Chronic obstructive pulmonary disease (COPD) is the name for lung disease with systemic consequences. Besides the respiratory symptoms there are also described changes in body metabolism, which could lead to the development of serious metabolic syndrome called cachexia. The main aim of this study was to compare the resting energy expenditure (REE) and nutrition substrate utilization in 12 patients with advanced form of COPD from the Czech Multicentre Research Database of COPD (5 females and 7 males, mean age 68 ± 6 years) and in 9 patients of control group without respiratory impairment (5 females and 4 males, 62 ± 4 years). Assessment of body metabolism was determined by method of indirect calorimetry. Measured REE was then compared with prediction based on Harris-Benedict equation. The utilization of main nutrition substrates was determined from the respiratory quotient and urea nitrogen loss in urine. Malignant lymphomas are tumors arising by clonal proliferation of lymphocytes stopped at a specific stage of differentiation. All tumor cells arising from the original clone thus share the same characteristics and that can be used in their detection. Finding a suitable molecular marker of tumor cells is an essential step not only to disease diagnosis, but also for monitoring of minimal residual disease. Minimal residual disease is defined as the subclinical disease level, which malignant cells are not detectable for conventional cytological methods during the therapy. These residual cells can cause relapse. The main goals of the diploma thesis are a detection and analysis of immunoglobulin genes rearrangement and chromosomal translocation t(11; 14) in the MTC region, and a development and optimization of RQ-PCR system for detection of minimal residual disease. Quantification of clonal rearrangement or chromosomal translocation allows the detection of minimal residual disease level in patients with malignant lymphomas. Clonal immunoglobulin genes rearrangement or characteristic chromosomal translocation were analyzed in 19 patients with malignant lymphomas. TITLE: Picture of illness and death in contemporary German literature for children and youth (The analysis and didactic adaptation in the context of selected works of world literature) SUMMARY:Presented thesis deals with literary reflections on illness and death in contemporary German literature for children and youth. The theoretical part explains the basic concepts such as illness, death, child and developmental stages of childhood, literature and literature for children and youth. These themes are then further processed. In more detail is the work devoted to analyzing literary works with that theme. The works are examined in terms of literary aesthetic and thematic components, composition and language components. In the practical part, the work focuses on the students perception of researched topic and work with specific literary texts. The final section compares the aspects of reception on illness and death of students of Czech and Austrian schools at the age of 14 to 19 years based on questionnaires filled in by those students which were used as the main research material. Philadelphia (Ph) chromosome, as a result of reciprocal translocation, is in majority of cases connected to two types of leukaemia - chronic myelogenous (CML) and acute lymphoblastic (ALL). The translocation occurs within large intronic sequences of BCR and ABL genes. The breakpoints are specific for individual patient and may be used as a target for monitoring of leukemic burden (MRD, minimal residual disease) during the treatment. In general, MRD is an important prognostic factor, which influences the treatment intensity. Two standardized methods are currently used for its monitoring. The first one is based on the detection of clonal specific Immunoglobulin and/or T-cell receptor genes rearrangements (and thus cannot be used for CML cases) at the DNA level, the second one utilizes detection of the BCR/ABL fusion gene at the mRNA level. Our aim was to optimize and standardize the process to find individual patient breakpoints on Ph chromosome and to use it for MRD quantification. We found the breakpoint in 80 % cases. The MRD data from 15 patients obtained by our method were compared to the levels obtained by standard methods (Ig/TCR and BCR/ABL transcript quantification). The diploma thesis deals with the emotional experience of patients with chronic renal failure. It aims to explore positive and negative emotional experiences, the presence of depressive and anxiety symptoms, the degree of anger and alexithymia in patients with chronic renal failure undergoing hemodialysis. The theoretical part summarizes previously published theoretical knowledge in this area and the empirical part describes the statistical measurement and comparison of selected variables of emotional experience in the research sample. The theoretical part is divided into 4 chapters, the first of them deals with the medical aspects of chronic renal failure and its treatment. The second chapter deals with the coping with illness, defines psychological aspects of chronic disease and describes the patient's subjective perception of the disease and attitudes towards the disease. The third chapter summarizes the definitions of emotions and emotional experience and defines the most respected theories of emotion. Contributes to its overall severity are extrapulmonary manifestations, especially cachexia and loss of lean tissue mass. Chronic inflammation of the respiratory airways and increased respiratory effort cause the hypermetabolic state to some patients with COPD. This thesis investigates the impact of increased metabolism on body composition in patients with COPD. Our study included 50 COPD patients (38 men, 12 women) who were examined by bioelectrical impedance. In this study, we compared the parameters of body composition of men with resting energy expenditure REE > 130% (hereinafter Men over 130%) (n = 9) and the group of men with REE < 130 % (hereinafter Men below 130 %) (n = 29). The diploma thesis deals with hospitalisation of a preschool child and the importance of education during this challenging period. The goal of the thesis is to find out if parents are aware of the importance of preschool attendance while their children are hospitalised. The thesis also aims to supply teachers and parents with activities they may use to develop and support the child suffering from Perthes Disease. The thesis has been divided into two main parts. Part I is based on expert sources. It contains characteristics of a preschool child and specifies his/her needs and interests during hospitalisation, including the description of family, its function and co-operation with the hospital school. The final section of Part I provides information on Perthes Disease. Part II focuses on the family of the child suffering from the disease. This part is based on qualitative research results. The research investigated the families' attitudes to preschool education of hospitalised children and the importance of family when coping with the difficult life situation. The analysis of the collected answers resulted in a proposal of specific activities designed as a programme supporting education of the child suffering from Perthes disease. The programme is included in the final section of the thesis.

