Numerical and experimental studies on the mechanical behaviour of the distal femur following total knee arthroplasty

The history of total knee arthroplasty stretches back over 70 years. Many studies have shown that TKA is, in general, a successful operation for the relief of joint pain, with patient satisfaction rates of 90-95% and implant survival rates at 10-15 years of greater than 90%. However, a number of studies have also shown the potential for failures or complications arising post-implantation leading to revision surgery. This thesis presents finite element (FE) models of the distal femur following primary and revision total knee arthroplasty. Pre-arthroplasty models are also developed for comparison. Particular attention is given to how femoral component design and method of fixation impacts the mechanical environment of the distal femur and stability of the prosthesis. FE analyses with fully bonded interfaces indicate that femoral components are subject to areas of low stress (stress shielding) immediately under the anterior flange and chamfer regardless of internal implant features. However, internal implant features were found to play a role in the pattern and magnitude of stress concentrations. Both stresses and motions were observed to increase with increasing flexion angle, indicating the importance of testing at multiple angles. The initial models of the distal femur were extended to incorporate the effects of ageing and endosteal thinning of the femoral cortex, through novel application of pre-existing FE modelling techniques, specifically the ability to assign variable material properties corresponding to the nodal temperatures output from a heat transfer analysis. The findings from this study indicate that older patients with osteoporosis may be at increased risk of periprosthetic fracture compared to younger healthy patients. The use of a revision femoral component with a cemented stem as a means to mitigate this fracture risk was also investigated. FE analyses using frictional interfaces were employed to determine the influence of femoral component design on micromotion at the interface. These models showed that all primary implants were subject to similar magnitudes of relative motion at the interface, however, the distinct internal implant features led to very different regional variations. Furthermore, certain internal implant features (i.e. femoral box) were found to be highly sensitive to errors in surgical bone cuts. This aspect of the thesis also concluded that the addition of a stem served to significantly reduce motions at the interface in comparison to primary stemless implants. Long stemmed prostheses were found to result in the smallest levels of interface motion. This study also detailed the design and creation of an in vitro test setup for the purposes of determining the influence of stem length and fixation on the stability of revision prostheses. Experimental results using this test rig showed that a cemented short stem provides as much initial stability as the uncemented long stem, and is easier to fit surgically. Corresponding FE models incorporating a virtual representation of the test rig and in vitro loading conditions revealed that the relative motion at the multi-planar bone-prosthesis interface cannot be adequately described using a single reference point. However, in vitro setups may be used to predict a general measure of implant stability and to provide a source of calibration for FE. The distal femur models were further modified to investigate how the presence of condylar defects as classified by AORI defect classification system (Engh 2006) and weak osseous support due to osteoporosis may adversely affect the survival of the prosthesis. These investigations revealed that fixation of the femoral component, the presence of a large condylar defect and the level of osseous support all had an impact on stress in the implant, it is concluded that a non-modular approach should be adopted in older patient groups with severe osteoporosis to mitigate the risk of component junction failure and distal femoral fracture.

The Significance of Microsaccades for Perception and Oculomotor Control

