The ethical conduct of employees in maternity wards at selected public hospitals in the Western Cape, South AfricaMdivasi, Vuyokazi January 2014 (has links)
Mini-thesis submitted in partial fulfilment of the requirements for the degree
Master of Technology: Public Management
in the Faculty of Business
at the Cape Peninsula University of Technology
2014 / Maternity service in South Africa faces particular problems in the provision of care to birthing mothers. Violence and abuse have been reported and maternity death rates are high, being related to inadequate provision of care (Myburgh, 2007:29). Ethical conduct plays a significant role in service delivery in Midwife Obstetrics Units (MOU) in general. This is of particular importance since every patient, especially pregnant women, should to be handled with the utmost care, respect and dignity. The research problem emanates from nurses’ behaviour towards patients in MOU labour wards, where women continue to be victims of abuse. Ironically, it is regrettable that they are abused by those who are supposed to be their advocates.
The objectives of the study were to assess if nurses in MOU labour wards conduct themselves ethically when dealing with patients, to determine the perceptions of patients towards nurses during child birth stages, as well as to examine factors in maternity wards that may influence a nurse’s performance when dealing with patients.
The study adopted the quantitative research method to answer the research question and data interpretation was based on statistical analysis. This method was deemed to be the most effective for collection of a large quantity of data and numerical (quantifiable) data is considered objective.
A Likert-type questionnaire comprising closed-ended questions was the measurement instrument. This was considered to least inconvenience nurses and postnatal patients to whom these questionnaires were administered. Answer choices were graded from 1 to 4, being strongly agree, agree, disagree and strongly disagree. The population comprised nurses and postnatal patients in MOUs in the Western Cape, South Africa. Consecutive sampling was conducted in two selected MOUs, being Michael Mapongwana (MM) and Gugulethu (GG), with 311 questionnaires being distributed to both nurses and postnatal Patients in these two facilities.
The findings indicated that the ethical conduct of nurses in both MM and GG maternity wards was relatively good. However, some survey findings revealed some unsatisfactory gaps that exist in what both hospitals currently offer to patients in the areas of individual patient care, communication and baby security certainty. Furthermore, the findings indicated that a significant number of patients who chose to make use of MM and GG hospitals, are satisfied with the standard of service received
during their stay. However, there were some discrepancies in terms of senior management service where excellence in the monitoring role emerged as being lacking. There is a need for improvement in the current levels of ethical conduct of nurses in both the MM and GG labour wards. These needs for improvement relate to working conditions, especially linked to the human resource (HR) function, leadership and management functions, and improved monitoring and control mechanisms.

A humanização do cuidado pré e pós-natal em uma organização não-governamental / The humanization of antenatal and postnatal care in a non-governmental organizationAna Paula Campos Lima 16 April 2010 (has links)
In Brazil the humanization of maternity care is marked by different meanings and experiences. Facing such a diversity, a concept of humanization has been elaborated in this study, based on the analysis of documents from the Program for Humanization of Prenatal and Childbirth Care (PHPN), launched by the Ministry of Health in June 2000. The concept herein developed guided this work, which consisted of a qualitative research with an existentialphenomenological approach, held in the light of PHPN, in a non-governmental organization\'s ambulatory that provides a pre- and postnatal care which is self-defined as humanized. The aims of this study were (i) to observe, describe and understand the humanization of such a care, considering the interrelationships of some of the subjects involved in its production (users, collaborators and coordinators) and the environment in which the interrelationships take place, (ii) to understand how the processes of user embracement and promotion of bonding are related to the humanization of care, in the cases where the service studied included such aspects, (iii) and to reflect about elements that help to overcome some of the contradictions that are found in the humanization field. The data were collected through field participant observation, data collection from pregnants\' attendance card and interviews with health professionals related to the service and women there attended. The data analysis demonstrated the axes of the production of care were (i) the user embracement, which appeared in the dimension of professional attitude (herein exposed as a dialogical attitude and/or as a secure attachment figure attitude, according to the gestaltic approach) and in the service reorganization dimension, (ii) the promotion of bonding; and (iii) the comprehensiveness (as understood in Anthroposophy, mainly). The humanization was associated with such processes, which showed themselves to be especially important to the restitution and/or reaffirmation of the integrality condition of users and professionals and to the experience of ampliation and realization of potentialities of these subjects. Finally, the following aspects proved to be essential points to implement the necessary changes which solidify the humanized care: (i) the recognition, the valorization and the institutional legitimation of the health professionals\' personal condition, (ii) the grounding of the production of health care in a humanistic and holistic approach, (iii) and the overcoming of the contradictions identified not only at the level of discourses but also at the existential dimension of each subject that produces and propagates those discourses

