Evaluating and expanding knowledge and awareness of health professionals on the consumption and adverse consequences of Novel Psychoactive Substances (NPS) through innovative information technologic tools

Background: The rapid diffusion of Novel Psychoactive Substances (NPS) constitutes an important challenge in terms of public health and a novelty in clinical settings, where these compounds may lead to erratic symptoms, unascertained effects and multi-intoxication scenarios, especially in emergency situations. The number of NPS available on the illicit drug market is astonishing: official reports suggest the appearance of a new drug every week. NPS may be enlisted in many different families such as synthetic phenethylamines, tryptamines, cathinones, piperazines, ketamine-like compounds, cannabimimetics and other plant-derived, medical products and derivatives. Therefore, healthcare services and professionals are often called to face this unknown 'galaxy' where NPS users seem to perceive traditional services 'unfitting' for their needs, requiring an attention which is quite different from known classic drug abusers. In this context, the Recreational Drugs European Network (ReDNet), a research project funded the European Commission and led by the University of Hertfordshire, aimed to explore the NPS galaxy and develop information tools for vulnerable individuals and professionals working with them. This initiative reported specific Technical Folders on new drugs and disseminated the collected information through innovative communication technologies (e.g. multimedia tools, social networking and mobile phone services) internationally. Aim and objectives: The aim of this work is to evaluate and contribute to expand the knowledge of health professionals on NPS. The key objectives are: 1) to assess the level of knowledge on NPS amongst a sample of Italian healthcare professionals; 2) to evaluate the effectiveness of dissemination tools developed by ReDNet, including an SMS-Email/mobile service (SMAIL); 3) to understand the clinical impact of NPS by providing four Technical Folders and collecting two clinical cases on NPS. Methodology: According to the objectives, the methodological approach has been articulated in the following three phases. Phase 1: investigating knowledge and preferred channels of information via an online survey among health professionals in Italy. This first Italian study on NPS awareness had been online from February to July 2011, recruiting participants from Departments of Addiction, Psychiatry and other services. Phase 2: evaluating the ReDNet initiative. An evaluation questionnaire was designed and disseminated online to assess the various resources provided by ReDNet project; it had been online from April to July 2013, targeting professionals registered to ReDNet services. This phase also investigated the SMAIL service, a mobile application that was the latest technological tool developed by ReDNet team. Phase 3: promoting evidence based work in clinical practice through the preparation of four Technical Folders and two case reports. Technical Folders followed the methodology optimised during the ReDNet experience, organising NPS data under specific headings, measured for the need of health professionals. Case reports were collected in a Dual Diagnosis Unit in Italy ('Casa di Cura Parco dei Tigli'); assessed patients revealed for the first time the use of NPS; clinical interviews were conducted to collect a full anamnesis while for the first time psychopathological characteristics were measured in NPS abusers, using a psychometric instrument (MMPI-2). Results: In Phase 1 Italian services, in particular interviewees (n=243) from Departments of Psychiatry and Addiction, showed a strong interest for the subject but a poor understanding of NPS: 26.7% of respondents did not know if their patients ever used NPS; at the same time they considered this phenomenon as very relevant to their profession (e.g. psychomotor agitation [75.7%], errors in the assessment [75.7%], management of the clients [72%]); in addition less of a quarter of them had reliable information on new substances. Interviewees also reported the need for easily accessible channels of information to expand their expertise in the field (including emails [70%] and dedicated websites [51.9%]). The ReDNet initiative (Phase 2) reached professionals (n=270) from European countries and various other regions; they appreciated the website above all (48.5%), which provided access to other information (in form of academic papers, news, technical folders, etc.). The integration of technological-based and classic educational resources was used to self-educate professionals (52.6%) and supply information for research (33.7%) with up-to-date and 3 reliable information; in the same Phase the SMAIL service was analysed in its first 557 searches: in the pilot period 122 professionals used SMS inquiries (95%), asking information on NPS while highlighting the increasing number of NPS available on the market. Technical folders (Phase 3) described two new phenethylamines (Bromo-dragonfly and 25I-NBOMe), a novel ethno drug (Kratom) and a new synthetic cathinone (alpha-PVP) whose severe effects were also described in one of the clinical cases. The first case report (Alice) involved a clubber who used mephedrone and other NPS with a severe worsening of her psychiatric disturbances; the second one (Marvin) described a patient who was referred by a psychiatric service and revealed himself as a 'psychonaut' with an intense abuse of alpha-PVP. Conclusions: The exploration of the NPS galaxy is a new challenge for healthcare professionals. In this study, Italian services seemed to be unprepared to face the emergency and requested rapid access to reliable information; the ReDNet project provided both technology-based and traditional resources to expand knowledge on NPS, making professionals more aware of emerging issues and helping especially clinicians working in the field (e.g. via SMAIL service and Technical Folders). Overall, it can be observed that effective information services on NPS targeted at professionals initiatives should include an online interface integrating up-to-date information, describing NPS through specific Technical Folders and disseminating scientific literature; the use of technological tools, including mobile applications, is an important strategy to support health professionals in their activity. Finally, more 'visual' guidelines, possibly in the form of a 'map' of these heterogeneous compounds, could be a useful framework to describe NPS to physicians and other professionals who are often unprepared and unconfident to face such an expanding galaxy.

