Title: The importance of myofeedback rehabilitation in faecal incontinence Introduction: Faecal incontinence is a serious symptom significantly worsening quality of life. Following results of international studies myofeedback has been considered an effective option of conservative treatment of this disease. Despite that it has so far been used at just a few centres in the Czech Republic. Purpose: The main purpose of this thesis was to assess the relevance of myofeedback as part of recovery therapy. Other objectives included assessment of use of a vaginal electrode instead of an anal one and comparison of the result correlations between examination by anal manometry, surface EMG and subjective improvement of continence for the patients. Methods: The research project included 8 females. The objective method of assessment of effects of the treatment involved examination of pelvis bottom muscles with the help of anorectal manometry and surface EMG. Success of the therapy was evaluated through the St Marks incontinence score and by the test population themselves filling out a quality of life questionnaire. Results: In the course of three-month recovery most patients experienced improvement of subjective as well as objective parameters. Urinary incontinence is defined as the involuntary escape of urine that is objectively demonstrable and causes social and hygienic problems. It is a common problem, that is in its results also important medical, social and economical defect. The object of the research of this work was a group of woman patients with the stress form of urinary incontinence, that was instruated by Kegel's or Mojžíšová's methods in home exercises. The goal of this thesis was to influence contractility, power and tense of pelvic floor muscles and to evaluate the effect of the chosen methods. Objectively the results of the work don't demonstrate change of assessed parameters, but it has came to subjective improvement of urinary incontinence's symptoms and differences in kineziological investigation. Title: An examination of pelvic floor activity following the application of special exercises Objectives: The aim of this thesis is to validate the usefulness of carrying out local pelvic floor muscle examinations as an indication and education for therapy. A second objective is for the women involved in the research to subjectively evaluate the examination methods and therapy. Furthermore, the thesis aims to establish an example of a comprehensive and detailed local examination of the pelvic floor muscles. Methods: We measured the pelvic floor activity per vaginam using an EMG biofeedback device on clients with a pelvic floor dysfunction and grade I stress incontinence symptoms. Based on the results of the examination, therapy using the vaginal device was recommended and in six weeks a follow-up examination was carried out. The changes in pelvic floor activity were evaluated by comparing the initial and follow-up examinations. Thus the usefulness of carrying out an examination of the local pelvic floor muscles before indicating a therapy was validated. At the end of the examination an interview with participating clients was conducted in order to subjectively evaluate the method. Možnosti edukace v prevenci vzniku inkontinence po operacích prostaty / The possibilities of education in the prevention of incontinence after prostate surgery

The goal of the work is to ensure a safe feeling for the patient in preoperative preparation so that it can cooperate more actively in the prevention of incontinence in the postoperative period. Another goal of the work is to create an educational material intended for patients with planned prostate surgery. The research was carried out by a qualitative method using questioning in the form of interviews. One group of patients was educated according to the custom of the department and the other group was edited using a brochure. When searching for the respondents, patients were approached by the patients of the Písek hospital. Data analysis was carried out by the hand-coded method, also referred to as the paper and pencil method. The results can be divided into four areas according to which research question they apply. The first part of the results provided information that if the patient is subjectively well informed, we will ensure active co-operation in the prevention of incontinence in the postoperative period. In the second question, we investigated whether the lack of information has an impact on the feeling of safety and security. This has also been confirmed. The third area gave the answer to whether the patient is satisfied if he feels time pressure during reception. The majority of men were unsatisfied with the time pressure. The fourth area assessed the suitability of education through a brochure and according to the practice of the department in the success of the preventive measure of postoperative incontinence. The men who used the brochure were aware of the possible complication and its prevention. The work provides a comprehensive view of the issue of education in prevention of incontinence after prostate surgery. The output of this work is an information brochure for patients of the urological ambulance. Results of the research are used to improve the educational process in patients coming for surgery of the prostate.

