Znalosti vybraných studentů Univerzity Karlovy o posttraumatické stresové poruše / Knowledge of selected students of the Charles University of posttraumatic stress disorder

This diploma thesis focuses on posttraumatic stress disorder and the related psychosomatic conditions looking into the diversity of somatic responses to stress factors or a traumatic event. In the opening part, trauma processing mechanism is described as well as clinical changes in the brain after an unprocessed traumatic event. The following chapters describe therapy options in case of an existing posttraumatic stress disorder, accepting the traumatic event as part of a closed personal history and creating an added value in the form of higher resistance. The closing theoretic part deals with psychosomatic medicine. The practical part inquires into knowledge of students of the teaching and medical study programmes regarding posttraumatic stress disorder and compares the individual groups. The survey results discover surprising gaps in the professional knowledge of future physicians and teachers. By comparing the related data, a recommendation is being made that students' knowledge has to be broadened in this respect. About 4 % women in general population and up to 19 % women in at-risk population suffer from PTSD related to childbirth. The main risk factors for postpartum PTSD are negative birth experience and obstetric emergencies. However, the findings about risk factors and prevalence rates vary across studies, which is mainly due to the inconsistency in the diagnostics of postpartum PTSD. Researchers use various PTSD measures that differ in the number and type of symptoms monitored and many of these measures do not cover all DSM criteria for PTSD. Furthermore, the PTSD diagnostic criteria have been significantly modified in the new DSM-5 and the existing instruments need to be revised. The first part of the thesis presents the concept of PTSD related to childbirth. Special attention is paid to the risk factors identified in previous research and to the analysis of PTSD diagnostic and screening measures used in the postpartum context. The aim of the second part of the thesis is to evaluate the psychometric properties of the unpublished Childbirth-Related PTSD Questionnaire on a sample of 620 Czech women. This thesis focuses on the initial validation of the PTSD Checklist for DSM-5 (PCL-5) in its Czech version. It is a self-report method that measures symptoms accompanying posttraumatic stress disorder (PTSD). The theoretical part of the thesis describes the development of the diagnosis of PTSD over time and its current criteria. It also presents a list of diagnostic tools for measuring PTSD, including the PCL-5 and its psychometric properties. The research part of the thesis describes the process of data collection and processing, the results of the hypotheses and discussion. The research sample consisted of 444 respondents (370 women; 74 men) with a mean age of 32,6 years (SD = 10,6). The mean PCL-5 score was 29,3 (SD = 19,9). When comparing the scores of women (M = 30,2; SD = 20) and men (M = 24,7; SD = 18,8), a statistically significant difference was observed (p = 0,029). When comparing respondents who were psychiatric patients (PP; M = 39,5; SD = 20) and those who were not (M = 21,7; SD = 16,6), a significant difference (p < 0,001) was also observed. In case of comparison of respondents by gender who were not PP, no statistically significant difference was confirmed. Psychosociální poradenství pro účastníky dopravních nehod v Českých Budějovicích / Psychosocial counselling for the participants of traffic accidents in České Budějovice

This dissertation focuses on the field of psychosocial counselling intended for the people involved in traffic accidents. Traffic accidents often result in considerable whole-society damages and significant reduction in the life quality of affected individuals or their relatives. Traffic accidents may often represent nerve-racking, traumatic events which significantly go beyond the scope of common human experience. People involved in traffic accidents may suffer from mental problems which might impair an area of their social functioning. As a result, traffic accidents have significant impact not only to the lives of persons directly involved, but also to their relatives. Victims´ self-help may fail and, without the expert intervention, may lead to the whole-life misery. One of possible ways of how to help the people involved in the traffic accident is the psychosocial counselling which is the subject of this dissertation. This thesis, among others, aims to analyse the offer of psychosocial consulting services intended for traffic accident participants in the town of České Budějovice. To this end, the combination of qualitative and quantitative data collection techniques, questionnaire survey of experts and organizations and semi-standardized interviews with experts operating in České Budějovice were carried out. Other goals of the thesis included mapping of public awareness concerning the possible utilization of psychosocial counselling for traffic accident participants in České Budějovice, as well as, mapping the interest in using such services on the public{\crq}s part in this town. A public questionnaire survey was performed to reach these objectives. The findings revealed that there is a public demand for services of an organization providing the people involved in traffic accidents with psychosocial counselling. The public awareness of the possibility of using psychosocial counselling in this area, however, proved to be low in České Budějovice. The survey has also shown that the offer of psychosocial counselling focused on traffic accident participants does exist in this town. The dissertation may contribute to higher awareness of not only the professional employees who provide the assistance (psychologists, social workers, non-profit organization employees, physicians, police officers, fire fighters), students of humanities and volunteers involved in giving psychosocial assistance to the victims of disasters, but also of those people involved in traffic accidents whom anyone of us may become.