No. of bitstreams: 0 / Resveratrol (3, 4, 5-trihidroxiestilbeno) é um polifenol produzido por uma grande variedade de plantas em resposta ao estresse e encontrado predominantemente em cascas de uvas. Este princípio ativo apresenta vários benefícios à saúde, como a capacidade antioxidante, relacionada à prevenção de diversos tipos de câncer e do envelhecimento precoce da pele. No entanto, apresenta baixa biodisponibilidade quando administrado por via oral, o que torna interessante sua aplicação tópica. O principal objetivo deste trabalho foi a incorporação de resveratrol em hidrogéis poliméricos para obtenção de um sistema de liberação utilizado topicamente contra o desenvolvimento de desordens cutâneas, como o envelhecimento cutâneo e o câncer de pele. As matrizes poliméricas compostas por poli(N-vinil-2-pirrolidona) (PVP), poli(etileno glicol) (PEG) e ágar ou PVP e propano-1,2,3-triol (glicerina) e irradiadas a 20 kGy foram caracterizadas pelos ensaios de fração gel e intumescimento; sua biocompatibilidade preliminar foi avaliada in vitro por meio do ensaio de citotoxicidade utilizando o método de incorporação do vermelho neutro. Devido à baixa solubilidade do resveratrol em água, verificou-se o efeito da adição de 2% de etanol às matrizes. Todas as matrizes estudadas, contendo ou não álcool, apresentaram alto grau de reticulação, capacidade de intumescimento e não apresentaram toxicidade em ensaio preliminar de biocompatibilidade. Os dispositivos foram obtidos pela incorporação de resveratrol nas matrizes poliméricas, realizada de forma direta e indireta, ou seja, antes e após irradiação, respectivamente. Os dispositivos obtidos pelo método direto foram submetidos aos ensaios de fração gel, intumescimento e citotoxicidade e apresentaram-se semelhantes às respectivas matrizes. Os dispositivos contendo 0,05% de resveratrol obtidos pelo método direto e os dispositivos contendo 0,1% de resveratrol obtidos pelo método indireto foram submetidos ao ensaio de cinética de liberação durante 24 h. A quantificação do resveratrol liberado foi realizada por cromatografia líquida de alta eficiência (HPLC). Nos últimos 40 anos o uso dos hidrogéis têm sido investigado para diversas aplicações como curativos. Neste trabalho foram sintetizadas membranas de hidrogéis com poli(N-2- pirolidona) (PVP), poli(álcool vinílico) (PVAl), quitosana e argila laponita em encapsulamento do fármaco para liberação controlada de glucantime sobre a superfície cutânea de tecidos lesados por leishmaniose. O tratamento tradicional dos pacientes infectados pelos parasitas é feito com antimoniato pentavalente de forma injetável. Entretanto estes antimoniatos são muito tóxicos e provocam efeitos colaterais nestes pacientes, além disso, pacientes portadores de doenças cardíacas e renais não podem fazer uso deste tratamento. No tratamento com membranas de hidrogéis aplicadas na superfície de tecidos lesados pela leishmaniose, o fármaco é liberado diretamente no ferimento de forma controlada, diminuindo os efeitos colaterais. As membranas preparadas neste trabalho foram caracterizadas por difração de raios X (DRX), análise de termogravimetria (TG), intumescimento, fração gel, espectroscopia no infravermelho (FTIR), microscopia eletrônica de varredura (MEV) e microscopia de força atômica (AFM). As caracterizações funcionais foram feitas com teste de citotoxicidade e de liberação do fármaco in vitro e in vivo, de acordo com o protocolo de ética do Instituto de Medicina Tropical do Hospital das Clinicas da Faculdade de Medicina da USP. O teste \"in vivo\" dessas membranas provou ser eficiente na liberação controlada de fármacos diretamente nas superfícies lesadas pela leishmaniose. No. of bitstreams: 0 / O desenvolvimento de novos procedimentos e tecnologias tem proporcionado um grande avanço no tratamento de feridas e queimaduras, melhorando a qualidade de vida das vítimas e reduzindo as taxas de mortalidade de pessoas com queimaduras graves. Entretanto as complicações infecciosas continuam sendo um desafio e uma das principais causas de óbito de queimados. Por outro lado, em todo o mundo tem-se observado um crescente interesse no uso de hidrogéis para aplicação como curativos para queimaduras, ferimentos e úlceras de pele, tendo em vista que são capazes de absorver exsudatos, ajudam na cicatrização e proporcionam conforto ao paciente, uma vez que favorecem o alívio da dor. Além disso, os hidrogéis também podem ser aplicados como matrizes para sistemas de liberação controlada de princípios ativos e agentes antimicrobianos. O