Genetické a klinické aspekty syndromu neklidných nohou / Genetic and clinical aspects of the restless legs syndrome

Introduction: The Restless Legs Syndrome (RLS) is a frequent neurological disorder with a prevalence ranging from 5 - 10%. RLS is characterized by an urge to move the lower extremities during the night, thus RLS causes sleep disturbance. It presents as both idiopathic and secondary form. Idiopathic RLS is associated with common genetic variants in MEIS1, BTBD9, PTPRD and MAP2K5/SCOR1. Recently, multiple sclerosis (MS) was identified as a common cause for secondary RLS, the prevalence of RLS in patients with MS ranges from 13.3 to 37.5%. The aim of our study was to analyse the clinical and genetic aspects of this disorder, especially in patients with multiple sclerosis. In the clinical part, we evaluated the prevalence of RLS among Czech patients with MS and we compared the extent of brain damage between patients with and without RLS using magnetic resonance imaging (MRI). In the genetic part, we further analysed the impact of known genetic variants (MEIS1, BTBD9, MAP2K5/SCOR1, PTPRD) for RLS in other European populations and in patients with MS. Title: Effects of dance sport in the musculoskeletal system of the individual Objectives: The goal is to musculoskeletal examination of selected individuals and evaluation the effect of dance sport on examined status. The results should be the basis for further research in this area. Methods: Kinesiology analysis, bioimpedance analysis and X-ray were used for scrutiny of the musculoskeletal system. Primary and secondary data were analyzed afterwards. Results: Findings of struktural deformities: flat feet (both), both sided hallux valgus with incipient degenerative changes of I. MTP joint (male), limit angle of I. MTP joint of left leg (female). These deviations are related to the using of dance shoes. Muscle imbalance upper thoracic region and lateral imbalance of tested people are the result of the prescribed holding and leading used in dance. This dissertation thesis is pointed to production of lightweight table and bed legs. The subject of the work is to design a lightweight table legs, which is suitable for small businesses with basic machinery. The proposal was drawn up on the basis of information obtained from research developed on furniture market. The base condition of the proposal is the use of laminated chipboard with resolving sharp edges when combined joint of miter. Specifically, the foot was designed for manufacturing companies DSP furniture. Thanks to the cooperation with the company were made 3 kinds of feet with different types of connections and two dining tables. In the proposed and designed foot strength of construction was tested. At the tables were tested durability and stiffness of the construction with different options for the connection to table rib and legs. All tests were organized in an accredited furniture laboratory of Mendel University in Brno. The dissertation thesis is divided into two parts. The first part deals with the theory related to the issue of lightweight table legs. The second part is practical, focusing on the production and testing of quality of designed legs. The knowledge and outputs are summarized in the conclusion, part of the dissertation thesis attached drawings documentation.

Title: Functional disorders in the foot by members of the Honor Guard of the Army of the Czech Republic Objectives: The thesis deals with the activities of soldiers Honor Guard and leg problems arising therefrom. The intention of the theoretical part is generally characterize the activity Honor Guard units, anatomical structure and kinesiology feet, including functional and structural pathology. The aim of the study was to analyze how subjective Musculoskeletal problems are most commonly found among soldiers and assess the impact of the standard used by prestige and sports shoes on his feet when the functional aspects of the activities simulating workloads soldiers. Methods: To obtain basic data on individual participants in research is used surveys using non-standardized anamnesis questionnaire. The main research method to develop practical part of the thesis was to strain gauge device (measuring system) Pedar-X detection and evaluation of plantar pressure, force, contact area and contact time. For processing and evaluation of the data obtained was used program Pedar-X Standard version 3.19.30. Physiotherapist laboratory CASRI was carried out by soldiers basic clinical examination. Title: Acquired flatfoot in adults Background: Flatfoot deformity is one of the common diagnoses in the orthopedic field and physiotherapy, its common in adults as acquired, there is still a large incidence of flatfoot in adolescence and many cases of adult flatfoot are frequently presented as residual pediatric flatfoot. The treatment also has wide field according to type of cause. Aims: The aim of this literature review was to enhance knowledge on the persistence of the acquired flatfoot condition in adult's population by determining the causes of flatfoot with contributing factors that affect the medial longitudinal arch and progression of the deformity, and review of the most common treatment. Method: An electronic database search was conducted to obtain articles from relevant journals (from early 1990 to end 2015). The information was collected also from textbooks. Results: Regarding the causes of acquired flatfoot in adult, a good base of evidence stems from a number of articles review, the posterior tibialis tendon dysfunction is the most cause of acquired flatfoot. Endocrine and Metabolic Aspects of Various Sleep Disorders MUDr. Zuzana Vimmerová Lattová Abstract: Recent epidemiological and experimental data suggest a negative influence of shortened or disturbed night sleep on glucose tolerance. However, no comparative studies of glucose metabolism have been conducted in clinical sleep disorders. Dysfunction of the HPA axis may play a causative role in some sleep disorders and in other sleep disorders it may be secondary to the sleep disorder. Moreover, dysfunction of the HPA axis is regarded as a possible causative factor for the impaired glucose sensitivity associated with disturbed sleep. However, data on HPA system activity in sleep disorders are sparse and conflicting. We studied 25 obstructive sleep apnea (OSA) patients, 18 restless legs syndrome (RLS) patients, 21 patients with primary insomnia and compared them to 33 healthy controls. We performed oral glucose tolerance test and assessed additional parameters of glucose metabolism. The dynamic response of the HPA system was assessed by the DEX-CRH-test which combines suppression (dexamethasone) and stimulation (CRH) of the stress hormone system. The presented work deals with the conception of the Eucharist in the Gospel of John. The first chapter is dedicated to terms of the Eucharist, the Last Supper and the Lord{\crq}s Supper and also to the existing division of possible approaches to the sacramentality in the Gospel of John. In the subsequent chapters it deals in detail with five places of the Gospel of John, that are mostly mentioned in connection with the Eucharist. It is the wedding at Cana (2,1-11), the eucharistic section (6,51-58), washing of the feet of disciples (13,1-20), the Vine and the Branches (15,1-17), opening of the side of Jesus (19,34). The work tries to solve the topic (question) in the context as wide as possible including the question of the johannine sources of inspiration. In this way it can be seen the mutual correspondence of these varios places and the deep theological conception of the hole Gospel. The replacement of the institution of the Eucharist in the thirteenth chapter with the washing of the feet and it{\crq}s displacement to {\clqq}the bread of life discourse`` in the sixth chapter is due to the proper evangelist{\crq}s intention. There was an impending danger, that the Eucharist became a magical medicine without any relation to the incarnation and the sacrifitial death of Jesus. These facts have theirs own consequences for the life of the christian community. So the subject of the sacraments has been subordinated to the christological accent.