abstract: During attempted fixation, the eyes are not still but continue to produce so called "fixational eye movements", which include microsaccades, drift, and tremor. Microsaccades are thought to help prevent and restore vision loss during fixation, and to correct fixation errors, but how they contribute to these functions remains a matter of debate. This dissertation presents the results of four experiments conducted to address current controversies concerning the role of microsaccades in visibility and oculomotor control. The first two experiments set out to correlate microsaccade production with the visibility of foveal and peripheral targets of varied spatial frequencies, during attempted fixation. The results indicate that microsaccades restore the visibility of both peripheral targets and targets presented entirely within the fovea, as a function of their spatial frequency characteristics. The last two experiments set out to determine the role of microsaccades and drifts on the correction of gaze-position errors due to blinks in human and non-human primates, and to characterize microsaccades forming square-wave jerks (SWJs) in non-human primates. The results showed that microsaccades, but not drifts, correct gaze-position errors due to blinks, and that SWJ production and dynamic properties are equivalent in human and non-human primates. Development and Validation of a Novel Biomechanical Testing Setup and Procedure for Olecranon Fracture Fixation Assessment

abstract: Olecranon fractures account for approximately 10% of upper extremity fractures and 95% of them require surgical fixation. Most of the clinical, retrospective and biomechanical studies have supported plate fixation over other surgical fixation techniques since plates have demonstrated low incidence of reoperation, high fixation stability and resumption of activities of daily living (ADL) earlier. Thus far, biomechanical studies have been helpful in evaluating and comparing different plate fixation constructs based on fracture stability. However, they have not provided information that can be used to design rehabilitation protocols such as information that relates load at the hand with tendon tension or load at the interface between the plate and the bone. The set-ups used in biomechanical studies have included simple mechanical testing machines that either measured construct stiffness by cyclic loading the specimens or construct strength by performing ramp load until failure. Some biomechanical studies attempted to simulate tendon tension but the in-vivo tension applied to the tendon remains unknown. In this study, a novel procedure to test the olecranon fracture fixation using modern olecranon plates was developed to improve the biomechanical understanding of failures and to help determine the weights that can be safely lifted and the range of motion (ROM) that should be performed during rehabilitation procedures. Design objectives were defined based on surgeon's feedback and analysis of unmet needs in the area of biomechanical testing. Four pilot cadaveric specimens were prepared to run on an upper extremity feedback controller and the set-up was validated based on the design objectives. Cadaveric specimen preparation included a series of steps such as dissection, suturing and potting that were standardized and improved iteratively after pilot testing. Additionally, a fracture and plating protocol was developed and fixture lengths were standardized based on anthropometric data. Results from the early pilot studies indicated shortcomings in the design, which was then iteratively refined for the subsequent studies. The final pilot study demonstrated that all of the design objectives were met. This system is planned for use in future studies that will assess olecranon fracture fixation and that will investigate the safety of rehabilitation protocols. / Dissertation/Thesis / Masters Thesis Bioengineering 2015

Objetivou-se com esse estudo avaliar o bloqueio paravertebral cervical do plexo braquial em cães com ropivacaína 1% comparando-se duas técnicas de localização das raízes nervosas: a palpação de referências anatômicas específicas e a utilização de estimulador de nervos periféricos. Foram empregados 24 cães submetidos à osteossíntese radio-ulnar sob anestesia geral inalatória com isofluorano, divididos aleatoriamente em quatro grupos distintos de acordo com os tratamentos: TI - SP: solução salina 0,9% perineural utilizando as referências anatômicas para localização das raízes nervosas cervicais; TII - SN: solução salina 0,9% utilizando as referências anatômicas e estimulador de nervos periféricos; TIII - RP: ropivacaína 1% perineural utilizando apenas referências anatômicas para localização das raízes nervosas cervicais; e TIV - RN: ropivacaína 1% utilizando as referências anatômicas e estimulador de nervos periféricos dessas raízes. Foram monitoradas as frequências cardíaca (FC) e respiratória (f), as pressões arteriais sistólica (PAS), média (PAM) e diastólica (PAD), a temperatura esofágica (Tº), a pressão parcial de dióxido de carbono (ETCO2) e a concentração de isofluorano (ETISO) ao final da expiração... / Animals are capable of feeling pain and this influences the quality of anesthesia recovery and outcome following surgery. The peripheral nerve blocks are increasingly used to prevent and relieve pain, and cervical paravertebral approach to the brachial plexus (roots C6 , C7, C8 and T1) is