Abstact The diploma thesis is focused on forms and dimensions of breast-feeding support regarding different levels of women and child care in the health care system of the Czech Republic from the perspective of a woman, a mother, who is the health care recipient in the context of the legal framework of Czech legislation and relevant international documents. The findings are based on empirical research results gained from ten semi-structured in-depth interviews. Ten individual research participants gave birth in the same medical institution with the title Baby Friendly Hospital during the last three years, which is the unifying element of the participants. The perspective of a woman, based on a thematic analysis of interviews , shows a lot of perceived dimensions of the provided health care. This work identifies the most obvious women themes: their perception of breastfeeding in pregnancy, importance of breastfeeding for woman, the mother's contact with the baby after the childbirth, medical staff attitude and quality of provided care, aspects of pediatric care and support of breast-feeding out of the health care system. This diploma thesis deals with the degree of autonomy of women in the issue of breastfeeding in the Czech environment. It examines in particular the form of current breastfeeding and its normativity through narativ interviews with women who had breastfeeding experience. The aim of this work is to expose the form of the current brestfeeding standard, its limits, negativity and impacts through feminist optics and offer possible solutions in the discourse of breastfeeding. The basic theoretical starting point of the thesis is the development of infant feeding standards, its promotion and legislative anchoring in the Czech Republic as social approach of brestfeeding as well. In particular, the thesis deals with the feminist critique of the discourse of the fight, which provides the necessary arguments for looking at the phenomenon of breastfeeding standard in a different way and introducing the standard as social and cultural constructed. The analysis is the result of introducing current breastfeeding standards in the Czech republic as a directive reuqirement towards women to breastfeed which threatens women's integrity. In my thesis I describe problems related to breastfeeding in children with cleft disordres. In the first part, I present general knowledge general about breastfeeding and cleft disorders and specific problems that occur in particular cleft defects, and their management. I describe alternative feeding methods, considering advantages and disadvantages, and the possible effect on breastfeeding. Second part deals about the effect of breastfeeding on specific complications in children with cleft disorder, such as otitis media, growth retardation and others. The importance of lactation counselling is mentioned in the last part. The goal of my work is to bring the topic to the awareness of health professionals, to provide relevant information, and make the challenge of breastfeeding children with cleft disorders successful.

This diploma thesis deals with the general knowledge of pregnant women about appropriate diet during pregnancy and breastfeeding. The thesis is divided into the theoretical and practical part. The theoretical part summarizes the knowledge of preconception nutrition, important nutrients during pregnancy, dangerous substances for the developing fetus, gestational diabetes mellitus and also includes general recommendations for pregnant and nursing women. The main objective of the practical part was to verify the respondents' knowledge of appropriate diet during pregnancy and lactation. Next, confirm or refute hypothesis number 1, whether multiparae are more informed about a diet more than the primiparae and hypothesis number 2, how many women were using preconceptual folic acid. An electronic questionnaire was developed to collect data and was distributed through social networks in pregnancy-oriented groups. So the questionnaire could be filled by every pregnant woman in the Czech Republic at any time according to her time. In total 491 questionnaires were processed. The results were entered into a table in Microsoft Excel and then summarized and evaluated in the form of graphs and tables. The aim of this study is to explore the psychosocial aspects of care provided in maternity hospitals. The study is divided into three parts: i) the psychosocial aspects of care for parturients, ii) the psychosocial aspects of care for the newborn, and iii) the psychosomatic issues in obstetrics. Each part includes a review of the relevant literature and a presentation of the specific research carried out for the purpose of this thesis. The first part discusses the psychosocial competences of healthcare providers (social support, communication, providing of information, etc.), while subsequent research focuses on the psychosocial factors that influence parturients' satisfaction with perinatal care. The second part is devoted to psychological importance of birth experience for the newborn and early mother-child contact, psychosocial needs of the health-risk newborn and initiation of breastfeeding, with the research section containing the inquiry into the factors that may influence the start of breastfeeding. The aim of my doctoral thesis is the detailed study of the behavioural and sociobiological principles associated with the period of lactation. Suckling data covering almost 80 foals of domestic horse (Equus caballus) was obtained within four years of observation in the National Stud in Kladruby nad Labem, Czech Republic. The data were evaluated together with the reproductive history of the mothers, their agonistic interactions and following analysis of the dominance position of subadult foals. The first study describes longer suckling (greater maternal care) in dominant mares compared to submissive ones. Dominant mares are also less disturbed during nursing, whereas they terminated the nursing of submissive mares more often. A possible competitive advantage for the offspring of dominant mares may arise from such behaviour. The second paper enlightens the reproductive strategy of mares facing double maternal investment; suckling of the foal and pregnancy at the same time. I found out, contrary to the expectations, that the pregnant mares neither suckled their foals less nor terminated more suckling compared to non-pregnant herdmates. According to the experts, the breastfeeding support and education of mothers in the Czech Republic is insuficient. There is declining number of children who are brestfed exclusively to six months of life, premature babies posing even greater problem in this matter. Much of this problematic trend resides in the lack of support for mothers. A