Dietary approaches to stop hypertension dietary pattern, physical activity, and risk of atherosclerotic cardiovascular diseaseGlanz, Eric 09 October 2019 (has links)
BACKGROUND: Atherosclerotic cardiovascular (ASCVD) disease is the leading cause of death in the United States. Longstanding poor nutrition habits and lack of exercise contribute to chronic inflammation and promote the development ASCVD. In clinical trials, the Dietary Approaches to Stop Hypertension (DASH) eating pattern, a diet rich in nutrients with anti-inflammatory and antioxidant properties, has been shown to reduce the risk of hypertension. Its role in overall cardiovascular (CV) health, both alone and in combination with an active lifestyle, is less clear.
OBJECTIVES: The primary goal of this study is to examine the relationships between the DASH eating pattern, physical activity, the development of incident cardiovascular disease (CVD). Specifically, we will evaluate the independent and combined effects of DASH and moderate to vigorous physical activity (MVPA) on cardiovascular event risk among men and women in the Framingham Heart Study Offspring (FOS) cohort.
METHODS: The DASH score was derived from two sets of three-day dietary records gathered during examination visits 3 and 5 in the prospective FOS cohort. Activity was self-reported and used to generate a composite score for MVPA. Scores were constructed for each individual by multiplying the number of hours spent during each day in sedentary, light, moderate, and vigorous activity by a standard weights reflecting the intensity of that activity. Subjects included 2503 individuals (1137 men and 1366 women), aged 30 years and older. Individuals were free of type II diabetes mellitus (T2DM), cancer, and cardiovascular disease at baseline. The DASH score was categorized according to adherence to the eating pattern, first as low, moderate, or high adherence and then dichotomized (DASH score <4.45 vs. ≥4.45). The MVPA score was classified into quintiles and also dichotomized (MVPA score <6.1 vs. ≥6.1). The dichotomous DASH and MVPA scores were then cross-classified and each subject was assigned to one of the following categories: (1) low DASH score / low activity (referent), (2) low DASH / high activity, (3) high DASH / low activity, and (4) high DASH / high activity. Follow-up started at exam 5 and continued through the ninth examination cycle in 2014. Cox proportional-hazard regression models were used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) for all subjects and for men and women independently, adjusting for confounding by age, BMI, cigarettes smoked per day, and grams per day of alcohol consumed. Factors found not to confound the effects of DASH and MVPA on CV risk were excluded from final models.
RESULTS: During a median follow-up of approximately 20 years, 286 and 177 incident CVD cases were documented among men and women, respectively. Women in the highest DASH adherence category had approximately 28% lower total CVD risk than women in
the lowest adherence group (HR, 0.72; 95% CI: 0.48-1.10). Effects in men were weaker. Women in the highest quintile of MVPA score had approximately 44% lower risk of CVD than women in the lowest quintile (HR, 0.66; 95% (0.40-1.10). Overall, CVD risk was 26% lower (HR: 0.74 (95% CI: 0.55-0.99) for those in quintile 5 vs. quintile 1. Women in the High DASH/High MVPA category had a 43% lower total CVD risk than women in the Low DASH/Low MVPA group (High DASH/High MVPA: HR, 0.57; 95% (0.36-0.90)). These associations ere generally stronger in women than men.
CONCLUSIONS: In this prospective cohort study, active individuals with higher adherence to DASH was associated with a lower atherosclerotic CVD risk compared to those with lower activity and DASH adherence. The beneficial effects of diet in particular were stronger among women than men.

Ideologisk illusion : En idéanalys av Moderaternas familjepolitik / Ideological illusion : An analysis of the Moderate Coalition party's family policySjöholm, christoffer January 2022 (has links)
This bachelor's thesis examines the ideological position of the Swedish Moderate party. The evaluation of the moderate ideology is done between the years 2009-2022. This is done to understand the ideological position of today but also to present a modern contribution to a field that tends to focus on more historical periods. Furthermore, the ideological evaluation focuses on family policy, as ideological changes should be visible in areas that are not the most relevant for political parties. To determine ideological position this paper uses ideal types of conservatism and liberalism and examines the material concerning these. The conclusion is that the Swedish moderate party has taken some twists and turns in relation to the ideologies presented but that the moderate party of today is liberal.

