On Weak Limits and Unimodular Measures

In this thesis, the main objects of study are probability measures on the isomorphism classes of countable, connected rooted graphs. An important class of such measures is formed by unimodular measures, which satisfy a certain equation, sometimes referred to as the intrinsic mass transport principle. The so-called law of a finite graph is an example of a unimodular measure. We say that a measure is sustained by a countable graph if the set of rooted connected components of the graph has full measure. We demonstrate several new results involving sustained unimodular measures, and provide thorough arguments for known ones. In particular, we give a criterion for unimodularity on connected graphs, deduce that connected graphs sustain at most one unimodular measure, and prove that unimodular measures sustained by disconnected graphs are convex combinations. Furthermore, we discuss weak limits of laws of finite graphs, and construct counterexamples to seemingly reasonable conjectures.

Development of a career meta-competency model for sustained employability

The purpose of this study was to construct a career meta-competency model for sustained employability in the contemporary world of work. This study explored a convenience sample (N = 304) of early career employees’ personality preferences (measured by the Myers-Briggs Type Indicator), self-esteem (measured by the Culture-free Self-esteem Inventories for Adults) and emotional intelligence (measured by the Assessing Emotions Scale), as a composite set of their personality attributes, in relation to their employability attributes (measured by the Employability Attributes Scale). The participants comprised 81% blacks and 64% females employed in the business management field in managerial/supervisory (53%) and staff (28%) level positions. Their ages ranged between 25 and 40 years (early adulthood and establishment phase) (79%). The correlations, canonical correlations and multiple regression results indicated that the participants’ personality attributes were significantly and positively related to their employability attributes. Structural Equation Modelling indicated a moderate fit between the theoretically hypothesised career meta-competency model and the empirically tested structural model. The results indicated job level as a significant moderator of the relationship between the participants’ personality and employability attributes. Middle management level was associated with an inverse relationship between the personality and employability attributes. Staff and middle managers did not significantly differ regarding their mean scores on these variables. On a theoretical level, the study deepened understanding of the cognitive, affective, conative and interpersonal behavioural dimensions of the hypothesised career meta-competency model. On an empirical level, the study produced an empirically tested career meta-competency model in terms of the various behavioural dimensions. On a practical level, career v counselling and development interventions for guiding employees’ sustained employability in terms of the career meta-competency behavioural dimensions were recommended. / Industrial & Organisational Psychology / D. High-amylose carboxymethyl starch matrices for oral sustained drug-release : in vitro and in vivo evaluation