an alternative to anesthesia of the canine forelimb. The local anesthesics ropivacaine is highly soluble, long-acting and produces reliable analgesia with minimal motor block. The objectvice of this study was to evaluate the cervical paravertebral block of the brachial plexus with ropivacaine 1% and compare two techniques for localization of the nerve roots: palpation of anatomical landmarks and use of peripheral nerve stimulator. It was use 24 animals submitted to radio-ulnar osteosynthesis, randomly divided into four distinct groups according to treatment: TI - SP: perineural administration of 0,9% saline solution using anatomical landmarks to localization of the cervical nerve roots; TII - SN: perineural administration of 0,9% saline solution using anatomical landmarks and peripheral nerve stimulator; TIII - RP: ropivacaine 1% perineural administration using only anatomical land marks; and TIV - RN: ropivacaine 1% administration using anatomical landmarks and peripheral nerve stimulator to localization of the nerve roots. It was monitored heart rate (HR), respiratory rate (FR), systolic (SAP), mean (MAP) and diastolic arterial pressure (DAP), body temperature (T) and expired CO2 (ETCO2)) and isoflurane (ETISO) concentrarion. Increase in HR and/or SAP and/or MAP in 20% of baseline were characterized as the need for “rescue” analgesia with fentanyl (5 μg/kg - IV). O presente estudo avaliou e comparou a osseointegração de implantes cilíndricos de superfície porosa com implantes cilíndricos de superfície rugosa, ambos de titânio, em ratas osteopênicas e normais, por meio de análise histológica e histomorfométrica. Avaliou também a influência do tratamento com laser de baixa potência (Arseneto de Gálio Alumínio) sobre a osseointegração. Os implantes foram inseridos em 84 ratas, cada rata recebeu um implante poroso no fêmur esquerdo e um implante rugoso no direito. Quarenta e dois dos animais foram submetidos à cirurgia de ovariectomia (OVZ) e 42 foram falsamente operados (SHAM). Após um mês, as ratas foram divididas em 4 grupos, com 21 animais cada, de acordo com o tratamento recebido: a) grupo 1 (SHAM): cirurgia de colocação dos implantes (CI); b) grupo 2 (SHAM): CI e aplicação do laser de baixa potência (LLLT); c) grupo 3 (OVZ): CI; d) grupo 4 (OVZ): CI e LLLT. Sete animais de cada grupo, selecionados aleatoriamente, foram sacrificados 2, 4 e 6 semanas após a cirurgia, os fêmures contendo os implantes foram submetidos à análise histológica e histomorfométrica. Na análise histológica observou-se que houve osseointegração em ambos implantes nos diferentes grupos. Após avaliação da análise histomorfométrica verificou-se que o grupo 4, os implantes do tipo poroso, com animais sacrificados 4 semanas após a cirurgia de implante exibiu maior porcentagem de neoformação óssea, seguido do grupo 2, com implante poroso 4 e 6 semanas de período de sacrifício. Concluiu-se que a laserterapia melhora significantemente a neoformação óssea e que os implantes de superfície porosa apresentam maior área para contato na interface osso-implante. / With increasing life expectancy of the population is important to the study of alternative treatments and biomaterials which aim to assist in the process of osseointegration in people with osteoporosis. .This study involved an assessment and comparison of the osseointegration of cylindrical titanium implants with porous surfaces and with rough surfaces in osteopenic rats, based histological and histomorphometric analyses. An evaluation was also made of the influence of low level laser therapy (LLLT) with gallium-aluminum arsenate (Ga-Al-As) on osseointegration. The implants were placed in 84 rats, two per rat, i.e., a porous implant in the left femur and a rough implant in the right femur Forty-two of the animals were then ovariectomized (OVZ) while the other 42 were subjected to a sham ovariectomy (SHAM). After one month the rats were divided randomly into four groups of 21 animals each, according to treatment received: a) group 1 (SHAM) implant surgery (IS); b) group 2 (SHAM) IS and low-level laser therapy (LLLT); c) group 3 (OVZ): IS; d) group 4 (OVZ) IS and LLLT. Seven animals from each group were selected randomly and killed 2, 4 and 6 weeks after surgery and the histomorphometric analyses. The histological analysis revealed that osseointegration occurred in both types of implants in all the groups. The histomorphometric analysis indicated that the porous implants in the group 4, killed 4 weeks after implantation, showed the highest percentage of new bone formation, followed by the porous implants in group 2, killed 4 and 6 weeks after implantation. It was concluded that laser therapy significantly improves new bone formation and that implants with porous surfaces have a larger contact area at the bone-implant interface.