Socialdemokrater, liberalkonservativa eller bara EU-vänner? : En studie av ideologi och konflikt iriksdagsdebatter om EU / Social democrats, liberal conservatives or just friends of the EU? : A study of ideolory and conflict inSwedish parliamentary debates about the EUBergsten, Klara January 2021 (has links)
National debates on European Union politics have long been viewed as a consensus-oriented activity. This applies especially to big mainstream parties, both to the center-left and the center-right. The perceived lack of ideological conflict over European issues is connected to the problem of the democratic deficit, and has led scholars to call for deeper politicization. Consequently, the aim of this study is to describe the presence of classical ideologies and ideological conflict in Swedish EU debate. This is done by analyzing statements from two major parties, the Social Democratic Party (Socialdemokraterna) and the Moderate Party (Moderaterna), in chamber debates in the Swedish parliament during the parliamentary year 2018/19. A qualitative ideational analysis is performed by systematically categorizing the ideological content based on ideal types. The results show that although ideological arguments and left-right conflict did occur, they were not common in the debates as a whole. Ideological conflict was most pronounced in debates on the EU’s social dimension and migration policy, while the economic issue area was characterized by liberal consensus. It is also argued that the Social Democratic Party faces greater challenges in transferring their ideology to the EU level. Moreover, I discuss how the results relate to previous research and the issue of the democratic deficit.

Current Concepts for the Surgical Management of Carotid Body TumorKnight, Theron, Gonzalez, Jose Andres, Rary, John M., Rush, Daniel S. 01 January 2006 (has links)
Background: Carotid body tumor (CBT) is a rare lesion of the neuroendocrine system. Chronic hypoxia has long been recognized as an etiology of CBT and other paragangliomas. Recent biogenetic discoveries reveal that mutations in oxygen-sensing genes are another etiology, accounting for approximately 35% of cases, and that these 2 etiologies are probably additive. Data Sources: (1) A retrospective analysis of fifteen cases of CBT in a 6-year period occurring in the mountains of Southern Appalachia; (2) an extensive review of the literature on the surgery of CBT and on the expansive biogenetic understanding of the disease. Conclusions: Improved imaging, vascular surgical techniques, and understanding of the disease have vastly improved outcomes for patients. The necessities for long-term follow-up and appropriate genetic testing and counseling of patients and their families are documented. Surgeon and institutional competence are critical in achieving maximal outcomes.

