Conhecimentos e opiniões de usuários sobre a importância do exame e das amostras de escarro no diagnóstico da tuberculose pulmonar / Knowledge and opinions of patients regarding the usefulness of bacteriological method and samples sputum in the diagnosis of Pulmonary Tuberculosis

A tuberculose continua sendo um sério problema de saúde pública no Brasil. O coeficiente de mortalidade vem tendo aumento significativo, apesar de todo o avanço tecnológico no campo dos recursos diagnósticos e de tratamentos disponíveis. Esta pesquisa teve como objetivo identificar conhecimentos e opiniões de usuários num serviço de saúde do Município de São Paulo Brasil, sobre a importância do exame das amostras de escarro no diagnóstico da tuberculose pulmonar pelo método bacteriológico. Setenta e oito (78) usuários foram entrevistados, durante o período de junho e julho de 1998. Os resultados mostram uma população com baixas condições sócio-econômicas. Todos os entrevistados haviam sido submetidos à baciloscopia de escarro e radiografia de tórax, e (46,2 por cento ) confiavam mais no RX do que na baciloscopia de escarro. Embora 93,6 por cento alegassem terem sido orientados quanto à colheita de escarro, constatou-se que 33,3 por cento deles, não sabiam colher o exame e 15,4 por cento apresentavam dúvidas. Quando indagados sobre a importância da colheita de mais de uma amostra, 52,1 por cento não souberam responder. Quanto à importância atribuída ao exame de escarro, 10,4 por cento citaram que o exame serve para avaliar o tratamento e, 16,6 por cento que o mesmo descobre se a doença é ativa; embora, uma grande maioria (72,9 por cento ) mencionassem que fizeram o exame porque o médico solicitou. As dificuldades quanto ao entendimento das orientações recebidas apresentam-se relacionadas às condições sócio-econômicas precárias da população estudada, ao modo de trabalho da equipe de saúde, aliadas à atitude passiva dos usuários diante da assistência prestada, ao forte estigma quanto ao ato de escarrar e a crença de que o exame radiológico é o melhor método diagnóstico. A somatória destes fatos promove falhas na qualidade da assistência prestada e na efetivação das ações do Programa de Controle da Tuberculose, o que traz, consequentemente, uma perda da qualidade dos serviços e o agravo do problema da tuberculose em nosso meio. / Tuberculosis continues to be considered a serious public health problem in Brazil. Despite the technological advancements and in addition to diagnosis resources and available treatments, mortality rates have been steadily increasing. The present investigation was intended to study either the knowledge and opinions of patients attending a health service located in the Municipality of São Paulo, Brazil, regarding the usefulness of the bacteriological method and samples sputum used to diagnose pulmonary tuberculosis using a bacteriological method. Seventy-eight (78) patients were interviewed from June to July, 1998. Results showed that the studied population presents a low social and economical level. All the interviewed individuals had been submitted to sputum smears tests and thorax radiographies, while 46,2 per cent of the patients reported a higher confidence level in the radiographic screening that in the sputum smear test. Although 93,6 per cent of the patients claimed to have been oriented on sputum collection, it was evidence that 33,3 per cent of them failed to know the way to collect the material and 15,4 per cent were doubtful in relation to the procedure. When inquired on the importance of collecting more than one sample, 52,1 per cent of the patients failed to know the answer. Regarding the importance attributed to the sputum smear test, 10,4 per cent reported that the test was useful in evaluating the treatment, and 16,6 per cent replied that the test was able to find out whether the disease was active. Most patients (72,9 per cent ) reported having carried out the test advised by medical request. The difficulties regarding the received orientations show to be related to the studied populations precarious socioeconomic conditions, health team working mode, patients passive attitude concerning the help provided, the strong stigma related to the expectoration act, and the belief that the radiographic examination is the best diagnostic method. The totality of findings show deficiencies in the quality of the provided assistance and in the accomplishment of the Tuberculosis Control Program, since they lead to a worsening quality of the service and aggravation of the tuberculosis program within our milieu.

