Análise da usabilidade de um dispositivo robótico desenvolvido para reabilitação de tornozelo em indivíduos hemiparéticos pós acidente vascular encefálico / Usability analysis of a robotic device developed for ankle rehabilitation of post-stroke hemiparetic subjects

As doenças cerebrovasculares, incluindo o acidente vascular encefálico (AVE), são a segunda causa de morte no mundo, e o principal causador das incapacidades na população adulta. Devido à reorganização cortical, quanto mais precoce a reabilitação é realizada, melhores são os resultados. Desta forma, novos tratamentos e soluções tecnológicas foram desenvolvidos para enfrentar os desafios da reabilitação pós AVE, por exemplo, aumento da intensidade e duração da terapia incluindo manipulação externa, treinamento do movimento bilateral, e reabilitação robótica. Os dispositivos robóticos possibilitam a realização de tarefas específicas repetidas vezes, de forma controlada e confiável, fator determinante para a facilitação da reorganização cortical, com aumento da habilidade motora e melhora do desempenho das atividades funcionais. O objetivo principal deste trabalho foi verificar a adequação e usabilidade da Plataforma Robótica de Reabilitação de Tornozelo - PRRT em indivíduos que precisam de reabilitação de tornozelo decorrente de sequela de AVE, a fim de realizar eventuais ajustes antes que o equipamento seja incorporado na prática clínica. A metodologia consistiu em verificar a adequação ergonômica e técnica do equipamento, e avaliar a satisfação do usuário após o seu uso por meio de questionário. O aparelho foi avaliado tanto em pacientes com déficit motor de hemiparesia, como em indivíduos sem deficiências. Os resultados experimentais mostram que o equipamento é adequado como um recurso auxiliar na avaliação da amplitude de movimento articular do tornozelo e da força muscular, beneficiando os terapeutas na avaliação dos ganhos reais obtidos com as terapias. A partir das respostas ao questionário, verifica-se que os indivíduos sentiram-se satisfeitos com o recurso. Portanto, conclui-se que a PRRT é um recurso auxiliar promissor no tratamento do tornozelo de indivíduos que sofreram lesão neurológica, podendo trazer benefícios em relação à amplitude de movimento e ganho de força, além de proporcionar uma terapia prazerosa e estimulante aos pacientes. / Cerebrovascular diseases, including stroke, are the second leading cause of death worldwide and the leading cause of disability in the adult population. Due to cortical reorganization, the earlier rehabilitation is performed, the better the results. In this way, new treatments and technological solutions were developed to meet the challenges of rehabilitation after stroke, for example, increase of intensity and duration of therapy including external manipulation, bilateral movement training, and rehabilitation robotics. Robotic devices allow performing specific tasks repeatedly in a controlled and reliable way, a key factor for cortical reorganization, with increase of motor skills and improvement of functional activities performance. The main objective of this work was to verify the adequacy and usability of the Robotic Platform for Ankle Rehabilitation in post-stroke patients who need ankle rehabilitation, in order to make any necessary adjustments before the equipment be incorporated into clinical practice. The methodology consisted in verifying ergonomic and technical adequacy, and evaluates user satisfaction after use through a questionnaire. The device was evaluated in patients with motor deficit of hemiparesis, as well as in individuals without disabilities. Experimental results show the proposed equipment is suitable as an auxiliary resource in evaluating the range of motion and muscle strength of the ankle, assisting therapists in the evaluation of real gains obtained with the therapies. From the questionnaire responses, the individuals said they were satisfied with the resource. Therefore, it is concluded that the PRRT is a promising auxiliary resource in the treatment of individuals who have suffered neurological damage, bringing benefits with relation to range of motion and muscle strength, as well as providing pleasurable and stimulating therapy to patients.

Vipassana Meditation and Teacher Decision-Making

The purpose of this grounded theory study was to build a theory about the process and function teachers construct around the effect Vipassana meditation has on stress, teaching, and decision-making. This study addressed the problem of how teachers respond to daily tasks and demands that can negatively impact their longevity in the profession. The starting point was the conceptual framework, including resolving cognitive dissonance, choice theory, mindfulness, and the perspective of Vipassana meditation. The research questions addressed how Vipassana meditation influences a teacher's daily routine, decision-making, classroom management, general procedures, and stressful situations. The data collection was done in 2 stages and included triangulation through 2 interviews, journals, and a questionnaire for all 9 participants. The analysis used pre, open, axial, and selective coding with both inductive and deductive processes which connected the conceptual framework to emerging concepts including equanimity, awareness, observation, context, detachment, nonjudgment, flexibility, being present in the moment, and engagement. Using these concepts, a possible theory involving the anicca perspective (one of non-permanence) on the decision-making process and as a stress management tool was generated. Implications for positive social change include a demonstrable positive effect on relationships in the classroom, pedagogy, and classroom management. This process can be considered in teacher training and professional development programs to decrease stress in order to help prolong teachers' careers.

The application of learning organization principles to church growth

While many studies showed evidence of the use of learning organization theory in a variety of venues, these theories have been studied in a limited capacity in church settings. This research attempted to substantiate the presence of learning organization principles in churches experiencing growth, and to refine a tool to measure these characteristics in churches. Relationships and strengths of association between and among 3 learning organization principles of leadership, job structure and systems, and performance and development, and degrees of growth defined as negative, plateau, and positive growth were examined in a sample of Nazarene churches via a revised survey completed by senior pastors. Pre and post survey analyses were employed, resulting in stronger reliability and validity outcomes for the instrument and contributing to a significant gap in the literature. Correlation, multiple regression, and ANOVA methods were used to assess relationships between the 3 learning organization principles and 3 levels of church growth. Outcomes did not show significant substantiation of these relationships, except for slightly higher evidence of leadership in the positive growth group. This study adds to the scientific knowledge of church growth via the creation of a new survey instrument for church use. The promotion of social responsibility and professional application of knowledge to church venues is an important tenet of this study, and lends valuable insight and knowledge for church leadership to engage in strategies that lead to social change.