Deficits in Spatial Learning and Memory in Adult Mice Following Acute, Low or Moderate Levels of Prenatal Ethanol Exposure During Gastrulation or NeurulationSchambra, Uta B., Lewis, C. Nicole, Harrison, Theresa A. 01 July 2017 (has links)
Debate continues on the merits of strictly limiting alcohol consumption during all of pregnancy, and whether “safe” consumption levels and/or times exist. Only a relatively few experimental studies have been conducted that limit the timing of exposure to specific events during development and the exposure level to one that might model sporadic, incidental drinking during pregnancy. In the present study, the effects of two acute gavage exposures to low and moderate levels of ethanol (peak blood ethanol concentrations (BEC) of 104 and 177 mg/dl, respectively) either during gastrulation on gestational day (GD) 7 (at GD7:0 h and GD7:4 h) or during neurulation on GD8 (at GD8:6 h and GD8:10 h) on the spatial learning and memory abilities of adult mice in the radial arm maze (RAM) were examined. Mice were selected from a prenatal ethanol exposure (PAE) cohort that had been tested as neonates for their sensorimotor development (Schambra et al., 2015) and as juveniles and young adults for open field activity levels and emotionality (Schambra et al., 2016). Mice exposed on either of the two gestational days to acute, low or moderate levels of ethanol were deficient in overall performance in the RAM in adulthood. Importantly, mice in ethanol exposed groups took longer to reach criterion in the RAM, and many mice in these groups failed to do so after 48 trials when testing was terminated. Exposure to a low level of ethanol on either GD7 or GD8, or a moderate level on GD7, resulted in significant impairment in spatial reference (long-term) memory, while only mice exposed on GD7 to the low level of ethanol were significantly impaired in spatial working (short-term) memory. Mice exposed to the low ethanol level on either day had significantly shorter response latencies, which may reflect impairment of processes related to response inhibition or executive attention in these mice. For all measures, distributions of individual scores revealed a relatively small subset of mice in each PAE group who scored well outside the range of the control group, which skewed the population distributions to varying degrees in the direction of worse performance for the PAE groups. Overall the data suggest that after acute, low level ethanol exposure early in gestation, the likelihood that an individual mouse embryo experienced measureable ill-effects due to the exposure was rather low, but in a few of the embryos, damage occurred that resulted in significant deficits in later performance. The overall characteristics of our cohort of PAE mice, including delayed sensorimotor development, mild hypoactivity and increased emotionality, as shown in previous studies, together with deficits in spatial learning and memory as shown here, resemble those in a subset of human Fetal Alcohol Spectrum Disorder (FASD) diagnoses, specifically ADHD-Inattentive type (ADHD-I) and/or Sluggish Cognitive Tempo (SCT). Although possible correspondences between mechanisms underlying PAE-induced deficits in mice and those operating in humans remain undefined, further study with this mouse PAE model may ultimately help advance understanding of the causes of these conditions in affected children. This study highlights the possibility of risk associated with low to moderate sporadic alcohol consumption during the first month of human pregnancy.

Well-Being Among Parents of Young Adults With Intellectual Disabilities When Transitioning From High SchoolThompson, Teshawnia 01 January 2018 (has links)
Transition out of high school for young adults with intellectual disabilities (IDs) has been described as a stressful time for individuals and their families, with increased demands for caregiving and parental support. A lack of research is associated specifically with those individuals with moderate IDs and their parents' experiences of well-being during the transition process. The purpose of this phenomenological study was to understand how parents of young adult-aged children with moderate IDs experience well-being as their child transitions from high school to adulthood. Ryff's model of psychological well-being was used as the conceptual framework to better understand and explore the psychological well-being of parents as they navigate through the transition process. Eight parents from an urban Georgia school district were recruited through criterion sampling and participated in individual semistructured face-to-face interviews. Constant comparison analysis was used to analyze the data. There were 8 major themes that emerged from describing the lived experience of parents: (a) expectations and preparation, (b) experience with school, (c) accessing and coordinating services, (d) social support systems, (e) daily activities and planning, (f) life as an adjustment, (g) personal growth, and (h) looking toward the future. School districts and adult service providers may gain insight from parent perspectives to help alleviate rather than exacerbate the challenges parents face during the transition process, which would contribute to the parents' psychological well-being.

MODERATERNA OCH ENERGIÖVERENSKOMMELSEN – VAD HÄNDE? : The Swedish Moderate party and The energy agreement. What happened?Eriksson, Johan, Wikgren, Carl January 2021 (has links)
The purpose of this study has been to investigate if and how the Swedish Moderate party´s views and ideas regarding nuclear energy, within the energy production has changed in the timeframe of 2010 - 2021. This has been investigated through an idea analysis. The study's research question was asked as follows: Have the Swedish Moderate party´s views and ideas regarding nuclear energy within energy production changed within the timeframe 2010 - 2021? And if that is the case, how have the views and ideas changed? The material that this study used to maintain its purpose contained motions from the Swedish Moderate party within the time frame 2010 - 2021. The motions involved nuclear energy. The conclusion of this study is that the Swedish Moderate party has changed its views and ideas concerning energy politics focusing on nuclear energy. The changes are marginal. However, the Energiöverenskommelsen is an anomaly regarding this case. Energiöverenskommelsen does not match the views and ideas presented by the Swedish Moderate party within the study´s timeframe.
