Quantitative determinants of need and demand for primary care in the district of Columbia

This study, quantitative determinants of need and demand for primary health care in the District of Columbia (DCPC), analysed data over a twenty-year period from 1985 to 2004, on need and demand for primary care using standard and epidemiologically innovative statistical measures for physician distributions and socio-demographic characteristics in the District of Columbia (DC). The study attempted to answer the question: Using U.S census-based small area aggregations, Census Tract Groupings (CTGs), that are not zip-code areas or legislative/political boundaries, can a multivariate predictive model be developed using physician distributions, primary care service index (PCSI) and composite need scores (CNS) to explain variations in primary care visits shortages? Primary care visits shortages and priority scores (PCPS) were calculated, analysed and presented for CTGs in the District of Columbia from 1985 to 2004. Results indicated that the abundant supply of DC-based physicians – indicated by decreasing population per physician ratios of 239 (1985) to 146 (2004) – appear to be a long-term trend. As raw physician counts increased, the ratio of satisfied visits to demand decreased, from 2.62 (1985) to 1.80 (in 2004). This result appears to indicate that, due to inequities in distribution of primary care physicians in DC’s small areas, the increasing numbers of primary care physicians were by themselves, not sufficient to address the city’s overall primary care visits need. Epidemiological profiles and physician distribution analytical methods appear to be useful for small area analysis of urban primary care shortage areas and for setting priorities. This thesis is divided into a theoretical and a practical part. The theoretical part deals with two separate realms which are then interconnected in the practical part. The first realm is narrative therapy, its description, function and implementation. One of the tools of narrative therapy is a therapeutic narration, which is discussed therein and is set into the field of social work. Next comes the second realm which is the topic of a divorce and children. There we discuss the influence of a divorce on children, the participants, and we are presented with the potentiality of how to diminish the negative influence. In the following practical part, we fist find a draft of a narration with therapeutic features from narrative therapy for children from divorced and divorcing families; the draft is supposed to alleviate the complications during and after a divorce. The other section of the practical part describes the research which verified whether the composed narration really helped alleviate the negative impact of a divorce on children. Evaluation of the state of the problematic areas performed both before and after the cooperation showed that narrative therapy helped to significanly decrease the number of serious problems in monitored areas. The general body of academic knowledge on the commercial development of smaller towns is in its infancy. Yet its inherent potential is well perceived from a development and market perspective. Shopping centre development could have an important function in getting products and services to the marketplace in an economical way. Furthermore, it could have a very significant socio-economic impact in the central business district (CBD). Although such a shopping centre will surely create new businesses, employment and production opportunities for local businesses it is of importance to first determine the financial viability and impact of the new development on existing businesses. Developers and researchers often approach the subject of shopping centre development from different perspectives. The research output is often of limited impact as the critical link between demographical analysis and financial viability is not made. Researchers are usually excluded from the physical establishment, rental structures, tenant mix, design, cost of construction, return on investment, funding and future management of the proposed new shopping centre project. Linking demographic assessment and financial viability is a critical output of this study. Bethlehem and its district are quite unique in a number of ways, especially regarding the presence of a large farming community. Bethlehem provides goods and services to residents of Bethlehem, Clarens, Kestell, Harrismith, Heilbron, Paul Roux, Petrus Steyn, Reitz, Senekal and Warden. The main shopping activity occurs in the CBD of Bethlehem and its surrounding areas. Bethlehem only has one major shopping centre (the Metropolitan Centre) that provides goods and services for the people in the surrounding area. This causes an over concentration in the CBD and too much traffic in an already limited space. There is a high need for Bethlehem to provide a bigger shopping centre for the citizens of the town, as well as the surrounding areas. This study investigates the need for a new shopping centre in Bethlehem and will determine whether a new shopping centre will be viable within the area The empirical study revealed that approximately half of the respondents are not satisfied with the current shopping centres in Bethlehem. A greater amount of respondents felt that the shopping centres do not offer enough parking. The study revealed that, from a consumer point of view, there is definitely a need for a new shopping centre in Bethlehem and that there is a gap of approximately 12 892m² GLA (Gross leasable area). However, this was determined before the opening of the new Dihlabeng Mall. Die gesin is noodsaaklik vir die modeme mens se soeke na vastigheid, geborgenheid en identiteit. Die oorspronklike verhouding tussen ouer en kind binne die kerngesin is onvervangbaar. Die vraag wat in die studie aan die orde kom is of die modeme gesin wel so 'n belangrike plek inneem in die huidige samelewing. Is die gevolg van al die probleme waarmee die moderne gesin le kampe het en die wisselvallige wereld waarin ons lewe nie juis dat die gesin se plek as veilige hawe al hoe meer bedreig word nie? Daar moet altyd in die bediening aan die gesin onthou word dat die gesin 'n ontwikkelende sisteem is. Daar moet veral by die behoeftes in die verskillende lewensfases aangesluit word. ln God se handeling met. die mens vind ons dat daar 'n baie noue verband is tussen die gesin en die verbond. Die kerk is by uitstek in die posisie om die wye verskeidenheid van gesinstipes in die samelewing te kan bedien. Die kerk kan ook gesinne help om aanpassings te maak in 'n veranderende samelewing. 'n Gesinsbediening kan 'n uitstekende nuwe fokus word vir die missie van die kerk. / Researches on the family came to the conclusion that the family is irreplaceable in the socialising process of a person. The family is necessary for the modern persons need for stability, security and identity. The original relationship between a parent and child in the family is irreplaceable. The question which arises in this study is whether the modern family is so important in the current society. Is the result of all the problems which the family faces and the changeable world not that the family as secure harbour is in danger? In the ministry it must always be borne in mind that the family is a developing system. Special attention must be paid so that all the different phases of life are included. The congregation must educate families as well as demonstrate God's purpose with the family. The church is pre-eminently in a position to serve the various types of families in society. The church is also able to help families to make adjustments in a changing society. Variables distinguishing entrepreneurs and non-entrepreneurs from different ethnic groups in the South African environment