major problem in the Czech Republic is the fact that there are still a few maternity hospitals which are arranged so that mothers can be with their babies immediately after birth, especially if we talk about preterm newborns. Breastfeeding is the most important way to ensure the healthiest nutritional needs of an infant. It allows not only nutrition, but also feeling of safety and security, which is very important in proper development of the child. Furthermore, the breastfeeding contributes to the creation of solid emotional relationship between mother and child. Among other things, breastfeeding is undoubtedly the cheapest possible child nutrition. Goal of this thesis is to draw attention to a small number of premature babies, who are fully breastfed at discharge from the hospital leading to many negative consequences such as reduced immunity, psychosocial issues, et cetera. In our opinion, the problem is caused by lack of time, excessive workload of nurses and doctors' little initiative. Through this thesis we would like to draw attention to current knowledge and information about the breastfeeding preterm infants in perinatal centers. We also mapped the number of premature babies fully breastfed at discharge from hospital and identified through research the causes of low prevalence of breastfeeding these children. Another objective was to map the knowledge and skills of nurses in practice, to determine the connection between these skills and the difference in diet between compared hospitals. Finally, we propose solutions to the identifed shortcomings. The theoretical part processed characteristics of a premature baby, anatomy and physiology of breastfeeding and its benefits for the child, proper breastfeeding technique, the importance of nurses in the management of breastfeeding and activities to promote breastfeeding in the Czech Republic and in the world. In the empirical part we used qualitative and quantitative research. We collected data in three randomly selected perinatal centers in the Czech Republic during the fourth quarter of 2012. The stated objectives of the study were processed using a questionnaire, content analysis of documents and methodology and regression trees. The research showed that all of the top centers have similar level of child care. However, there are significant differences in nutrition and ways of feeding the children and nursing care. The study showed that child nutrition at discharge from hospital is different, and this may have an impact on the later development of the child, as evidenced by other studies. Differences in nursing care most relevant to the proper and effective education of the mothers, which both can be observed from the investigation, have the greatest impact on the type of diet at the end of the hospitalization of the infant.

This diploma thesis examines the role of paediatric nurses caring for newborns in the community. It aims to determine if and how newborn care in the home environment takes place and the role of the paediatric nurse. The research is based on one of the primary objectives set out by the member states of the WHO to improve the health of the entire population in the twenty-first century with the focus on community care and its development. The theoretical part is dedicated to the definition of community care and its history in the Czech Republic. We initially focus on how childcare at home was provided in the past. This is followed by a description of the current situation regarding care of a newborn and the mother during the first weeks following birth, both here and abroad. We evaluate the role of the paediatric nurse in terms of nursing care for newborns and research the duties that the nurse carries out within the clinic, family, and community.We have set out two objectives and five research questions. The first objective was to identify nursing care problems encountered by mothers at home and how they solve these problems. The second objective was set out in order to explore how nurses become involved in the community care of a newborn. We have chosen a qualitative method for the research part of the thesis. Data was collected through semi-structured interviews. Subsequently, a qualitative analysis and categorization of collected data was conducted. Two research sample groups were created in the first phase. The first consisted of mothers with newborns, i.e. children from birth to 28 days. The second group consisted of paediatric nurses who work in outpatient care with children and adolescents. The second phase of the research addressed paediatric nurses working in the neonatal clinic, which is run by the hospital's paediatric department. The result of our thesis is the finding that community care provided by the paediatric nurse in the field, in a natural environment and with the cooperation of the entire family and community, as defined, is almost nonexistent. The long-established system of primary care directs all nursing work into the surgeries of the paediatric practitioner, and only the first visit to the newborn, but not in all cases, is conducted at home. This system is insufficient to cover all the specifics of childcare. The needs of the newborns and their parents are becoming more urgent and in the first weeks following the birth the mothers only very slowly begin to gain healthy self-esteem and confidence in their maternal abilities, hence the need for an individual approach and help in the proper care of their baby. They feel there is a lack of information and they do not know how to deal with the needs of their newborn. The most problematic issue described is breastfeeding. The addressed nurses are of the same opinion and also consider breastfeeding as the most common focus of their intervention, and therefore, the most important part of their work.We have discovered that the skills and competencies of the paediatric nurses are sufficient to enable them to become a relevant element in the comprehensive care of newborns within the community. The interventions delivered by the nurse are fully integrated into paediatric nursing.The parents of newborns would, according to our survey, prefer a home visit from health professionals, particularly if it concerns the first contact with the baby and family. The out-patient care does not provide enough time and space to pursue common nursing issues, therefore, home visits are an essential part of the care that should be offered to the mother and baby. Community nurses would help to coordinate the services provided to families as well as help solve the nursing issues that mothers encounter in the first weeks of their child's life. A paediatric nurse would, therefore, become a partner to the parents in a joint effort to ensure the healthy development of their child.