Estudo morfométrico da autólise acinar em glândulas sublinguais de ratos: sua relação com intervalo post mortem e o volume do fixador / Morphometric study of acinar autolysis in sublingual glands of rats: it\'s relation with interval post mortem and the formalin volume post mortem

A autólise acinar post mortem em glândulas sublinguais humanas é um fenômeno que prejudica a sua análise microscópica. Com o objetivo de esclarecer e prevenir tal ocorrência, a presente investigação foi planejada no sentido de analisar morfometricamente as possíveis influências do intervalo post mortem (IPM) e do volume de fixador histológico (VF) na ocorrência de autólise de ácinos em glândulas sublinguais de ratos. Dos sessenta animais utilizados na investigação, cinqüenta deles o foram no estudo do intervalo post mortem, sendo divididos em 2 grupos: o grupo I (25 animais) foi destinado para as avaliações morfométricas e o grupo II (25 animais) para determinação do fator de retração e densidade da glândula. Os grupos I e II foram subdivididos nos subgrupos: A e A1 (controle - 0 hora), B e B1 (3 horas post mortem), C e C1 (6 horas), D e D1 (12 horas) e E e E1 (24 horas), com 5 animais em cada um. A fixação foi realizada com 20 mL de solução de formol a 10% em tampão fosfato. Os 10 animais remanescentes foram destinados ao estudo da variação de volume do fixador, e foram divididos em 2 grupos iguais: no grupo 2mL, as glândulas dos 5 animais foram fixadas com 2 mL de solução de formol a 10% tamponada, e no grupo 20mL, as glândulas dos outros 5 animais foram fixadas com 20 mL da mesma solução. O tempo de fixação foi de 7 dias para todos. As glândulas foram processadas histologicamente, sendo os cortes histológicos corados com H.E. A análise morfométrica foi realizada em 50 campos histológicos por glândula, selecionados por amostragem sistemática, usando objetiva de 100x e ocular Kpl 8x contendo um retículo de integração constituído por 100 pontos simetricamente distribuídos. A densidade de volume dos ácinos íntegros e autolisados foi avaliada pelo método morfométrico de volumetria relativa de contagem de pontos. Houve diferença estatisticamente significante entre o IPM e a autólise acinar (p< 0,05), enquanto que não houve diferença significante quanto ao VF (p= 0,690). A autólise acinar aumentou significantemente com o aumento do período post mortem (p<0,05). Baseado nos resultados obtidos foi possível concluir que a autólise acinar em glândulas sublinguais de ratos está diretamente relacionada ao intervalo post mortem, não sendo influenciada pelo volume de fixador histológico testado no experimento. / Acinar post mortem autolysis is a phenomenon that difficult the microscopic analysis in human sublingual glands. The aim of the present study is to evaluate the influence of the post mortem interval (PMI) and formalin volume (FV) in the occurrence of acinar autolysis in sublingual glands of rats. Sixty animals were used in this study. Out of them fifty animals were divided in 2 groups for PMI investigation: group I (25 animals) for morphometric quantifications and group II (25 animals) to calculate the retraction factor and the density of the glands. The groups I and II were subdivided in subgroups with 5 animals each: A and A1 (control - 0 hour), B and B1 (3 hours post mortem), C and C1 (6 hours), D and D1 (12 hours) and E and E1 (24 hours). The remaining 10 animals were used for the FV study and were divided in two groups with different volume of formalin, 2mL and 20mL respectively. The fixation period was 7 days. The glands were processed and stained with HE. The morphometric analysis was performed in 50 histological fields, selected by systematic sampling, using lens of 100x and ocular Kpl 8x containing a Zeiss II integration grid with 100 points symmetrically distributed. The volume density of intact and autolysed acini was evaluated by the morphometric method of relative volume of counting of points. There was a statiscally significant difference between volume density acinar autolysis and PMI for all group tested (p=0,0001). The difference was not significant for FV (p = 0,690). We concluded that acinar autolysis in rat sublingual glands increased significantly with the PMI, not being influenced by the FV, as tested.

Foram incluídos, retrospectivamente, 57 pacientes, consecutivos, tratados com a placa em onda com técnica de osteossíntese biológica, com seguimento mínimo de um ano de pós-operatório. Obteve-se índice de consolidação de 94,74% (54 em 57 pacientes) em tempo médio de 12,88 semanas. Conclui-se que o método pode ser considerado uma alternativa às hastes intramedulares, com índices de consolidação semelhantes / Abstract: Mechanical studies with wave plates, were a higher rigidity was shown in models with cortical contact, suggested a study to analyze the use of wave plate in femoral simple shaft fractures. A consecutive series of 57 patients treated with wave plates and biological internal fixation, and a minimal follow up of 1 year after surgery. Healing occurred in 94,74% (54 of 57 