Lower extremity power and knee extensor rapid force development after knee injury, surgery, and rehabilitation

Typical rehabilitation strategies and performance tests after knee surgery are often based on peak lower extremity strength. However, people rarely generate maximal knee force in both daily and sports activities, which are characterized by brief periods of rapid muscle activation and relaxation. Thus, the ability to rapidly develop or modulate force may be more meaningful and more relevant to function. It is unclear how knee surgery influences the neuromuscular mechanisms controlling the ability to rapidly develop leg muscle force and produce power, or the functional relevance of these characterizations of muscle performance in relation to injury, surgery, and recovery. The primary purpose of this collection of studies was to assess rapid quadriceps muscle activation and lower extremity force production in people undergoing arthroscopic knee surgery for meniscal debridement and anterior cruciate ligament (ACL) reconstruction. People undergoing arthroscopic partial meniscectomy (APM) presented with significant deficits in knee extensor rate of torque development (RTD), leg press power, and rapid quadriceps muscle activation both prior to and in the initial month following surgery. Subjective knee function was significantly correlated with RTD variables but not with peak strength or quadriceps volume. Limitations in the ability to rapidly activate the involved quadriceps suggests that impaired centrally mediated neural function of the involved quadriceps may limit RTD and lower extremity power post-surgery. Next, the speed and intensity of quadriceps exercise performed in the early post-surgical period of patients post-APM and the relationships between training parameters, strength, quadriceps RTD, and subjective knee function were investigated. Subjects performed high intensity quadriceps contractions 2-3x/week in the first month following surgery. All subjects increased quadriceps strength, but people who trained with greater RTD following APM demonstrated greater improvements in RTD and had better patient-based outcomes scores than those who trained with a slower rate of torque rise. Finally, power and rate of force development (RFD) in people ≤ 1 year following ACL reconstruction were evaluated along with movement biomechanics, typical clinical measures of readiness to return to activity, and patient-based outcomes. Significant side-to-side asymmetries in quadriceps strength, RFD, leg press strength and power, and knee joint kinetics were noted. Deficits in voluntary quadriceps strength paralleled the deficits in early phase RFD, indicating that in this population RFD was limited by the intrinsic properties and force production capacity of the quadriceps, not the ability to rapidly activate the muscle. However, strong to very strong correlations were found between quadriceps RFD, movement biomechanics and subjective knee function, which were predominantly stronger than the correlations with peak quadriceps strength. Leg press strength, power, and acceleration were very strongly correlated with movement biomechanics and subjective knee function. In summary, this series of studies provides important insight into the neuromuscular mechanisms related to rapid lower extremity force development and muscle activation in the context of knee joint injury and recovery after arthroscopic knee surgery. Collectively, this work suggests that the inability to quickly develop or modulate quadriceps force may have significant functional consequences, and that rehabilitation efforts following arthroscopic knee surgery to incorporate both specific dosage of and earlier performance of rapid leg muscle contractions should be explored.

<p><strong>Syfte</strong>: Att belysa hur faktorer inom områdena person, miljö och sysselsättning kan påverka delaktigheten hos personer med stroke vid rehabilitering i hemmet. </p><p><strong>Metod: </strong>Genom en litteraturstudie har tio vetenskapliga artiklar av kvalitativa och kvantitativ art analyserats utifrån Internationell klassifikation av funktionstillstånd, funktionshinder och hälsa (ICF) och <em>Person Environment</em> <em>Occupation Model</em> (PEO).</p><p><strong>Resultat</strong>: Resultatet visade att vårdtagare som rehabiliterats i sin hemmiljö upplever mer delaktig i rehabiliteringen i jämförelse med vårdtagare som har genomgått sin rehabilitering på sjukhus. Betydelsefulla faktorer till den ökade delaktigheten är bland annat att vårdgivare får anpassa sig till vårdtagaren, som därmed får ett ökat medbestämmande. Vårdtagare upplever en trygghet när rehabiliteringen äger rum i hemmet, eftersom det är där de har sina gamla aktivitetsmönster. Vårdgivare får även större möjligheter att identifiera vilka aktiviteter som tidigare varit meningsfulla för vårdtagaren. Minskad delaktighet kopplas i flera studier till informationsbrist hos vårdtagarna angående deras nya livssituation och rehabiliteringsinsatser. The Canadian Occupational Performance Measure (COPM) är en klientcentrerad modell som visat stor användbarhet i att få vårdtagare delaktiga i rehabiliteringsprocessen, och då inte minst när det gäller information till vårdtagaren. Litteraturen visade även att det finns brister i samhällsresurser för att personer som drabbats av stroke ska uppleva delaktighet.                                                                         </p><p><strong>Slutsats: </strong>För att kunna göra en person delaktig vid rehabilitering i hemmet krävs det att åtgärderna utformas och genomförs i samråd med vårdtagaren. Vårdtagaren ska också få individuellt anpassad information om rehabiliteringsinsatser och förvärvat hälsotillstånd. Ett sätt att främja för vårdtagares optimala delaktighet är att ta hjälp av bedömningsinstrumentet COPM i rehabiliteringsprocessen. Ett bra stöd från samhället är en förutsättning för att bidra till delaktighet hos personer som drabbats av stroke.</p>
