Análise de recidiva após osteotomia Le Fort I para avanço maxilar em pacientes com fissura de lábio e palato / Recurrence analysis after Le Fort I osteotomy for maxillary advancement in digital cephalometric and digitized models of patients with cleft lip and palate

O presente estudo observou e avaliou retrospectivamente o índice de recidiva após cirurgia ortognática para avanço maxilar em pacientes com fissura de lábio e palato. A amostra foi composta por telerradiografias laterais analisadas através de traçados cefalométricos e imagens digitais tridimensionais de modelos de estudo de 17 pacientes de ambos os sexos (10 pacientes do sexo feminino e 07 pacientes do sexo masculino), com idade entre 17 e 33 anos. Os traçados cefalométricos digitais foram avaliadas em: T1 pré-operatório; T2 pós-operatório imediato e T3 pós-operatório de seis meses a 1 ano. A análise cefalométrica digital determinou as medidas de forma linear para possíveis mudanças verticais e horizontais no pós-operatório da cirurgia ortognática para avanço maxilar. Os modelos de estudo digitalizados foram avaliados nas seguintes fases: F1 préoperatório; F2 pós-operatório de seis meses a 1 ano e F3 pós operatório de 1 até 2 anos, nos planos frontal e lateral. Verificou-se ainda nos modelos digitalizados, a linha média dentária, trespasse vertical (plano frontal) e a relação pré-molar nos lados direito e esquerdo e trespasse horizontal (plano lateral). As análises dos arcos dentários foram realizadas diretamente nas imagens escaneadas, por meio do Scanner 3Shapes R700TM e avaliadas pelo 3D Software OrthoAnalyzerTM. Um avaliador previamente calibrado e treinado fez as avaliações. A análise estatística descritiva foi realizada com valores de média, desvio-padrão e porcentagem. O teste ANOVA de medidas repetidas foi aplicado nas variáveis estudadas (movimento vertical, movimento horizontal, trespasse horizontal, trespasse vertical e linha média dentária) para comparar os três tempos determinantes. Como verificou-se significância estatística nestas diferenças, o teste Tukey foi aplicado para avaliar os grupos entre si. O nível de significância de 5% foi adotado para todas as análises. Os resultados mostraram que na cefalometria digital o movimento vertical apresentou diferença estatisticamente significativa entre T2 e T3 (p=0,002). A relação pré-molar dos lados direito e esquerdo revelou que os pacientes apresentavam em F2, ¼ classe II e classe I, 29,4% e 23,5%, e em F3 classe I, 58,8% e 70,6%, respectivamente Donde conclui-se que houve recidiva na cefalometria quanto ao movimento vertical, após cirurgia ortognática para avanço maxilar, sem recidiva nos demais parâmetros avaliado / This observational study evaluated retrospectively forms the recurrence rate after orthognathic surgery for maxillary advancement in patients with cleft lip and palate. The sample consisted of radiographs with lateral cephalometric tracing and three-dimensional digital images of 17 patients study models of both genders (10 female patients and 07 male patients), aged between 17 and 33 years. The cephalometries were evaluated: T1 preoperative; T2 - immediate postoperative period and T3 - postoperative six months to 1 year. The digital cephalometric analysis determined the measures linearly for possible vertical and horizontal changes in the postoperative period of orthognathic surgery for maxillary advancement. Study models, scanned, were evaluated in the following phases: F1 - preoperative; F2 - postoperative six months to 1 year and F3 - postoperative 1 to 2 years, in the frontal and lateral planes. It was also found on scanned models, the dental midline, overbite, overjet (frontal plane) and pre-molar relationship, on the right and left sides (lateral plane). The analysis of dental arches were made directly on the scanned images through the scanner 3Shape\'s R700TM and evaluated by 3D Software OrthoAnalyzerTM. A calibrated and trained evaluator evaluations. Descriptive statistical analysis was performed with mean values, standard deviation and percentage. The ANOVA test for repeated measures was applied to the variables (vertical movement, horizontal movement, overjet, overbite and dental midline) to compare the times. As there was statistical significance in these differences, the Tukey test was used to evaluate the groups together. The 5% significance level was adopted for all analyzes. The results showed that the cephalometry vertical movement showed a statistically significant difference between T2 and T3 (p=0.002). The premolar ratio of the right and left sides showed that patients had in F2, ¼ class II and class I, 29.4%, and 23.5%, and F3 class I 58.8% and 70.6%, respectively. It is concluded that there was recurrence in cephalometrics as the vertical movement after orthognathic surgery for maxillary advancement, without recurrence in the remaining parameters