patients) with an average of 12 weeks. Fo-ram desenvolvidas 10 placas em Y para cada espessura, contendo 8 furos. As placas foram confeccionadas de acordo com projeto idealizado pelo autor e fabricado pela empresa Toride Indústria e Comércio Ltda., Mogi Mirim, SP, Brasil. As placas foram confeccionadas com titânio comercialmente puro (grau ll), conforme norma ASTM F-67. Os processos para desenvolvimento das placas foram feitos em programa de desenho mecânico Solid Works. A confecção das placas seguiu a metodologia de fabricação já existente na empresa, compreendendo processo de usinagem em equipamento modelo Discovery Romi de 4 eixos, o qual mantém precisão e controle dimensional com tolerância de ± 0,10 mm. Foram também confeccionadas 30 hemi-mandíbulas do lado esquerdo, com resina de poliuretano rígido de densidade 40 a 50 PCF onde as placas foram fixadas para conter fratura simulada da cabeça do côndilo. O teste de carregamento foi executado na máquina para ensaio universal Instron, modelo 4411, aplicando carga na posição médio-lateral e ântero-posterior na cabeça do côndilo. ANOVA com 2 fatores e teste de Tukey com 5% de significância foram aplicados nos resultados. Quando a carga foi aplicada no sentido médio-lateral, a placa de maior espessura (1,5 mm) obteve a maior resistência, mas sem diferença estatística entre os deslocamentos de 5,0 e 10,0 mm. No sentido ântero-posterior, a placa com maior resistência foi a de menor espessura, seguida pela de maior espessura. Pode-se concluir que: 1 - No sentido médio-lateral, os maiores valores de resistência foram observados no deslocamento de 15 mm, exceto para as placas 0,6 e 1,5 mm com valores similares em 10 e 20 mm respectivamente; 2 - A placa com espessura de 1,5 mm foi a única que apresentou maior valor médio em todos os deslocamentos; 3 - No sentido ântero-posterior, os maiores valores de resistência foram vistos no deslocamento de 15 mm e os menores em 5 mm para todas as espessuras de placas; 4 - A placa com espessura de 0,6 mm foi a que apresentou maiores valores em todos os deslocamentos e 5 - No sentido ântero-posterior, o carregamento de 20 mm não foi alcançado / Abstract: The purpose of this study was to develop a Y plate with thicknesses of 0.6, 1.0 and 1.5 mm for use in condylar mandible fracture and submit them to the me-chanical strength testing Compression. For this study 30 plates were used for surgical containment internal, Y geometry and holes 8, 10 plates with a thickness of 0.6 10 mm plates with 1.0 mm thickness and 10 plates with thickness of 1.5 mm. This model plate was designed by the author and manufactured by Toride Industry e Comercio Ltda., Mogi Mirim, Brazil. The plates were made from commercially pure titanium (grade II), according to ASTM F-67. The processes for developing the plates were made in program Solid Works mechanical design engineering company by Toride, following the procedures of the project. The making of the plates followed the methodology in an existing manufacturing company, including process equipment machining model Romi Discovery of 04 axes, which keeps ac-curacy and dimensional control with a tolerance of ± 0.10 mm, required by the quality system of the company Toride. Durante a sua utilização eventualmente o cirurgião depara-se com intercorrências cirúrgicas como quebra da placa durante a dobragem ou quebra de parafusos durante sua inserção e isso de fato ocorre na prática clínica. O objetivo do autor neste estudo foi avaliar se o processo repetido de autoclavagens influencia nas características de dureza e microscópicas das placas e parafusos do sistema 2,4mm "locking". A amostra foi dividas em placas: grupo I (0) controle, grupo II submetido a 20 processos de autoclavagem e grupo III submetido a 60 processos de autoclavagem. Os parafusos foram divididos da mesma forma: grupo I (0), grupo II (20) e grupo III (60). Após o preparo do corpo de provas foram realizados os testes de dureza e avaliação em microscopia da microestrutura. Encontrou-se uma média de valores de dureza de 180,2; 183,8 e 171,8 Vickers respectivamente para os grupos I, II e III das placas (p=0,497) e os valores médios de 328,8; 340,8 e 355,3 Vickers respectivamente para os grupos I, II e III dos parafusos (p=0,283). Na análise microscópica tanto dos parafusos quanto das placas observou-se imagens em 500x que revelaram a microestrutura do material, com grânulos em conformações diferentes, de acordo com a margem analisada, observam-se ainda formação de maclas, principalmente na margem interna, o que pode ser decorrente de força de pressão/cisalhamento durante processo de usinagem, mas sem diferenças entre os grupos. Baseado nos resultados obtidos nas amostras estudadas, observou-se que o processo repetido de autoclavagens não alterou significativamente as características de dureza e microscópicas do material analisado / Abstract: Stable internal fixation systems are used in bone surgery, either fractures or osteotomies, It can be submitted to multiple procedures sterilization procedures before implantation.
