Em 2000, um fator hipotalâmico da família RFamida, a mesma da kisspeptina, capaz de inibir o eixo HHG em codornas foi descoberto e denominado Hormônio Inibidor das Gonadotrofinas (GnIH). O peptídeo relacionado à RFamida-3 (RFRP-3), ortólogo do GnIH, e seu receptor GPR147 foram recentemente identificados no hipotálamo humano. No entanto, ainda não há evidências do papel funcional do sistema RFRP3/GPR147 em humanos. Objetivos: Investigar a associação do sistema inibidor das gonadotrofinas RFRP-3/GPR147 com a regulação do desenvolvimento puberal e capacidade reprodutiva em seres humanos. Métodos: Oitenta pacientes com puberdade precoce central idiopática (PPC) e 51 com hipogonadismo hipogonadotrófico idiopático normósmico (HHI) foram investigados. Cinquenta pacientes formaram o grupo controle. A sequência codante dos genes NPVF and NPFFR1, que codificam respectivamente o RFRP e o GPR147, foram amplificadas e sequenciadas. As frequências dos polimorfismos (SNP) foram comparadas entre os grupos e a razão de probabilidades (odds ratio) para a ocorrência de cada SNP de PPC ou HHI foi determinada. Resultados: Todos os SNPs já estavam descritos em bancos de dados genéticos (NCBI database). Uma deleção in frame de três nucleotídeos foi identificada no gene NPVF (p.I71_K72), com uma proporção levemente menor no grupo CCP, se comparado ao grupo com HHI (5% versus 15%, respectivamente; p=0.06). Isto resulta na deleção da isoleucina na posição 71, adjacente à lisina em um dos sítios de clivagem endoproteolítica do peptídeo precursor. Este SNP esteve associado a proteção para a occorrência de PPC (OR 0.33; IC 95%: 0.09-0.89); interessantemente, apenas dois homens com HHI foram homozigotos para esta variante. Um total de cinco SNPs missense foram encontrados no gene NPFFR1, com frequências semelhantes entre os grupos e sem associação com o tempo de puberdade. Esta é a primeira descrição de variantes genéticas dos genes NPVF and NPFFR1 em uma série de pacientes com desordens puberais GnRH-dependentes e controels saudáveis. Conclusão: Concluímos que o sistema RFRP-3/GPR147 deve ter um papel secundário na regulação do desenvolvimento puberal; propõe-se um efeito modulador da variante p.I71_K72 do gene NPVF sobre a ativação do eixo gonadotrópico. _________________________________________________________________________________________________ ABSTRACT / Introduction: Gonadotropin-releasing hormone (GnRH) is the main regulator of hypothalamic-pituitary-gonadal (HPG) axis, but new discovered neuropeptides can mediate the action of sexual steroids on hypothalamic level. In 2000, a hypothalamic factor of RFamide peptide family, the same one of kisspeptin, capable of inhibiting the HPG axis in quails, was discovered and named gonadotropin-inhibitory hormone (GnIH). RFamide-related peptide-3 (RFRP-3), the ortholog of avian gonadotropininhibitory hormone, and its receptor GPR147 have been recently identified in the human hypothalamus. However, there are still no evidences of the functional role of RFRP-3/GPR147 system in humans. Objectives: Our aim was to investigate the association of the gonadotrophin inhibitory system RFRP-3/GPR147 with the regulation of puberal development and reproductive capacity in humans. Methods: Eighty patients with idiopathic central precocious puberty (CPP) and 51 with normosmic isolated hypogonadotropic hypogonadism (nIHH) were investigated. Fifty healthy subjects comprised the control group. The coding sequence of the NPVF and NPFFR1 genes, which encode respectively RFRP and GPR147, were amplified and sequenced. The frequencies of polymorphisms (SNP) were compared among the groups and the odds ratio of each SNP for the occurrence of CPP or nIHH was determined. Results: All SNPs were already described in NCBI database. A 3- nucleotide in frame deletion was identified in the NPVF gene (p.I71_K72), with a slightly smaller proportion in the CPP (5%) as compared to the nIHH (15%) group (p=0.06). It results in the deletion of the isoleucine at position 71, adjacent to lysine at an endoproteolytic cleavage site of the precursor peptide. This SNP was associated to protection for the occurrence of CPP (OR 0.33; 95%CI: 0.09-0.89); interestingly, only 2 men with nIHH were homozygotes for it. A total of 5 missense SNPs were found in the NPFFR1 gene with similar frequencies among groups and no association with pubertal timing. This is the first description of NPVF and NPFFR1 genetic variants in a series of patients with GnRH-dependent pubertal disorders and healthy controls. Conclusion: We concluded that RFRP-3/GPR147 may play secondary, modulatory roles on the regulation of pubertal development; a restraining modulatory effect of the NPVF p.I71_K72 variant on gonadotropic axis activation is proposed.
