Ekonomická efektivnost zavedení léčby konopím v České republice / Economic effectiveness of the cannabis treatment in the Czech republic

Thesis deals with currently discussed topic -- the medical use of cannabis -- the missing possibility in the current medicine state in the Czech republic. Despite the fact that cannabis seems to be very effective for medical use, official medical use of cannabis permitted by law can be observed in only few countries. Czech republic doesn't belong among these countries, however there are some tendencies to change current situation. The goal of the thesis is to stand up for medical use of cannabis in the Czech republic and to contribute to above mentioned tendencies to change current situation. To reach the goal thesis introduces theoretical background of medical use of cannabis which explains advantages of its use, but primarily in the empirical part in which compares current (without the use of cannabis) and potential costs (with the use of cannabis) of the treatment of multiple sclerosis comes with the conclusion that with the use of cannabis is possible to save 1 931 923 444,- CZK.

Ekonomie trhu léčiv: Vedou státní dotace na léky k jejich plýtvání? / Pharmaceutical market economics: Do public subsidies on pharmaceuticals cause their wasteful use

This diploma thesis deals with the question of whether do public subsidies cause wasteful use of pharmaceuticals, and if so then to what extent. The hypothesis is tested on the Czech republic data for the years 2005-2012. The biggest contributions are defining the health system as an intra-temporal problem, which tells us that lower values of discount rate implies higher level of wasteful use. I express the environmental issues, which are caused by unprofessional pharmaceutical disposal. You can find the price (-0,11), the income (-0,2) and the cross-price (-0,14) elasticity of demand for pharmaceuticals in the empirical part. These estimates are obtained with the help of the Kuhn-Tucker method. Using the 2OLS method Model 2 comes with the major findings. With 10% increase of public subsidies, consumers tend to waste 13,2% more pharmaceuticals.

Name of the work: The content of the anthocyanidines and other pigments in commercial drugs and food supplements Author: Hana Karanská This thesis is focused on the determination of anthocyanins and anthocyanidins in food supplements, fruits and juices. The substances were extracted with methanol using an ultrasonic bath or boiling water bath. Analysis was performed by HPLC. The results of this work show that the content of anthocyanins and anthocyanidins are in the determined samples represented in varying degrees. All the analysed samples showed the content of these substances. Based on information from the manufacturer we can say that the product Ostrovidky plus contained the declared amount of anthocyanins. Manufacturers of remaining specimens does not provide sufficient information about the composition for unambiguous determination of the result, but we can say that the products approximately satisfy the content of substances declared by the manufacturer. The amount of anthocyanins in the fruit meets the values reported in literature. Juices did not indicate the exact amount of anthocyanins.

The introductory part of the diploma thesis deals with the concept of health and the factors that influence it. The aim is to grasp the effect of socioeconomic status on the health and to analyze complementary and alternative medicines. The specificity of the health market, the expenditures on the health service, subjects which finance a health care, the state health policy focusing on drug policy and pharmacoeconomic are remarked. The paper also refers to the history of drugs and medicines, their consumption and development in conjunction with pharmacoeconomic. Marketing mix in terms of pharmacy describes the product (i.e. a drug) and its life cycle, the price and price control in the Czech Republic, distribution and promotion associated with advertising. The practical part of the master's thesis specifically shows marketing, innovation, science and research in pharmacy. The examples illustrate the corruption problems and difficulty of fighting against them. The information about the two large pharmaceutical or medical manufacturers are summarized at the end of this thesis.

Zneužívání sedativ a hypnotik z pohledu uživatele / The abuse of sedatives and hypnotics from the user's perspective