Efektivita používání pomůcek pro inkontinentní nemocné v následné péči / The effectiveness of aids for incontinence patients in aftercare

This thesis deals with the effectiveness of use of aids intended for incontinent ill people in subsequent care. The thesis is divided into a theoretical part and experience part. The experience part of the thesis has been implemented by means of qualitative research. The data have been obtained on the basis of two different half pen and paper interviews with patients/clients who are using incontinence aids prescribed by a doctor on a coupon or by a matron or a ward sister from the subsequent care department in hospitals with the state-funded organization and in hospitals with the joint-stock company organization. The result of the qualitative research showed, that both groups of respondents use mostly only plastic pants or nappies out of the whole assortment of aids. For patients/clients living at home the reason is that on the basis of anamnesis the general doctors prescribe the incontinence aids only in the most serious incontinence cases. At the subsequent care department the situation is caused by a big number of immobile fully incontinent patients/clients. The results show, that the quantity of absorb aids prescribed by general doctors is insufficient. It does not reach the quantity limits given in the documents by VZP (Public Health Insurance Company). Taking into consideration the low income of seniors, the respondents have to pay considerable amounts of money for incontinence aids which are necessary for them. There is an absolute lack of absorption aids in the hospital with the state funded organization. The incontinence aids are financed by three different ways. Patients/clients themselves have to get and pay the absorption aids at the Geriatrics department I and subsequent care department 2. The hospital provides them only with one-off mats and hygienic aids free of charge. The reason for this is a low limit of material. The respondents regard this system of financing negatively. It burdens patients/clients and nurses too. If patients/clients do not have the aids, the hospital has to lend the aids, make a list of debts and collects them back. On this account conflict situations often arise in families. At these departments there are reserved five social beds which enable patients/clients the prescription of aids by a doctor on a coupon likewise the patients who are ill and cure at home. At the stopgap department patients/clients pay a monthly fee; this includes the costs on incontinence aids too. At the subsequent care department 2A, B of the hospital with the join-stock company organization the aids are fully paid by the hospital. There is available a sufficient assortment and quantity of absorb and hygienic aids, which enable an individual attitude.

Aim: This study was designed to examine the influence of female urinary incontinence on development of sexual dysfunctions. By means of the questionnaire investigation we inquired how often and how the sexual behaviour and response were modified. Our goal was to determine the prevalence of coital incontinence, describe the most common sexual disorders and to find out how women deal with the urinary incontinence in a partner relationship and how such situations are resolved. Attention was given to the interests of health care professionals in this area, the quality of their communication with patients, effect and satisfaction with therapy. Research sample: The study included 106 women with urinary incontinence (aged 30-44 years and 44-59 years). Characteristics of their sexual behaviour were compared between the two age groups and in relation to assessed diagnosis. Changes in sexual behaviour were evaluated both in relation to the population group who were a part of representative research study of Czech women in 2008 and control group of 112 healthy women of comparable age. Title of master's thesis: The relation between backache and urinary incontinence Aims: The aim of the thesis is to find relations between various body structures. The main focus is on the analysis of the connections between backache and urinary incontinence depending on duration of backache, localization, backache promotion and sports activities. Methods: The survey was carried out by a four-page questionnaire. In total there was a group of 97 respondents, 79 women and 18 men, who had problems with backache, urinary incontinence or both. The age range of the respondents was from 18 up to 84 years. The questionnaires were distributed to specialized clinical centers in the fields of rehabilitation and physical medicine. The survey data was processed and analyzed by using the statistical program SPSS, Statistical Package for the Social Sciences. Results: The results indicate that there is a relation between backache and urinary incontinence. There is also a relation between promotion of pain in the lower extremities and urinary incontinence. Moreover, it has shown that more than half of the respondents had backache and urinary incontinence at the same time. However, the last two results have not been found statistically significant. Title: The use of biofeedback in physical therapy Objectives: The aim of this thesis is to summarize the accessible literary sources mapping the area of use of biofeedback in physical therapy and forms of its use. Furthermore, it aims to make a research and evaluate the effect of biofeedback therapy of woman urinary incontinence and neurofeedback therapy with children with Attention Deficit Hyperactivity Disorder (ADHD syndrome). Methods: This thesis is a descriptive analysis based on a literature review. The first part includes theoretical background dealing with the description, principle of operation and the possibilities of influence the therapy using biofeedback. The next chapter focuses on different areas of use of biofeedback in physical therapy and identifies the different forms of use. Subsequent research contains a presentation of studies, their comparison and critically evaluation of the effect of therapy using biofeedback for selected diagnoses. Results: Based on 69 found studies were clearly mapped using various forms of biofeedback in physical therapy.