Využití koordinované rehabilitace v organizaci Fokus / The Use of a Coordinated Rehabilitation in the Organisation of the Focus

The diploma thesis is dedicated to the way of using and way of functioning of the coordinated rehabilitation and the multi-disciplinary team in the organization Focus in Pelhřimov which works with the people with mental disorders. The thesis is divided in the theoretical and practical part. The theoretical part describes the history and function of the coordinated rehabilitation, mental health care history, psychiatric reform and multi-disciplinary cooperation functioning. There is used the strategy of quality analysis in the research part. It was done by the questionnaire and the technique of half-structured interview. The results coming out of the interview were focused on the functioning of multi-disciplinary team in the institution and the cooperating subjects in the first part. The second part was dedicated to the basic parts of the coordinated rehabilitation provided in the organization in view of the fact that there were other participating professionals and non-formal participants during the care of the person with mental disorder. The aim of the research was to map the sectors of the coordinated rehabilitation used by the individuals with mental disorders in the organization Focus in Pelhřimov. The research team was created with the help of the method of the snow ball and basic criteria was employment of an "communication partner" in the particular organization. Their ability to evaluate their job and take part in the lead interview depended on their experience in Focus. In connection with the thesis aim there was also set the main research question: What parts of the coordinated rehabilitation are used in the organization Focus in Pelhřimov? Partial questions were devolved from that which were connected to the particular parts of the coordinated rehabilitation and the function of the multi-disciplinary team. The first contact and the next cooperation is developed by the seriousness, necessity and wishes of the client. The work with the client with mental disorder is based on the philosophy of recuperation, not recovery. In the relationship and the concentration, the work is done in all sectors of the coordinated rehabilitation. The client has the closest relationship to the key worker who provides the care in the multi-disciplinary team and other professionals in the organization. There is the conclusion from the results that there was an agreement that the coordinated rehabilitation is used in this institution and no other from the basic sectors can be omitted. The results of the diploma thesis can be used as feedback for the organization Focus in the practice where it can be useful for mapping of the sectors of coordinated rehabilitation and making clear the needs of the individuals with mental disorder. Taking into consideration the recommendation to use the feedback of the respondents about information sharing, to prefer common space of the whole team and bigger promotion.

This diploma thesis presents an analysis of speech impairments accompanying Parkinson's disease. This paper is divided into two sections - theoretical and practical. The first section gives a brief overview of available Czech and foreign literature and articles about this neurodegenerative disease. The section examines the questions of its aetiology, symptomatology, diagnostics and treatment. There can be found also a description of speech impairment connected with this disease, which is mainly hypokinetic and hyperkinetic dysarthria and dysphagia. It also deals with the problem of other limitations in communication such as facial bradykinesia or speech intelligibility. In the last chapter of theoretical part is described speech therapy and intervention with the emphasis on therapy, diagnostics and the effects of pharmacology an non-pharmacological treatment such as deep brain stimulation on speech performance in Parkinson's disease. The research part analyses speech impairment of people with Parkinson's disease. The aim of this diploma thesis was examination of this speech impairment using the Test 3F: Dysarthria profile and patient's own perception of this specific speech disorder. The research was conducted by a qualitative method.The sample consisted of 8 respondents.The aim of the study was to determine whether practitioners know how to proceed in case of a patient with a highly contagious disease in their office.This diploma thesis could serve as a source of information, whether the Czech Republic meet the requirements formulated by the World Health Organization in the International Health Regulations 2005.
