostrum and breast milk had fewer
diarrhoeal attacks. Other factors, for example education and cultural factors
influenced the feeding practices and number of diarrhoeal attacks.
It is recommended that breast-feeding should be promoted as an important

Apesar de existirem evidências suficientes sobre benefícios do aleitamento materno (AM), apenas 35% das crianças são amamentadas exclusivamente até o quarto mês de vida. Visando estender esta prática, OMS/UNICEF lançaram a Iniciativa Hospital Amigo da
Criança (IHAC), que estabelece sistema de credenciamento para maternidades de acordo com
grau de incentivo ao AM. Esta dissertação pretende investigar a efetividade da IHAC na duração de dois tipos de aleitamento materno: exclusivo (AME) e predominante (AMP) entre crianças usuárias de Unidades Básicas de Saúde (UBS) do Rio de Janeiro. Trata-se de estudo transversal, sendo a população de estudo constituída de 811 mães de crianças menores de 5 meses de idade, selecionadas aleatoriamente em cinco UBS na cidade do Rio de Janeiro. A
variável de exposição foi categorizada em local de nascimento ocorridos em HAC, naqueles
em vias de receber titulação (EVHAC) e naqueles sem titulação. Os desfechos considerados foram duração do AME e do AMP, que inclui também crianças em AME (AMEP). Na análise dos dados, optou-se pelo modelo log-log complementar, que permitiu recompor experiência longitudinal da coorte através do recordatório alimentar de 7 dias e da informação da idade das mesmas, caracterizando abordagem tipo current status data. Mesmo após controle por
variáveis sociodemográficas, relativas ao estilo de vida e aos aspectos psicossociais maternos, à utilização dos serviços de saúde, idade e saúde da criança, houve maior duração do AME e AMEP em crianças nascidas em HAC e EVHAC. As taxas de AME e de AMEP são mais de
duas vezes maiores entre recém-nascidos que nasceram em HAC e EVHAC. Tal efeito diminui ao longo da idade da criança, mantendo-se evidente até quatro (EVHAC) e dois (HAC) meses de vida quando se considera AME e até dois (EVHAC) e cinco (HAC) meses quando se considera AMEP. Os resultados confirmam a efetividade da IHAC nesta clientela,especialmente na manutenção de AME e AMEP nos primeiros meses de vida. throughout interviews of 811 mothers of children under 5 months old, randomly selected at five health centers in Rio de Janeiro. Exposure variable was classified according to hospitals compliance with the Ten Steps. There were three possible status: accredited hospitals (BFH), working in becoming BFH or certified hospitals (CBFH) and hospitals without BFHI accreditation or certification. Outcomes were EBF and EPBF duration. Data was analyzed by complementary log-log transformation models, which allowed capture cohort longitudinal experience through 7-day feeding recordatory and infants age (current status data). Even after adjusting analysis for sociodemographic, life style and psychological maternal factors, health services use and babies age and state of health, there was longer duration of EBF and EPBF of infants born in BFH and CBFH. EBF and EPBF rates were twice higher in newborns born in BFH and CBFH. This protective effect on EBF and EPBF decreases along childs age and its noticed until four (CBFH) and two (BFH) months of age when EBF is considered and until two (CBFH) and five (EBF) months for EPBF. The finding indicates the effectiveness of BFHI in maintaining EBF and EPBF through the first months of life in this population. In
order to extend BF duration until six months of life, as recommended by WHO, it would be necessary not only to strengthen the BFHI but also to develop and encourage more actions in favor of breastfeeding, focusing on primary health care facilities

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Fundação de Amparo à Pesquisa do Estado do Rio de Janeiro / Apesar de ser a primeira profissão da área social a conquistar regulamentação federal no Brasil, o Serviço Social, ainda hoje, encontra dificuldades na definição das suas competências e, principalmente, das suas atribuições privativas. A partir da análise da Lei 8662/93, bem como do processo legislativo que culminou na sua aprovação no Congresso Nacional, esse trabalho dissertativo busca desvelar, fundamentado nos pressupostos teóricos da sociologia das profissões, as conexões existentes entre a estrutura legal da profissão e a indefinição das tarefas específicas dos assistentes sociais, estas já sinalizadas por pesquisas da área e pelo CFESS. Para alcançar os objetivos propostos, também são analisadas e contrapostas as produções de importantes autores do Serviço Social que se dedicaram a refletir sobre os temas relacionados à legislação profissional e ao significado social da profissão, o que aponta para a construção de mais incertezas do que elucidações sobre o assunto. / Despite of being the first profession of social area to win federal regulation in Brazil, the social work still faces difficulties in defining their tasks, and especial its exclusives competences. From the analysis of Law 8662/93, as well as the legislative process that culminated in its adoption in Congress, this dissertation seeks to reveal, based on the conceptual framework of the sociology of professions, the connections between the professional legislation and the uncertainties in definition the specific tasks of the social workers, these already identified by the research from the CFESS. To achieve the proposed objectives are also analyzed and contrasted the productions of important authors of Social Work who have dedicated themselves to reflect on subjects related to professional legislation and the social significance of the profession, which points to the construction of more uncertainty than elucidations about the subject.