A distinguishing characteristic of the current world economy is the decline in the number of industrial job opportunities which is also affecting South Mrica. The multi-ethnical and multicultural society of South Africa is also experiencing low economic growth rates, high population growth rates and an unacceptably high unemployment rate. Entrepreneurship currently plays an important role in the successful economies of the world. The small business can make important contributions to economic growth, job creation and social development. For this reason it is important that entrepreneurial behaviour be promoted in South Mrica. In previous research studies, researchers isolated different variables of entrepreneurial behaviour such as the environment, personal characteristics, values and personal background of individuals. Of these variables, only personal characteristics may be manipulated to advance entrepreneurial behaviour. The objectives of this study were to indicate whether certain biographical and demographical factors are related to an individual's behaviour towards the formation of a new enterprise and to indicate whether certain personal characteristics, namely achievement motivation and locus of control, are related to an individual's behaviour in the formation of a new enterprise. It was decided to compare four groups, namely white non-entrepreneurs, black nonentrepreneurs, white entrepreneurs and black entrepreneurs. Three factors of achievement motivation and three factors of locus of control were identified by means of factor analysis, and statistical analyses were done on the four different groups. To establish the above-mentioned relationships, the researcher made use of sophisticated statistical analyses. The following conclusion were drawn from the results: The models of achievement motivation and locus of control seem to predict membership of the group black entrepreneurs better than the group white entrepreneurs. Using the locus of control scales, membership of the group black entrepreneurs can be predicted well. The study revealed that there are differences between black groups and white groups (entrepreneurs and non-entrepreneurs) in South Africa. It is clear from the present study that there have to be additional factors, other than achievement motivation and locus of control, that influence entrepreneurial behaviour. Factors contributing to the breakdown of foster care placements : foster parents' and adolescents' perspectives

As a social worker at the Desmond Tutu Child and Youth Care Centre in Gauteng province, the researcher noticed an increase in the number of applications for adolescents’ admission to the Centre due to a breakdown in their foster care placements. This raised a question regarding the factors contributing to this breakdown. The aim of the study was to develop an in-depth understanding of factors contributing to the breakdown of foster care placements from the perspectives of the adolescents who were fostered and from those of their foster parents. The qualitative research approach was used applying an explorative, descriptive and contextual design. Purposive sampling was used to select the participants. Data was collected using semi-structured interviews and was analysed according to the framework provided by Tesch (in Creswell, 2009:186). For data verification, Guba’s model (in Krefting (1990:214-220) was applied. Conclusions are drawn and recommendations made about foster care involving adolescents. / Social Work / M.A. (Social Work)