objetivo deste trabalho foi estudar as propriedades mecânicas e físico-químicas de hidrogéis compostos por blendas formadas por Poli(N-vinil-2-pirrolidona)/ Polietilenoglicol/ ágar (PVP/PEG/ágar), Poli(N-vinil-2-pirrolidona)/ glicerol/ ágar (PVP/glicerol/ágar); Poli(álcool vinílico)/ kappa-carragena/ ágar (PVA/KC/ágar), reticuladas por radiação ionizante. Para a preparação das blendas, utilizou-se planejamento de misturas como ferramenta, oferecendo assim subsídios a futuros desenvolvimentos de novos hidrogéis, que possam ser usados como curativos e como matrizes poliméricas para liberação de ativos hidrofílicos e lipofílicos. Foram otimizadas três formulações e sintetizadas seis, três das quais hidrogéis nanocompósitos de prata. As nanopartículas de prata (NPAg) foram sintetizadas, in situ, por radiação gama, sem uso de catalisadores ou outro reagente para obtenção de curativo mais puro e com ação antimicrobiana. A caracterização físico-química dos hidrogéis foi obtida por análises de fração gel, intumescimento, ensaios de tração e perfuração, microscopia eletrônica de varredura (MEV), espectroscopia na região do ultravioleta-visível (UV-vis). Também foi avaliada a atividade antimicrobiana dos hidrogéis e realizado estudo de citotoxicidade in vitro. O planejamento de misturas apresentou modelos confiáveis para os resultados de fração gel e intumescimento, porém, para as propriedades mecânicas, os resultados devem ser vistos com cautela. O uso da radiação se mostrou eficaz para a síntese de NPAg in situ nas blendas estudadas. Os resultados sugerem que os hidrogéis sintetizados não liberaram NPAg, porém os hidrogéis de PVP apresentaram atividade bactericida para S. aureus e P. No. of bitstreams: 0 / De acordo com o conceito de sistemas de liberação controlada, o presente estudo foi baseado na utilização de polímeros hidrofílicos biocompatíveis, formadores de hidrogéis, para o desenvolvimento de matrizes na forma de filmes finos. Os polímeros utilizados para a formação das matrizes foram a quitosana proveniente das cascas de camarão, o amido de milho modificado e a poli(N-vinil-2-pirrolidona) - PVP. As matrizes foram reticuladas utilizando glutaraldeído. O fármaco escolhido para testar a capacidade de liberação dos dispositivos foi o anti-inflamatório não esteroidal (AINE) diclofenaco sódico. Para obtenção das matrizes com propriedades adequadas para essa finalidade, foram testadas misturas de quitosana-amido e quitosana-PVP. Após a triagem qualitativa, os dispositivos foram avaliados quanto à citotoxidade, intumescimento máximo, fração gel, parâmetros cinéticos associados à absorção de vapor de água e à capacidade de liberação de diclofenaco sódico in vitro. As formulações de quitosana-PVP foram as que apresentaram melhores propriedades para a aplicação proposta nesse estudo, se destacando a formulação A3, com alto percentual de liberação, boas propriedades de manuseio, poucos componentes na formulação diminuindo o potencial alergênico e aprovação no teste de citotoxicidade em células de camundongo (NCTC) pelo método de incorporação do vermelho neutro. / Dissertação (Mestrado em Tecnologia Nuclear) / IPEN/D / Instituto de Pesquisas Energeticas e Nucleares - IPEN-CNEN/SP

<b>Source Sink Regulated Senescence in Maize: </b><b>Yield Impacts, Genetic Architecture, and Physiology</b>

<p dir="ltr">Uncovering the mechanisms of senescence in maize will give us a deeper understanding needed to drive future yield increases. Previous work on senescence response to sink disruption has identified a set of genes and biochemical mechanisms. Still, little is understood about the impact of this phenotype on yield and other commercially relevant traits. Uncovering the genetic basis of senescence in maize and testing the effect of these alleles on yield will provide a mechanistic framework for considering this trait to drive future yield increases.</p><p dir="ltr">Ear removal experiments demonstrated that senescence timing is insensitive to the presence or absence of an ear outside a critical window from 10 to 45 days after pollination. Nitrogen fertilization did not impact the SSRS response measured in the upper canopy. In further characterizing the SSRS phenotype, we have provided a spatial and temporal map of the B73 senescence response to sink disruption from the top of the plant to the ear leaf and discovered that this phenomenon is dose dependent and proportional to the size of the sink across two genotypes and years. This relationship was successfully used to predict kernel numbers and grain weight from spectral leaf properties as early as 4 weeks after pollination using remote sensing under agronomic conditions.