Background: Sedatives and hypnotics are substances to calm or relieve anxiety and to induce sleep. Long term prescription of benzodiazepines/Z-drugs may lead to the development of tolerance and the need to increase the dose. It is important to be aware of the risks of uncontrolled use of these drugs, such as harmful interactions with other substances, side effects and potential for abuse or addiction. Aim: The aim of this research was to describe abuse of sedatives and hypnotics from the user's perspective, to focus on circumstances leading to drug abuse, to describe beginning and course of the use of these drugs and self-awareness of the problem. The aim was also to describe the current state of users and their needs. Sample: The research sample consisted of 6 participants, of which 4 women and 2 men currently or recently using sedatives and hypnotics (specifically benzodiazepines/Z-drugs). Methods: Data were collected through a qualitative approach using a semi-structured interview. Interviews were conducted, rewritten into protocol summary, further analysed using thematic analysis, modified using open and axial coding and categorization methods. This diploma thesis focuses on drug approval efficiency. It is obvious that the drug approval process is lengthy and expensive. The question that arises is whether the length of the drug approval process, as well as associated costs, leads to better safety. The first part briefly summarizes the history of the drug regulation and the drug legislative in the Czech Republic. Also focuses on the theory of economic regulation and the theory of bureaucracy. The second part deals with the drug approval process for new drugs, describes preclinical and clinical testing in relation to the analysis of Sam Peltzman. The third part compares drug approval process in different markets. The last part summarizes off-label prescribing, especially in pediatric.

Faktory ovlivňující substituci v rámci generik v České republice / Determinants of generic drug substitution in the Czech Republic

This study uses logistic regression to estimate the probability of choosing the brand-name drug despite the presence of cheaper generics given patient and drug characteristics in the Czech pharmaceutical market in the period 2009-2013. Results of the analysis may help guide policies to decrease pharmaceutical costs. In order to motivate people who are more likely to choose the original version of drug, policy makers may impose higher copayments and lower subsidies on the original drugs which they use. Additionally, two supplementary analyses were applied to sub-sectors of the pharmaceutical market: antihypertensive drugs and antibiotics. Fixed effects logistic regression is employed to test the robustness of the results.

Attention should be paid to pharmacotherapy in the orthopedic department mainly because patients at this department make up a risky group, they are older and often tend to use more types of drugs at the same time. Nursing at the orthopedic department is both mentally and physically demanding. Nurses administer medication to patients throughout the whole day, that is why lack of certain medication may occur in this department, the same way they would with all routine procedures. Be it conscious or unconscious. The thesis sets its objective to map out the issue of pharmacotherapy in the orthopedic department. Further, to map out factors that may influence the origin of complications when administering medication to patients in the orthopedic deartment, and suggest improvements in the process of pharmacotherapy in the orthopedic department. A qualitative research via participant observation and semi-structured interviews carried out with the nurses and patients of the orthopedic department, was used. The results of this survey prove that nurses do have basic theoretical knowledge in the area of pharmacotherapy. At the same time certain shortcomings were discovered, i.e. this theoretical knowledge is not being applied in practice. Most surprising were the results in the area of patient identification before administering medication, this step not always being abode by the nurses as a part of medical error prevention. Quite on the contrary, it can be said, based on the results, that nurses do have excellent knowledge and skills in pain reduction. This is proven by both participant observation and the happiness of patients themselves. Partial results have been presented at a seminar in the orthopedic department, and they have been passed on to the management of this department. The resulting shortcomings have been used as aid material. The diploma thesis was written so as to serve as material of information to beginner or existing nurses at the orthopedic department as well.

1 ABSTRACT Background The annual drug overdose rates have been increasing exponentially since the 90's worldwide. Toxicological information centre (TIC) represents a valuable source of information for evaluating the trends in the drug poisonings in Czech Republic. Aim of the study The purpose was to analyze the number and trends in the calls concerning poisonings due to central nervous system (CNS) affecting drugs, identify the reasons of medication errors caused by laymen, frequency and consequences of these errors across all age groups and also to analyze the numbers, causes, symptoms and severity of the paracetamol intoxications. Methods During the reference period the data from the enquiries were extracted from the TIC electronic database, discharge reports from the hospital were studied or phone call follow-ups with the patients were carried out to be able to evaluate the outcome of the poisonings. Results In the years 1997-2002 the number of calls caused by poisoning with tricyclic antidepressants and barbiturates decreased (by 366.7 % and 340,0 %, respectively) whereas the calls due to selective serotonin reuptake inhibitors and benzodiazepines overdose increased (by 1347.4 % and 359.8 %).