Zkušenosti žen s poporodní péčí v českých porodnicích / Women's Experiences of Postnatal Care in Czech Maternity HospitalsVernerová, Karolína January 2019 (has links)
This diploma thesis focuses on the issue of postnatal care in maternity hospitals in the Czech Republic as it is perceived by women who went through the medicalised obstetric birth in the past year. It examines in particular women's expectations concerning the care on the postnatal ward, their experiences of various aspects of care provided, the moments women highlight and the strategies they adopt in case of possible clashes of their own "non-expert" opinions with the regime of particular maternity hospital or "authoritative knowledge" of the staff. The aim of this thesis is to present the authentic experiences of postnatal care and to implant them into broader conceptual framework, especially considering the medical discourse and possible disciplinary techniques which are implemented to strengthen the authority of medical staff and to provide the opportunity for physical and social control of women.

Development of guidelines to improve the uptake and quality of postnatal care in EthiopiaAdeme Tsegaye Adgo 07 1900 (has links)
Post-natal care is a care given to the mother and the newborn during post-natal period. This period is crucial to save the lives of the mother and the newborn. Despite its importance, the utilisation of this service is very low in Ethiopia.
This study was conducted to establish the views and experiences of women and health care workers on post-natal care services and determine the factors influencing its utilisation with the ultimate purpose of developing guidelines to improve uptake and quality of the services in South West Shoa Zone of Oromia Region, Ethiopia.
The objectives of the study were to explore and describe the women’s views and experiences on utilisation of post-natal care service. Furthermore, the study sought to explore and describe health care workers views and experiences on provision of the services and to assess the factors facilitating or hindering to the utilisation of it. Ultimately, the study had intended to develop guidelines that would contribute towards improving the uptake and quality of the post-natal care services.
The study used a qualitative descriptive and explorative research design to address the research questions. Qualitative data were collected from 19 women who gave birth in the past six months and from 24 health care workers providing post-natal care services (health care workers and health extension workers) and coordinating the overall maternal, neonatal and child health services in the study area (Zone and district health officials; primary health care units heads). The study used in-depth interview guides to collect data from the aforementioned study participants. Inductive thematic analysis was performed on the verbatim transcriptions using the Atlas ti version 8 qualitative data analysis software. / Health Studies / D. Litt. et Phil. (Health Studies)

Socio-economic factors contributing to exclusion of women from maternal health benefit in Abuja, NigeriaOyewale, Tajudeen Oyewale 18 February 2015 (has links)
The study was conducted to describe how socio-economic characteristics (SEC) of women affect their utilization of maternal healthcare services in Abuja Municipal Areas Council (AMAC) in Abuja Nigeria.
A non-experimental, facility-based cross-sectional survey was done. Data was collected using structured interviewer administered questionnaire in 5 district hospitals in AMAC. Sample size of 384 was calculated a priori based on the assumption that 50% of the target population utilized maternal healthcare services during their last pregnancy. Equal allocation of samples per facility was done. The ANC register was used as the sampling frame and proportionate allocation of samples per clinic days was undertaken in each facility. Data analysis included descriptive statistics, cross tabulations and measures of inequality. Logistic regression analysis was used to test the hypothesized relationship between socioeconomic characteristics (predictors) and maternal healthcare service utilization.
Other than birth order that showed consistent effect, the results of this study indicated that the predictive effect (predisposing and enabling factors) of the SEC of women included in this study (age, education, birth order, location of residence, income group and coverage by health insurance) on maternal healthcare service utilization were not consistent when considered independently (bivariate analysis) as opposed to when considered together through logistic regression. In addition, the study revealed that there was inequality in the utilization of maternal healthcare services (ante-natal care - ANC, delivery care and post natal care - PNC, and contraceptive services) among women with different SEC, and the payment system for maternal healthcare services was regressive.
Addressing these predictors in the natural co-existing state (as indicated by the logistic regression) is essential for equitable access and utilization of healthcare during pregnancy, delivery and the postnatal period, and for contraceptive services in AMAC, Abuja Nigeria. Targeted policy measures and programme actions guided by these findings are recommended to optimise returns on investment towards achieving national and global goals on maternal health in Nigeria / Health Studies / D.
