Domácí hospice a paliativní péče / Home Hospices

In our diploma thesis "Home hospice and palliative care" we deal with home hospices and, to a lesser extent, inpatient hospices. We also discuss palliative care, futile treatment, euthanasia and previously expressed wishes. The work also deals with legislative laws or regulations regarding hospice palliative care and health care in general and ethics. The work mentions mobile hospice teams, palliative care counseling teams in hospitals, care for terminally ill patients. The importance of the presence of loved ones at the time of the patient's death, the lack of palliative beds in the Czech Republic and the little public awarness of hospice care, previsouly expressed wishes and other home hospices as an alternative to euthanasia are determined. The diploma thesis entitled: ?A work with a home hospice care client´s family? focuses on an issue associated with a dying at home. The care for a dying man is in such a case provided by family members with a help of a home hospice. The theoretical part clarifies an area connected with providing of hospice services and amodern concept of the hospice care in the Czech Republic as well as in the world. This chapter´s key is a clarification of an issue of the home hospice care and its characteristics. Further it contains topics: a family, a finality of life, phases of dying according to Kübler- Ross, a palliative care, social work in the hospice care. The aim of the thesis was to map a system of the work with a family caring for a patient in the home hospice care. To work out the diploma thesis I used a strategy of a qualitative research. I applied a method of an oral questioning through a technique of a narrative interview with the caring people. As a complementary method I used a semi-structured interview with a social worker and a nurse of the home hospice. Respondents were chosen among people caring for dying men who are patients of the Tabitha Home hospice in Brno. I realized interviews during the month of February until early April 2012. The interviews took place after a mutual agreement, at respondent´s home or in the home hospice rooms. The interviews with women-home hospice workers were carried out in the hospice rooms. 10 interviews with respondents were realized. Data processing was done by categorizing data. Results show that respondents are satisfied with services of the home hospice and they evaluate them positively. They appreciate an approach, a work of the home hospice staff and their respect for the dying as well as wishes of theirs. A support from the side of the home hospice is provided in material, technical, psychological and also spiritual forms. Despite demands of the care provided by the respondents, they would care for the dying at home again. The diploma thesis has been intended for professionals dealing with the given issue, but also for a lay public who wants to learn more about the home hospice services. The thesis should show benefits of this service.

The issue of death and dying affects everyone, it is not easy and yet a natural part of everyone's life. Hospice care offers help and support a dying man and his close on the last trip "home". This thesis describes the possibilities of a preparatory course for the volunteers who play an irreplaceable role in hospice care. The practical part describes the specific example from the conditions of the Czech home hospice - The Home Coming, which is being commented on the ground of literature and my personal experience. The outcome of the research is the feedback for the management of the home hospice - The Home Coming, including recommendations for changes of the preparatory course.

In this Diploma thesis, a 3D model and building plans of hospice building in Rajhrad was created. These products can be used by building management for planning reconstructions and annexes. In the introductory part of this work, informations about hospice building are presented. The second part is devoted to describing data collection for the model. The last largest part describes the modeling in AutoCAD 2014, creation of building plans and the accuracy testing of the final product.

The diploma thesis deals with the issue of financing of hospice palliative care in the Czech Republic. The aim of the thesis is to evaluate the economic demands of particular palliative care providers with a closer focus on hospice facilities and to propose recommendations for the change of legislation. The theoretical part defines the basic concepts of palliative and hospice care and describes the existing legislation and hospice care abroad. The analytical part focuses on the description of incomes and costs of inpatient and home hospices, financing of hospice care abroad and other aspects affecting the hospice’s economy. Based on the findings and qualitative research carried out, recommendations leading to a possible improvement of the hospice financing system are set.

This diploma thesis deals with practical ethical issues in specialized mobile palliative care. First of all the terms necessary for understanding the context are explained. Than brief introduction to the legal order and the principles of palliative care in the Czech Republic follows. Diploma thesis summarizes the phases of coping with the disease and the loss of a close person. This is both from adult's and a child's point of view. Next section deals with spiritual and pastoral care of dying, four principles of medical ethics and informed consent. Based on an interview with MUDr. Irena Závadová, supported by specialized literature, the ethical issues and theire possible solutions are mentioned too. Vývoj hospicové paliativní péče se zaměřením na Německo a Českou Republiku Development of hospice-palliative care with focus on Germany and the Czech Republic Ruth Kubíčková The aim of the thesis was to map the development, crucial moments and current .shape of hospice-palliative care and simultaneously confirm the thesis that the care of spiritual feature of dying persons has been an integral part of organized development of hospice care since its beginning and nowadays, it is still one of its principal themes. The first chapter deals with branch terminology since its clear definition is essential, not only for dialogue inside the system of palliative care providers and experts, but for interdisciplinary communication and cooperation which is - according to the definition of palliative care - the foundation of its holistic approach to patients. The second and third chapters are concerned with the development of nursing care since the ancient times to the diversification of modern hospice-palliative care at the end of the twentieth century. They deduce that the Christianity, with its central commandment to love thy neighbour as thyself, has changed the attitude to care of suffering persons, both in the nursing care and in shapes and ways of providing care.