Neste estudo retrospectivo, 80 telerradiografias obtidas em norma lateral de pacientes submeditos à cirurgia ortognática foram analisadas, sendo 40 obtidas no período pré-operatório e 40 no pós-operatório. A amostra foi dividida em 2 grupos de acordo com a sequência cirúrgica executada. No Grupo 1 constaram as radiografias de pacientes submetidos à cirurgia através da sequência convencional, enquanto no grupo 2 constaram as radiografias dos pacientes submetidos à cirurgia pela sequência invertida. Em ambos os grupos foi analisada posição final do incisivo central superior, tanto no sentido vertical quanto no ântero-posterior, sendo este resultado comparado com o que fora planejado, na busca da precisão cirúrgica. O resultado encontrado nos grupos 1 e 2 foram então confrontados na busca de diferenças entre as duas técnicas. Foi testada a hipótese nula de que não havia diferença entre os grupos analisados. Após aplicado o teste t de Welch para comparação das médias das diferenças entre o desejado e o obtido nos grupos 1 e 2, considerando significância estatística de 5% (alpha) e um teste bi-caudal, chegou-se a evidência estatística de que a hipótese não nula foi rejeitada dados os resultados obtidos das amostras (p>0,05). Sendo assim, não houve diferença no grau de precisão do posicionamento maxilar entre os grupos. Concluiu-se que tanto a sequência cirúrgica convencional quanto a sequência cirúrgica invertida mostraram ser técnicas confiáveis no posicionamento maxilar após osteotomia. tipo Le Fort I / Abstract: The objective of this study was to evaluate maxillary positioning accuracy of double jaw orthognathic surgery using conventional surgical sequence and inverted sequencing, ie, when the mandible was osteotomized and fixed prior to the jaw. In this retrospective study, 80 lateral radiographs obtained in patients that underwent orthognathic surgery were analyzed, 40 obtained in the preoperative and 40 in the postoperative period. The sample was divided into two groups according to the surgical sequence performed. Group 1 consisted of the radiographs of patients submitted to conventional surgical sequence, while group 2 consisted of the radiographs of patients that underwent surgery by the inverted sequencing. In both groups the final position of the maxillary central incisor was analyzed (in the vertical and anteroposterior planes), and this result was compared with what was planned in the pursuit of surgical precision. The results found in groups 1 and 2 were then compared in the search of differences between the two techniques. The null hypothesis, which stated that there was no difference between the groups, was tested. After applying the Welch t test for comparison of mean differences between the desired and obtained in groups 1 and 2, considering a statistical significance of 5% (alpha) and a two-tailed test, the null hypothesis was not rejected given the results of the samples (p> 0.05). Thus, there was no difference in the accuracy of maxillary positioning between the groups. A Public History Project Atblakeley Historic Park, Alabama

The public history project described herein was performed at Blakeley Historic Park, Alabama. The project included the preparation of survey maps depicting the location, size and directional orientation of Confederate and Union earthworks, which were used during the siege and battle of Fort Blakely in April 1865. The project also included historical research and documentation of findings relative to the design, construction and use of the Confederate fortifications at Blakeley Park. This research attempts to answer the questions; who ordered or directed the earthworks to be built, who designed them and supervised their construction, when were they built, and who provided the labor for their construction? Recommendations are made for the acquisition potential of earthworks found that were outside of present park boundaries. In addition, recommendations are made for preservation of existing earthworks within the park. The historical essay on the Confederate fortifications advances the argument that the design and construction effort was beset with shortages of engineers needed for design and supervision, shortages of labor needed for construction, and a shortage of troops to man the fortifications. Because this project combined modern day Global Positioning System surveying and Geographic Information System mapping technology with historical research methodology, collaboration with faculty experts in the College of Engineering and Computer Science was essential.

In 1988, Galle Fort in Sri Lanka was declared a world heritage site by UNESCO. The new historic-preservation regulatory setup that came with this designation displaced its inhabitants in their own homes. This thesis examines on how these inhabitants negotiate preservation regulations and the government’s effort to maintain a “world heritage” status based on their view. It focuses on how people adapt to the newly regulated environment and create spaces for their own daily practices. This study identifies that there are people who follow the government rules and transform themselves into subjects, while others break government rules and create the lived spaces they want. In-between are the majority of people who both become subjects of the space and also negotiate with the government rules to create their (negotiated) living environments. Although the government has more power, the people with less power have been transforming the space to meet their own needs and create a sense of place. / Department of Urban Planning

The purpose of this study was to determine whether teachers' perceptions of principals' promotion and support of technology in the Fort Wayne Community Schools, Fort Wayne, Indiana, effected the successful integration of technology into the elementary school curricula. The selection of variables for this study was based on a comprehensive review of the literature and identified practitioners who had educational computing experience. These variables included teacher perceptions of principal support, teacher participation in staff development opportunities, availability of support with computer-related problems, and computer availability. These variables were correlated with teachers' use of computers, students' use of computers, and teachers' perceptions of their own improvement as measures of technology integration.A 75-item researcher-developed survey instrument, based on these variables, was presented to 558 elementary classroom teachers in the 22 participating elementary schools in Fort Wayne, Indiana, on two scannable pages. Pearson product-moment correlations were calculated for the pairs of variables.This research provided evidence of a strong correlation between the FWCS teachers' perceptions of principal leadership and support of technology integration and the teachers' evaluations of their own improvement in integrating technology into thecurricula. It also supported the premise that the level of staff development that teachers engage in directly affects their level of improvement in integrating technology into the curricula. / Department of Educational Leadership