Les amidons non modifiées et modifiés représentent un groupe d’excipients biodégradables et abondants particulièrement intéressant. Ils ont été largement utilisés en tant qu’excipients à des fins diverses dans des formulations de comprimés, tels que liants et/ou agents de délitement. Le carboxyméthylamidon sodique à haute teneur en amylose atomisé (SD HASCA) a été récemment proposé comme un excipient hydrophile à libération prolongée innovant dans les formes posologiques orales solides. Le carboxyméthylamidon sodique à haute teneur en amylose amorphe (HASCA) a d'abord été produit par l'éthérification de l'amidon de maïs à haute teneur en amylose avec le chloroacétate. HASCA a été par la suite séché par atomisation pour obtenir le SD HASCA. Ce nouvel excipient a montré des propriétés présentant certains avantages dans la production de formes galéniques à libération prolongée. Les comprimés matriciels produits à partir de SD HASCA sont peu coûteux, simples à formuler et faciles à produire par compression directe. Le principal objectif de cette recherche était de poursuivre le développement et l'optimisation des comprimés matriciels utilisant SD HASCA comme excipient pour des formulations orales à libération prolongée. A cet effet, des tests de dissolution simulant les conditions physiologiques du tractus gastro-intestinal les plus pertinentes, en tenant compte de la nature du polymère à l’étude, ont été utilisés pour évaluer les caractéristiques à libération prolongée et démontrer la performance des formulations SD HASCA. Une étude clinique exploratoire a également été réalisée pour évaluer les propriétés de libération prolongée de cette nouvelle forme galénique dans le tractus gastro-intestinal. Le premier article présenté dans cette thèse a évalué les propriétés de libération prolongée et l'intégrité physique de formulations contenant un mélange comprimé de principe actif, de chlorure de sodium et de SD HASCA, dans des milieux de dissolution biologiquement pertinentes. L'influence de différentes valeurs de pH acide et de temps de séjour dans le milieu acide a été étudiée. Le profil de libération prolongée du principe actif à partir d'une formulation de SD HASCA optimisée n'a pas été significativement affecté ni par la valeur de pH acide ni par le temps de séjour dans le milieu acide. Ces résultats suggèrent une influence limitée de la variabilité intra et interindividuelle du pH gastrique sur la cinétique de libération à partir de matrices de SD HASCA. De plus, la formulation optimisée a gardé son intégrité pendant toute la durée des tests de dissolution. L’étude in vivo exploratoire a démontré une absorption prolongée du principe actif après administration orale des comprimés matriciels de SD HASCA et a montré que les comprimés ne se sont pas désintégrés en passant par l'estomac et qu’ils ont résisté à l’hydrolyse par les α-amylases dans l'intestin. Le deuxième article présente le développement de comprimés SD HASCA pour une administration orale une fois par jour et deux fois par jour contenant du chlorhydrate de tramadol (100 mg et 200 mg). Ces formulations à libération prolongée ont présenté des valeurs de dureté élevées sans nécessiter l'ajout de liants, ce qui facilite la production et la manipulation des comprimés au niveau industriel. La force de compression appliquée pour produire les comprimés n'a pas d'incidence significative sur les profils de libération du principe actif. Le temps de libération totale à partir de comprimés SD HASCA a augmenté de manière significative avec le poids du comprimé et peut, de ce fait, être utilisé pour moduler le temps de libération à partir de ces formulations. Lorsque les comprimés ont été exposés à un gradient de pH et à un milieu à 40% d'éthanol, un gel très rigide s’est formé progressivement sur leur surface amenant à la libération prolongée du principe actif. Ces propriétés ont indiqué que SD HASCA est un excipient robuste pour la production de formes galéniques orales à libération prolongée, pouvant réduire la probabilité d’une libération massive de principe actif et, en conséquence, des effets secondaires, même dans le cas de co-administration avec une forte dose d'alcool. Le troisième article a étudié l'effet de α-amylase sur la libération de principe actif à partir de comprimés SD HASCA contenant de l’acétaminophène et du chlorhydrate de tramadol qui ont été développés dans les premières étapes de cette recherche (Acetaminophen SR et Tramadol SR). La modélisation mathématique a montré qu'une augmentation de la concentration d’α-amylase a entraîné une augmentation de l'érosion de polymère par rapport à la diffusion de principe actif comme étant le principal mécanisme contrôlant la libération de principe actif, pour les deux formulations et les deux temps de résidence en milieu acide. Cependant, même si le mécanisme de libération peut être affecté, des concentrations d’α-amylase allant de 0 UI/L à 20000 UI/L n'ont pas eu d'incidence significative sur les profils de libération prolongée à partir de comprimés SD HASCA, indépendamment de la durée de séjour en milieu acide, le principe actif utilisé, la teneur en polymère et la différente composition de chaque formulation. Le travail présenté dans cette thèse démontre clairement l'utilité de SD HASCA en tant qu'un excipient à libération prolongée efficace. / Unmodified and modified starches represent a particularly interesting group of biodegradable and abundant excipients. They have been widely used as excipients for various purposes in tablet formulations, such as binders and/or disintegrants. Spray-dried high-amylose sodium carboxymethyl starch (SD HASCA) was recently proposed as an innovating hydrophilic excipient for sustained-release (SR) in solid oral dosage forms. Amorphous high-amylose sodium carboxymethyl starch (HASCA) was first produced by the etherification of high-amylose corn starch with chloroacetate. HASCA was then spray dried to obtain SD HASCA. This new excipient has shown advantageous and effective properties in the production of SR delivery systems. SR matrix tablets prepared from SD HASCA are inexpensive, simple to formulate and easy to produce by direct compression. The main objective of the present research was to continue the development and optimization of matrix tablets using SD HASCA as the retarding excipient in view of their ultimate application as sustained drug-release delivery systems for oral administration. For this purpose, dissolution tests simulating some of the most relevant physiological conditions of the gastrointestinal tract, taking into account the nature of the polymer under investigation, were employed to evaluate the drug-release characteristics and demonstrate the performance of SD HASCA SR formulations. An exploratory clinical study was also carried out to evaluate the SR properties of this new drug delivery system in the gastrointestinal tract. The first article presented in this thesis evaluated the drug-release characteristics and the physical integrity of formulations containing a compressed blend of drug, sodium chloride and SD HASCA in biorelevant media. The influence of different acidic pH values and residence times was investigated. The SR profile from an optimized SD HASCA formulation was not significantly affected by both the acidic pH value and the residence time in the acidic medium. These results suggest a limited influence of intra- and inter-subject variability of gastric pH on the release kinetics from SD HASCA matrices. In addition, the optimized formulation maintained its integrity throughout the duration of the dissolution tests. The exploratory in vivo study demonstrated extended drug absorption after oral administration of SD HASCA matrix tablets and that the matrix tablets did not disintegrate while passing through the stomach and resisted hydrolysis by α-amylase in the intestine. The second article reports the development of once-daily and twice-daily SD HASCA tablets containing tramadol hydrochloride (100 mg and 200 mg). These SR formulations presented high crushing strengths without requiring the addition of binders, which facilitates tablet processing and handling. The compression force (CF) applied to produce the tablets did not significantly affect the drug-release profiles. The total release time from SD HASCA tablets increased significantly in function of the tablet weight and can be used to modulate the total release time from theses formulations. When exposed to a pH gradient and to a 40% ethanol medium, a very rigid gel formed progressively on the surface of the tablets providing controlled drug-release properties. These properties indicated that SD HASCA is a robust excipient for oral, sustained drug-release, likely to minimize the possibility of dose dumping and consequent adverse effects, even when co-administered with high doses of alcohol. The third article investigated the effect of α-amylase on drug-release from previously developed SD HASCA tablets containing acetaminophen and tramadol hydrochloride (Acetaminophen SR and Tramadol SR). Mathematical modeling showed that an increase in α-amylase concentration resulted in an increase of polymer erosion over drug diffusion as the main mechanism controlling drug-release, for both formulations and both residence times in acidic medium. However, even if the mechanism of release was affected, α-amylase concentrations ranging from 0 IU/L to 20000 IU/L did not significantly affect the drug-release profiles from SD HASCA SR tablets, regardless of the residence time in acidic medium, the drug used, the polymer content and the different composition of each formulation. The work presented in this thesis clearly demonstrates the value of SD HASCA as an efficient SR excipient.

