Background: Pregnancy associated cardiovascular pathologies have a significant impact on outcome for mother and child. Bioimpedance cardiography may provide additional outcome-relevant information early in pregnancy and may also be used as a predictive instrument for pregnancy-associated diseases. Methods: We performed a prospective longitudinal cohort trial in an outpatient setting and included 242 pregnant women. Cardiac output and concomitant hemodynamic data were recorded from 11th-13th week of gestation every 5th week as well as at two occasions post partum employing bioimpedance cardiography. Results: Cardiac output increased during pregnancy and peaked early in the third trimester. A higher heart rate and a decreased systemic vascular resistance were accountable for the observed changes. Women who had a pregnancy-associated disease during a previous pregnancy or developed hypertension or preeclampsia had a significantly increased cardiac output early in pregnancy. Furthermore, an effect of cardiac output on birthweight was found in healthy pregnancies and could be confirmed with multiple linear regression analysis. Conclusions: Cardiovascular adaptation during pregnancy is characterized by distinct pattern described herein. These may be altered in women at risk for preeclampsia or reduced birthweigth. The assessment of cardiac parameters by bioimpedance cardiography could be performed at low costs without additional risks.

Considerando-se a estreita relação entre o psíquico e o somático, o conhecimento das fantasias inconscientes por parte dos profissionais envolvidos no tratamento delas, pode favorecer um melhor e mais completo atendimento dessas gestantes. OBJETIVO - Investigar sobre as fantasias inconscientes da gestante portadora de hipertensão arterial crônica. Estudo prospectivo e qualitativo, de 35 gestantes hipertensas com idade entre 20 e 35 anos, idade gestacional inicial entre 16 e 36 semanas, Para o estudo das fantasias inconscientes foi utilizado a técnica projetiva de Desenho-estória com tema. Predominam fantasias construtivas e amorosas, destrutivas e persecutórias e fantasias de caráter onipotente. As fantasias amorosas expressaram-se como necessidade de fortalecimento do objeto bom interno, procura de harmonia e de equilíbrio na personalidade, inserção do bebê na história de vida do casal e de valorização do ambiente. As fantasias destrutivas e persecutórias caracterizaram-se por angústias de ser possuidora de objetos maus internos, de ser destrutiva. It is a moment that is, at the same time, rich and painful, influential and unforgettable and that sends the pregnant woman to her own origin. There are many common symptoms in the gestation, and despite provoking a reasonable perturbation, they can be lived with pleasure as they make the existence of the pregnancy evident. This way, the pregnant woman can live such moments with relative tranquility. The same does not happen when the pregnant woman carries any pathology considered of great risk, which determines a sense of threat that results in apprehension and insecurity as it is the case of high bloodpressure. Considering the strait relation between the psychic and the somatic, the knowledge of the unconscious fantasies by the professionals involved in the care of these women can favor a better and more complete service to these pregnant women. OBJECTIVE - To investigate the unconscious fantasies of the pregnant woman that carries chronic high blood-pressure. Qualitative observational, prospective study of 35 hypertensive pregnant women with ages varying from 20 to 35 years-old, gestational period between 16 and 36 weeks. The projective technique of the drawing-story was used for the study of the fantasies. Loving and constructive fantasies and destructive and persecuting fantasies of unconscious type predominate. Loving fantasies expressed themselves as the need of strengthening of the good inside object, the search for harmony and balance in the personality, the insertion of the baby in the life story of the couple and the valuing of the environment. The destructive and persecuting fantasies were noticed by the angst for being the possessor of bad inside objects, for being destructive. The unconscious fantasies of the omnipotent...