Well-being of teachers in secondary schools

Schools worldwide are experiencing challenges in terms of ensuring quality education and good retention of its teaching staff. The highly stressful nature of the teaching profession as well as the high demands placed on teachers with the constant changes in curriculum, not enough resources and insufficient support from supervisors, cause secondary school teachers to show high turnover intention rates and high attrition rates which are extremely costly and detrimental to the success of the school. The well-being of the teacher is mostly overlooked within a highly stressful environment where the focus is on results. Demands on schools and teachers are becoming increasingly complex. Teacher issues are discussed on policy agendas as a result of concerns raised by teachers themselves about the future of their profession and whether they are sufficiently rewarded and supported in their work. The morale and motivation of teachers are important for future teacher retention. Teachers are now expected to have much broader roles, taking into account the development of the learner, the handling of teaching processes in the classroom, the focus of the entire school as a “community-in-action” and the relations with the larger community and the world of teaching in general. Thus expectations are higher and demands are more – but the well-being of the teacher does not seem to be a priority within the larger school environment and global teaching picture. Efforts to improve the psychological well-being and optimal functioning of secondary school teachers will affect individual and organizational outcomes. A teacher who functions well is more likely to stay in the profession and will be more motivated than one who is not engaged and demotivated. Investments in the well-being of teachers will lay the basis for positive school outcomes such better retention, better performance and job satisfaction. The aim of this study was to investigate the psychological well-being of a sample of secondary school teachers in North West Province and to determine the antecedents and outcomes thereof. A cross-sectional survey design was used to gather data regarding the well-being of secondary school teachers and its outcomes. A stratified sample (N = 513) was taken of secondary school teachers in North West Province in South Africa. The measuring instruments used were the Supervisor Behaviour Scale, Work-Related Basic Need Satisfaction Scale, Balanced Measure of Psychological Needs, Work Engagement Scale, Turnover Intention Scale, Work-Life Questionnaire, Revised Job Diagnostic Survey, Co-Worker Relations Scale, Work and Meaning Inventory, Personal Resources Scale, Self-Rated Performance Scale, and Positive Practices Questionnaire. The results of study 1 showed that supervisor support (for autonomy, competence, and relatedness) was positively related to employees’ psychological need satisfaction and engagement and negatively related to intention to leave. Supervisor support affected engagement positively and intention to leave negatively via employees’ autonomy satisfaction. The findings suggest that supervisor support and psychological need satisfaction play a significant role in the engagement and retention of employees. The results of study 2 showed that a calling orientation, job design, and co-worker relations explained a large percentage of the variance in experiences of meaningful work. A low calling orientation and poor co-worker relations predicted a moderate percentage of the variance in burnout. A calling orientation, a well-designed job, good co-worker relations, and meaningful work predicted work engagement. Job design was moderately associated with self-ratings of performance. The absence of a calling orientation predicted teachers’ intentions to leave the organisation. The results of study 3 showed that teachers with the highest levels of psychological functioning derived the most meaning from their work. These teachers are renewed by the work they are doing. Positive organizational practices predicted positive outcomes such as meaning, engagement and self-determined behaviour. Psychologically-well and healthy teachers are more likely to focus on the meaningfulness of the work they are doing. It seems that the most important positive practices in the pathway to better psychological well-being at work are those of meaningful work and inspiration.

Research has shown that technical information is often not read by its target group. This can mean that users are unaware of crucial information included in a handbook or manual. Lack of information can cause, for example, inappropriate usage of products. Research has presented several source preference criteria that individuals define when they choose a preferred information source. My goal is to design technical information that derives from these criteria. The question to answer is: How can information for a computer-based rendering software be designed to derive from the situation and information needs of the target group, and from the criteria individuals define when they elect an information source? This study is a collaboration with Company X in Västerås. The aim is to design an artifact that supports the employees of the company during a work procedure in the rendering software Lattice 3D Studio. Based on empirical data and a theoretical framework, I have designed information that derives from the situation and information needs of the target group, as well as from the criteria individuals define when they elect an information source. I have designed an instruction for a rendering software, with step-by-step instructions based on work procedures shown in empirical studies. From this work I conclude that good knowledge of the target group can give a better understanding of what information the target group needs and wants. By designing a sequence of steps based on work procedures documented during empirical methods, the target group can be assisted in their work procedure. By implementing information design principles, user friendly information is created. Based on the theoretical framework and methods, the instruction is designed to meet the criteria individuals have when they elect an information source. / Studier visar att teknikinformation ofta inte blir läst av användarna. Detta kan innebära att viktig information i exempelvis manualer inte når fram till användarna, vilket i sin tur kan orsaka felaktig användning av produkt. Forskning har redovisat ett antal kriterier som individer ställer när de väljer informationskälla. I denna studie avser jag att utforma teknikinformation som tar hänsyn till dessa kriterier. Frågan som jag ställer mig är: Hur kan man utforma ett informationsmaterial för ett datorbaserat bildrenderingsprogram som tar hänsyn till målgruppens förutsättningar och informationsbehov, samt de kriterier som individer ställer på en informationskälla för att den ska bli vald? Studien är utförd som ett samarbete med Företaget X i Västerås. Syftet är att jag ska utforma ett gestaltningsförslag som stödjer företagets medarbetare under en arbetsprocess i bildrenderingsprogrammet Lattice 3D Studio. Utifrån empiriska studier och ett teoretiskt ramverk har jag gestaltat ett informationsmaterial som utgår från målgruppens perspektiv: målgruppens förutsättningar, informationsbehov samt de kriterier som individer ställer när de väljer informationskälla. Jag har utformat en instruktion för bildrenderingsprogrammet, med steg-för-steg-instruktioner baserat på de arbetsmoment som framkommit genom empiriska studier. Slutsatser från arbetet är att en god kunskap om målgruppen kan leda till en bättre förståelse för vad målgruppen behöver och vill ha för information. Genom att utforma sekventiella steg baserade på dokumenterade arbetsmoment, erbjuds målgruppen stöd i deras vardagliga arbetsprocess. Med hjälp av informationsdesignsprinciper blir informationen användbar. Utifrån insamlad teori och empiri är instruktionen utformad för att uppfylla de kriterier individer ställer på en informationskälla för att den ska bli vald.