Accurate Methodology for Monitoring Biomembrane Events

Sidorov ASSOCIATE PROFESSOR, DEPARTMENT OF CHEMISTRY This study describes the synthesis and characterization of a new receptor (cyclen 1) capable of strong selective binding of pyrene-based anionic dyes under near-physiological conditions. This receptor comprises four naphthylthiourea groups tethered to a cyclen core via an ester linkage. The most important finding was the ability of cyclen 1 to bind efficiently to a pH-sensitive pyranine dye, a dye that is commonly used in various biomembrane assays. The high affinity of cyclen 1 to pyranine, its impermeability to the lipid bilayer membrane, fast kinetics of binding, and ability to quench pyranine’s fluorescence were used as a basis for a new membrane leakage assay. This membrane leakage assay is fully compatible with the commonly applied pH-stat transport assay, and therefore it allows for differentiation of ion transport and nonselective leakage mechanisms within a single set of experiments. In the second part of this study a new methodology for the detection of lipid flip was developed. This methodology relies on the quenching of the fluorescence of a newly synthesized cascade-blue-labeled lipid through complex formation with cyclen 1. This receptor-dye complexation also has high affinity for binding at micromolar concentrations and can be reversed by either competitive displacement of the lipid probe or by enzymatic degradation of the receptor leading to the label release and fluorescence dequenching. This new methodology is suitable for the study of lipid flip in both model spherical bilayer membranes and in-vitro experiments, and is less invasive to the model and cell membranes than the commonly utilized 7-nitro-1,2,3-benzoxadiazol-4-yl (NBD)-dithionite methodology. Lastly, new pH-sensitive lipids were synthesized and utilized in the formulation of liposomes suitable for controlled drug release. These liposomes contain various amounts of internal NaCl and undergo internal acidification upon the exogenous addition of an HCl co-transporter in a physiologically relevant NaCl solution. Therefore, acidification ultimately leads to the hydrolysis of the pH-sensitive lipids and subsequent contents release. These liposomes were found to be insensitive to physiological concentrations of human serum albumin and to be non-toxic to cells at concentrations exceeding pharmacological relevance. These results render this new drug release model potentially suitable for in vivo applications.