</p><p dir="ltr">A population of 343 exPVP inbred lines was evaluated for source-sink regulated senescence and hybrid testcrosses were made for a subset of 200 inbred lines to testers for measurement of yield and ear photometry phenotypes. Source-sink regulated senescence of inbred parents was correlated with the yield of intra-family hybrids but was not generally correlated with the yield of hybrids made from crosses between two heterotic groups. The presence of multiple significant SNP association at the Bonferroni-corrected threshold at loci that are associated both with kernel traits and SSRS suggests shared genetic regulation of two traits that is likely driving the observed trait correlations of SSRS with kernel size and yield.</p><p dir="ltr">The maize nested association mapping (NAM) parents reveal a previously unknown breadth of SSRS phenotypes in the global diversity of maize germplasm. Mapping genes for SSRS in the NAM populations supports previously reported loci with large, dominant effects as well as evidence for previously unreported modifiers that are capable of suppressing the dominant alleles and producing a quantitative distribution in SSRS phenotypes. There are distinct alleles within sub-populations worth further study such as sweetcorn populations with non-senescence responses to sink disruption. A multi-factor analysis for QTL mapping, GWAS, and mutant variant sequencing identified highly significant loci on chromosomes 1 from 30.4Mbp to 35.8Mbp, chromosome 2 from 183.2Mbp to 190.8Mbp, chromosome 4 from 38.2Mbp to 134.8Mbp (crosses a centromere), chromosome 5 from 140.8Mbp to 233.9Mbp, chromosome 8 from 112.5Mbp to 123.8Mbp, and chromosome 8 from 155.7Mbp to 163.9Mbp. Candidate genes co-located with Bonferroni SNP in these regions may contribute to SSRS phenotypes through regulation of autophagy, accumulation of flavonoids, and sequestration of sugar in cell walls as an alternative sink. It is possible that co-regulation of these genes could cause all of them to be involved in the stress response of B73 to sugar accumulation. To find the causal variants for these traits, fine mapping and comparisons of near-isogenic lines will be required to narrow the list of candidate genes. Uncovering the alleles responsible for SSRS in global maize diversity could provide the building blocks for a physiological approach to increasing yields through optimizing the senescence responses to elevated sugar levels during grain-fill.</p>

Engineering of Pharmaceutical Particles : Modulation of Particle Structural Properties, Solid-State Stability and Tabletting Behaviour by the Drying Process

<p>Relationships between stresses during the drying process, particle structural and functional properties, and particle engineering by the drying process were addressed in this thesis. In the first part, the importance of the drying phase and the effect of the drying rate on the intragranular porosity of microcrystalline cellulose pellets were investigated. Differences in porosities of dried pellets could be explained by liquid-related differences in densification during convective drying rather than by differences in densification during wet agglomeration. An increased drying rate gave more porous pellets with a lower compression shear strength, and thereby stronger tablets. The next part dealt with modulation of solid-state stability and tabletting behaviour of amorphous lactose by incorporation of different polymers by spray drying. Increased content and molecular weight of poly(vinylpyrrolidone) (PVP) resulted in an increased resistance to crystallisation provoked by heat and moisture. The stabilising effect was even more evident after long-term storage. However, the glass transition temperature was almost unaffected and may, therefore, be questioned as a stability indicator for these types of materials. The presence of the polymers resulted in somewhat less deformable particles. Incorporation of PVP increased the compactability, whilst a surfactant decreased it, which could be shown to be related to differences in particle-particle adhesivity between the different particles. This thesis contributes to increased mechanistic understanding in the area of particle engineering that may lead to better prediction and optimisation of the functionality of pharmaceutical particles, which is of the utmost importance in the development and production of solid dosage forms.</p>