Introduction: There is a frequent incidence of post-traumatic stress disorder (PTSD) in people with substance use disorder (SUD) and there is also a frequent occurrence of substance use in people with PTSD. Many foreign authors highlight the complications that occur in individuals with this dual diagnosis: higher risk of drop-out from treatment, a higher number of suicide attempts, more serious consequences of substance use, more frequent relapses of both of two diagnoses, etc. The negative impact of this comorbidity on treatment outcomes, highlights the need to focus on improving procedures in the diagnosis of PTSD in people treating from SUD and a need of integrated treatment of both disorders. Many studies also suggest that improving the condition of PTSD increases the likelihood of improvement in substance use. In the Czech Republic, there is a lack of sources that focus on the relationship of these comorbidities and their treatment. Examples of a good practice are also missing. Aims: The aim of this thesis is to introduce the issue of dual diagnosis - PTSD and SUD. The thesis deals with the psychological aspects of stress in police work from the perspective of police psychologists. Firstly, key terms and psychological aspects of police work are described in the theoretical part. Subsequently, a chapter of individual aspects of stress in police work is introduced. The aspects are divided into four separate groups - organizational stress, stress and lifestyle, stress and family life and borderline situations in police work. The following chapter deals with psychological care for police officers. The research part of this work brings findings obtained qualitatively in in-depth interviews. A wide range of themes arose - exceeding bureaucracy and control, incessant need to keep up with the police role, limited career growth and the absence of meaning leading to burn-out. In the context of family life, themes of alienation, infidelity or other common family problems were registered. Alcohol abuse as an escape strategy was mentioned. Borderline and traumatizing events are marginal matter according to participants. A range of relevant themes arose - encounter with death, children victims, car accidents and suicide among police officers.

The aim of this thesis is to describe the stress factors and burnout, particularly in ambulance firefighters. In the theoretical section provides basic information on the Fire Rescue Service of the Czech Republic, the specifics of their work and are also given information regarding stress, posttraumatic stress disorder , burnout and psychological services to the Fire and Rescue Service of the Czech Republic. This is complemented by the findings of other states. The practical part is focused on the processing and evaluation of a questionnaire survey to map the most important factors of stress and burnout among staff of the Fire Rescue Corps of the Karlovy Vary Region and the Central Region.

Faktory ovlivňující názor adolescentů na problematiku domácího násilí / Factors influencing the opinion of adolescents about the issue of domestic violence

Abstract The diploma thesis on the theme ?Factors influencing adolescents´ opinion on issues of the domestic violence? deals in the theoretical part with all important aspects of the domestic violence and stalking. The research part has been focused on adolescents aged 17 to 19 years living in Vysočina region, the research sample has consisted of 858 respondents. The aim of the thesis was to find out, by statistical methods on a statistically significant sample of adolescents, key factors influencing the adolescents´ opinion on the issue of the domestic violence. In relation to the aim of the thesis there were determined following hypotheses, which were tested on 5% significance level with six survey questions. Hypothesis 1: ?Statistically significant factor for the domestic violence among adolescents is their personal experience? was confirmed in four areas. Respondents having the personal experience with the domestic violence differ from the respondents without such experience in the following: they are more aware of a possibility of a partner´s violence at their age, they more refuse to tolerate the domestic violence, they better recognize a definition of the domestic violence and they are often aware of a possibility of a trans-generation transmission of the violence. In connection with this hypothesis it is important the finding, that 57 respondents (7%) have their personal experience with the domestic violence; 49 girls and 8 boys. Hypothesis 2: ?Statistically significant factor for the domestic violence among adolescents is the education? was not confirmed due to a low variability of responses and insufficient data to evaluate the hypothesis. Hypothesis 3: ?Statistically significant factor for the domestic violence among adolescents is the gender (women)? was confirmed in two areas. Girls differ from boys in the following: they are more aware of a possibility of the partner´s violence at their age and they more refuse to tolerate the domestic violence. The diploma thesis can serve as a source of current information related to the issue of the domestic violence, it provides an up-to-date data on the prevalence of the domestic violence in the adolescence and it presents information and opinions of adolescents on the domestic violence or partner relationship. The thesis can be used by professionals as well as lay public interested in the domestic violence and it can be a basis or a comparison for other studies and researches.

The main aim of this thesis was to gather information concerning psychological aspects in the field of critical management from available sources and to introduce a reader- friendly survey of this topic at the same time. Basic terms were explained, as well as various psychological dispositions of critical staff members and, last but not least, fundamental forms of psychological assistance with regard to citizens in extreme life situations. The psychological awarness survey of both critical staff members and people affected, was carried out by means of a public survey. The idea behind was to demonstrate up-to date level of psychological awarness in particular regions of the Czech Republic.