L’immunothérapie orale pour le traitement des allergies alimentaires multiples

La prévalence des allergies alimentaires IgE-médiées aurait triplé au cours de la dernière décennie avec des études Nord-Américaines atteignant les 8% chez les enfants. Quoiqu’il n’y ait à ce jour aucun traitement curatif pour les allergies alimentaires, l’immunothérapie oral (OIT) constitue une nouvelle approche expérimentale prometteuse. Cette dernière consiste en l’administration de doses progressive d’allergènes par voie orale sur une période prolongée dans le but d’instaurer un état de désensibilisation et possiblement une tolérance orale soutenue. Cette approche a été démontrée sécuritaire et permettrait la désensibilisation à haute dose de plus de 80% des participants allergiques aux arachides, lait ou œufs. Dans cette thèse, nous présentons 2 études de phase 1 portant sur des protocoles d’OIT, destinés à optimiser l’efficience du traitement chez les sujets avec allergies alimentaires multiples. Près de 30% des enfants avec allergie alimentaire sont allergiques à plus d’un aliment, une proportion qui augmente à 70% lorsqu’on considère les cas les plus sévères. Ces enfants sont à risque augmenté de réactions accidentelles et souffrent d’un impact plus grand sur leur qualité de vie. Dans la première étude, en créant un mélange individualisé avec un ratio stochiométrique 1:1 entre les protéines des aliments allergiques de l’enfant, nous démontrons qu’il est possible de désensibiliser jusqu’à 5 aliments simultanément avec un profil d’innocuité similaire à une monothérapie. Dans la seconde étude, nous utilisons un traitement à l’omalizumab, un anticorps monoclonal anti-IgE, pour permettre une désensibilisation orale multi-allergénique fortement accélérée. Lorsque comparé à l’approche sans omalizumab, ce protocole s’associe à une nette diminution du temps requis pour atteindre les doses d’entretien, passant d’une médiane de 21 à 4 mois, sans affecter le profil d’innocuité. Alors que ces études fournissent des approches cliniques raisonnables pour désensibiliser la population multi-allergique, plusieurs questions persistent, notamment en ce qui a trait à l’induction de tolérance permanente. Une barrière majeure à cet égard réside dans notre piètre compréhension des mécanismes sous-jacents à l’immunothérapie. Prenant avantage d’échantillons cliniques bien caractérisés provenant des essais cliniques ci-haut mentionnés, nous utilisons les nouvelles technologies de séquençage TCR pour suivre la distribution clonale des lymphocytes T spécifiques aux arachides durant une immunothérapie orale. Nous démontrons que l’OIT s’associe à des changements significatifs dans les fréquences des clones spécifiques, suggérant un processus d’épuisement clonal et de remplacement. Nous démontrons par ailleurs que le test de prolifération lymphocytaire, traditionnellement utilisé pour évaluer la réponse cellulaire allergique, est dominé par une distribution polyclonale hautement non-spécifique. Cette observation a des implications majeures considérant que la plupart de la littérature actuelle sur la réponse T se base sur cette technique. En somme, cette thèse jette les bases pour des programmes de recherche translationnelle pour optimiser et personnaliser les protocoles cliniques actuels et développer de nouvelles avenues d’investigation et de traitement pour améliorer la prise en charge des sujets avec allergies alimentaires. / The prevalence of IgE-mediated food allergies has tripled over the last decade with prospective studies indicating that up to 8% of children may be affected in North America. There is currently no cure for food allergy but oral immunotherapy (OIT) is an experimental approach to treat food allergies. It consists in the progressive administration from minute to large amounts of the allergenic food by the mouth over a prolonged period of time to induce a state of desensitization and possibly sustained tolerance. This approach has been shown to be safe and to allow desensitization to high doses in over 80% of participants allergic to peanuts, milk or egg. In this thesis, we present two phase 1 trials on OIT protocols designed to efficiently treat multiple foods allergies. About 30% of children with food allergy are allergic to more than one food. This proportion increases to 70% when considering the most severe cases. Children with multiple food allergies are at higher risk of accidental reactions and suffer from greater impact on quality of life than those with single food allergies. By creating a customized treatment mix with a 1:1 stoichiometric ratio for the child’s relevant food proteins, we were first able to safely desensitize up to 5 foods simultaneously with a safety profile similar to single allergen therapy and a minimal increase in time to maintenance. Then, taking advantage of recent evidence showing that omalizumab, an anti-IgE receptor monoclonal antibody, can significantly raise reaction thresholds in food allergic subjects, we used short courses of omalizumab to allow very rapid oral desensitization to various foods in a second phase 1 study. When compared to “standard” multi-OIT, the omalizumab-enabled rush protocol resulted in a decreased time to maintenance from a median of 21 to 4 months. While these studies provide reasonable clinical approaches to this population, many questions remain, especially with regards to long term tolerance. A major limit to our progress in improving these protocols stems from our lack of understanding of the underlying immune mechanisms of oral immunotherapy. Taking advantage of well phenotyped samples from the afore-mentioned trials, we used next-generation high-throughput TCR sequencing to follow clonal distribution of peanut specific T cells during oral immunotherapy. We found that OIT is associated with significant changes in food-specific clonal frequencies, suggesting clonal exhaustion and replacement as an underlying mechanism of OIT. In addition, we show that the proliferation assay which is traditionally used to assess the cellular response is dominated by a highly non-specific polyclonal distribution. This observation has important implications considering most of the current literature on T cell response to immunotherapy is based on this assay. This highlights the need for the development of new tools to assess the cellular allergic response. Overall this thesis lays the ground for further comprehensive translational research programs on the treatment of food allergy.