Study of Improved Sampling and Preparation of East-Swedish Glacial Clay

The aim of this study is to determine if the mini block sampling technique couldoffer a complement to the Swedish standard StII piston sampler for achieving higherquality samples. The principle is that having a greater diameter the sample is meticulouslytrimmed to the usual geometry, so that the periphery of the sample is not disturbedby the sampling procedure and the same testing equipment can be employed.Hence, the test would provide higher and more accurate values of deformation andshear strength properties.The testing campaign executed for this study includes routine analysis with fallcone tests, CRS oedometer tests, DSS tests, CAUC triaxial tests and sedimentationtests. All of them have been performed on East-Sweden varved clay from Barkarby,Stockholm. Afterwards, the results and the sampling procedure were analysed toevaluate the performance of the mini block sampler.The mini block sampler provided more pronounced preconsolidation pressureson CRS tests as well as higher values of the quality index Mi=ML. DSS tests presenthigher values of undrained shear strength but no difference in shear strain at failure.In addition, the values of shear modulus show stiffer behaviour for block samplesas a prove of better clay structure preservation. Regarding triaxial results, theundrained shear strength values are higher for block samples with higher stresspeaks and lower strain to failure in the stress-strain graphs. Finally, several qualityindex were analysed obtaining better results for block sampling in most of them.It can be concluded that mini block sampler generally presents better qualitysamples than piston sampler for East-Swedish clay. Additionally, it is highlyfavourable for commercial projects due to each block provides up to 8 standard 50mm samples

META-ANALYSIS OF GENE EXPRESSION STUDIES

Combining effect sizes from individual studies using random-effects models are commonly applied in high-dimensional gene expression data. However, unknown study heterogeneity can arise from inconsistency of sample qualities and experimental conditions. High heterogeneity of effect sizes can reduce statistical power of the models. We proposed two new methods for random effects estimation and measurements for model variation and strength of the study heterogeneity. We then developed a statistical technique to test for significance of random effects and identify heterogeneous genes. We also proposed another meta-analytic approach that incorporates informative weights in the random effects meta-analysis models. We compared the proposed methods with the standard and existing meta-analytic techniques in the classical and Bayesian frameworks. We demonstrate our results through a series of simulations and application in gene expression neurodegenerative diseases.

[pt] Este trabalho teve como objetivo avaliar a qualidade de amostras intactas da Argila do Sarapuí II extraídas com amostradores tubulares de pistão estacionário Geonor/NGI 76 mm. Este amostrador possui comprimento útil de 800 mm e diâmetro interno de 76,2 mm sem folga interna, dispondo de um pistão estacionário com ponta cônica, não sendo necessária a execução de pré-furo para a sua cravação. Duas amostras foram extraídas entre as profundidades de 3,50 e 4,30 m, sendo que um dos tubos de amostragem teve a sua superfície interna revestida com um polímero de baixo coeficiente de atrito, com o intuito de reduzir a tensão cisalhante entre a amostra e o amostrador durante a manobra de cravação do tubo no solo e, assim, avaliar a influência dessa redução na qualidade das amostras. As qualidades das amostras foram avaliadas com base nos resultados de ensaios de adensamento edométrico de carga incremental realizados em corpos de prova moldados com diâmetros de cerca de 50 mm e de 70 mm e altura de 20 mm, o que possibilitou também a avaliação da influência do diâmetro do corpo de prova na qualidade. Todos os corpos de prova foram classificados como de qualidade excelente a muito boa segundo o critério de Lunne et al. (1997), não tendo sido observada influência do diâmetro do corpo de prova nos resultados. Da mesma forma, a aplicação do polímero na superfície interna do tubo de amostragem parece não ter exercido influência nas qualidades dos corpos de prova. / [en] The aim of this work is to evaluate the quality of intact Sarapuí II clay samples taken with stationary piston tube samplers Geonor/NGI 76 mm. This sampler has a net tube length of 800 mm and an inner diameter of 76,2 mm with no inside clearance. It has a cone ended stationary piston, and pre-drilled boreholes are not required when driving the sampler. Two samples were taken from a depth l of 3.50 m to 4.30 m. One of the sampler tubes was coated on the inside with a low friction polymer in order to try to reduce the shearing stress between the sample and the sampler tube when pushing the tube during sampling, as to evaluate the influence of such reduction on the quality of the samples. The sample s quality was evaluated based on the results of incremental-loading one-dimensional consolidation tests carried out on specimens trimmed with diameters of about 50 mm and 70 mm, and a height of 20 mm. This also allowed the evaluation of the influence of the specimen s diameter on quality. All specimens were classified as excellent to very good according to Lunne et al s (1997) criterion. Moreover, no influence of the specimen diameter on the results was observed. Furthermore, the use of the polymer on the inner surface of the sampler tube seems to have had no influence on the quality of the specimens.