An evaluation of Training for Trainers (T4T) as an aid for developing sustained church planting movements (CPMs)

This paper attempts to evaluate Training for Trainers (T4T) as an aid for developing healthy and sustained church planting movements (CPMs). The thesis is that Training for Trainers (T4T) can enable and sustain (by the Spirit’s power) healthy church planting movements because a discipleship process is built into the methodology that develops believers in their personal and communal growth and equips them to repeat the process with other individuals they reach. The very format of the T4T process provides a context for developing disciples inwardly and training disciples to minister outwardly. The T4T process continues over the course of months and years to systematically move believers through the essential stages of sustained church planting movements: Bridges in conversations with the lost from non-spiritual topics to the gospel in order to find those God is preparing (knowing whom to talk to and how to start) • Reproducible evangelism methods that are effective in the local context and can be learned by any new believer • Reproducible discipleship that addresses both short-term and long-term spiritual growth in a manner appropriate to the local worldview and able to be passed on by a new believer • Reproducible church models appropriate to the local context and able to be led and passed on by new believers • Leadership development and multiplication patterns that develop leaders rapidly in the context of ministry and enable the number of leaders to keep pace with the number of new churches. T4T moves each new generation of disciples (trainers) and churches through this process because it casts vision for and gives loving accountability for disciples to truly become trainers of others. It does this primarily through a three-thirds training process. T4T is training for trainers who will train trainers who will train trainers. T4T attempts to initiate movements of God in which at least four generations of new disciples and churches emerge. Work, play and ride the storms : an ethnography of sustained innovation

Cette thèse est composée de trois essais empiriques issus d’une enquête ethnographique conduite dans une entreprise innovante en Inde, et étudie les processus permettant l'innovation durable. Cette recherche aborde le rôle du jeu dans les processus de travail innovant, ainsi que le rôle de l'identité organisationnelle comme une réponse de l'organisation aux menaces extérieures. Cette thèse montre comment le jeu se déroule dans les organisations, sa nature et son rôle dans les processus de travail créatifs. Les résultats indiquent que le jeu a plusieurs effets sur les tâches et les relations qui affectent les processus collectifs de travail créatif qui soutiennent l'innovation durable. Je montre aussi les transitions entre le travail intense et jeu intense et expose un modèle incluant des conditions initiales, des mécanismes et des signaux qui facilitent les transitions. Ce faisant, cette thèse construit les bases d'une théorie du travail et du jeu. Cette recherche décrit un nouveau genre de jeu dans les organisations, différent de ceux observés et étudiés précédemment. Elle contribue aux théories du jeu et au travail créatif dans l'organisation. Prenant en compte le fait que l'innovation durable dépend aussi de la capacité de l'organisation à faire face aux menaces extérieures, cette recherche montre également comment l'identité organisationnelle joue un rôle crucial dans l'élaboration des méthodes de travail pour faire face aux menaces extérieures. Ainsi, cette étude contribue à la littérature sur la formation de l'identité organisationnelle, et son rôle dans les pratiques organisationnelles, la survie et l'innovation durable / This dissertation consists of three empirical essays based on ethnographic fieldwork conducted in an innovation-based firm in India, and investigates the processes enabling sustained innovation. More specifically, it addresses the role of play in innovative work and its processes, as well as the role of organizational identity in organization’s response to external threats. I show how playfulness unfolds in work organizations, its nature, and its role in the creative work processes. Results indicate that play has several task and relationship related effects on group creative work processes on which sustained innovation rests. It also show how people transition between intense work and intense play – and explicate a model of initial conditions, mechanisms and cues for such transitions. By doing so, this study starts to lay the grounds for a theory of work and play, and provides an answer to how innovative work gets accomplished amidst playfulness in organizations. This study describes a new kind of play in work organizations, different to those observed and investigated in previous organizational studies and contributes to theories of play and creative work in organization. Mindful of the fact that sustaining innovation also depends on organization’s ability to cope with external threats, this study also shows how organizational identity play a crucial role in shaping its work practices for responding to external threats, and how threat can even lead to the formation of an organization’s initial identity. Thus, this study also contributes to the literature on organizational identity, and its role in organizational practices, survival, growth and sustained innovation

O objetivo do presente trabalho foi desenvolver e avaliar péletes para compor sistema multiparticulado contendo ácido ascórbico, bem como validar metodologia analítica por cromatografia líquida de alta eficiência (CLAE) para aplicação em ensaio de dissolução destas formas farmacêuticas. A técnica de extrusão-esferonização foi utilizada por ser de fácil aplicação a nível laboratorial e industrial obtendo-se péletes matriciais, compostos de Methocel® K4M, Methocel® K100M e Eudragit® L 100, e péletes de liberação convencionais, para revestimento em leito fluidizado com Kollicoat® SR 30 D como agente regulador da liberação do fármaco. O sistema multiparticulado foi preparado na forma de comprimido pela compressão dos péletes com menor nível de revestimento, avaliando-se o nível de força aplicada e a presença da Microcel® 101 como protetor da camada de revestimento. Os péletes obtidos foram caracterizados quanto à esfericidade por análise imagem, friabilidade, dureza e perfil de dissolução comparativo com apresentações comerciais. Os péletes matriciais foram obtidos com grau de dificuldade variável em função do polímero utilizado, porém apresentaram esfericidade e resistência mecânica adequadas. O perfil de dissolução destas formulações demonstrou que não houve controle na liberação do fármaco, mesmo naquelas onde havia maior concentração de polímero. Os péletes revestidos com três níveis de polímero (5,07; 8,26 e 10,35% em relação à massa do pélete) apresentaram boas características granulométricas e o perfil de dissolução daqueles revestidos com 5,07% de polímero demonstrou semelhança com o da apresentação comercial. O perfil de dissolução comparativo entre os péletes isolados e os comprimidos obtidos sem a presença de excipiente mostrou que ocorre dano na camada de revestimento nos maiores níveis de força de compressão aplicada. Para os comprimidos obtidos a partir da mistura de péletes e Microcel® 101 este fato não foi observado, indicando que a presença do excipiente é fundamental para a manutenção da integridade da camada de revestimento além de promover a desintegração parcial dos comprimidos. / The present work aimed to develop and evaluate pellets to compound a multiparticulate system containing ascorbic acid, as well as, validate analytic methodology through high performance liquid chromatography and evaluate the essay of dissolution from these dosage forms. The extrusion-spheronization technique was used for its laboratorial and industrial application to obtain matrix pellets compounds of Methocel® K4M, Methocel® K100M, Eudragit® L 100, and conventional release pellets, for fluid bed coating with Kollicoat® SR 30 O as a regular occurrence release agent for the drug. The multiple system was prepared in tablets through the compression of the pellets with a lower coating levei, evaluating its applied compression force and the presence of Microcel® 101 as a protector of the coating layer. The obtained pellets were characterized according to its roundness through image analysis, friability, hardness and dissolution profile compared with those marketed products. The matrix pellets were obtained on varied difficulty degree due to the polymer utilized, even through they presented adequate spheroid and mechanical properties. The dissolution profile of these formulations didn\'t show any control in the release of the drug even in those, which had a high concentration of the polymer. The pellets coated with three polymers leveis (5,07%, 8,26% and 10,35% related to the pellets mass) presented good sphericity, and the dissolution profile of those pellets coated with 5,07% polymer demonstrated similarity with those ones marketed products. The comparative dissolution profile among isolated pellets and the tablets obtained without excipients presence, showed that damage right occur on the higher compression force levei is applied. This fact is not observed on the tablets obtained from the pellets blend and Microcel®101, which indicates that the presence of the excipients is fundamental for the maintenance and integrity of the coating layer, moreover, it causes the partial disintegration of the tablets.

Filmes finos do ácido poli 3-tiofeno acético / Thin films of poly 3-thiophene acetic acid

O ácido politiofeno acético (PTAA) é um derivado do politiofeno bastante versátil. Sua solubilidade em alguns solventes orgânicos e em soluções básicas aquosas lhe confere extensa processabilidade na forma de filmes finos, característica importante para dispositivos e sensores. Neste trabalho, investigou-se a formação de filmes de PTAA com as técnicas de automontagem e Langmuir-Blodgett (LB). Os filmes automontados foram preparados com dois policátions, hidrocloreto de poli-alilamina (PAH) e cloreto de poli-dialildimetilamônio (PDAC). O crescimento dos filmes depende do pH das soluções e do tipo de policátion, sugerindo dependência do mecanismo de crescimento com as interações específicas polímeropolímero. A conformação do PTAA em filme tem correlação com sua conformação em solução, apontando para um efeito de memória. Por outro lado, a energia de superfície destes filmes não sofre influência da arquitetura nem dessas diferenças conformacionais. Ou seja, embora o volume do filme possa ser distinto, as superfícies possuem propriedades semelhantes. A morfologia dos filmes foi caracterizada a partir de imagens de AFM utilizando conceitos de geometria fractal e estatística. A dimensão fractal dos filmes é semelhante, indicando o mesmo processo de crescimento dos filmes, independentemente das condições da deposição. Os filmes obtidos em pH ácido tinham tamanho de grão e comprimentos de correlação maiores, sugerindo a deposição de cadeias mais enoveladas. Foi possível fabricar filmes autossustentados sem degradação aparente do material a partir de filmes automontados de PAH/PTAA, entrecruzando termicamente os grupos ácido carboxílico e amina. Este é o primeiro relato de filmes deste tipo com derivado do politiofeno. Os filmes LB de PTAA foram obtidos sem adjuvantes, mas as condições de deposição precisam ser aprimoradas. Para explorar a elevada afinidade química entre compostos contendo enxofre e metais pesados, filmes foram utilizados para detecção espectroscópica e eletroquímica. Espectros de fotoluminescência e UV-Vis demonstraram que os metais interagem apenas com os estados excitados resultando na supressão da fluorescência; no entanto, sem especificidade e apenas para longos períodos de exposição. Espectros de FTIR mostraram a presença dos sais na matriz dos filmes. Por sua vez, voltametrias cíclicas permitiram detectar Pb+2 e Hg+2, mas a irreversibilidade dos processos eletroquímicos, causando alargamento dos picos de oxirredução, inviabiliza a detecção simultânea. / Polythiophene acetic acid (PTAA) is a versatile polythiophene derivative. Its solubility in some organic solvents and in basic aqueous solutions makes it attractive for processing thin films, an important feature for the fabrication of devices and sensors. In this thesis, we investigate the formation of PTAA films using the layer-by-layer (LbL) and the Langmuir- Blodgett (LB) techniques. The LbL films were prepared with poly(allylamine hydrochloride) (PAH) and poly(diallydimethylammonium chloride) (PDAC), with film growth depending on the pH of the solutions and type of polycation, thus indicating that the growth mechanism depends on polymer-polymer interactions. The conformation of the PTAA molecules in solid state was correlated with that in solution, in a kind of memory effect. The surface energy of the films was not affected by the film architecture or different conformations. The film morphology was characterized with AFM images using concepts of fractal geometry and statistics. The fractal dimension was similar for all films, and therefore the overall growth obeys the same process regardless of the deposition conditions. Nevertheless, films obtained at acidic pH exhibited larger grain size and correlation lengths than those produced at basic pHs, suggesting deposition of more coiled chains. It was also possible to fabricate selfsustained films without apparent PTAA degradation from the PAH/PTAA LbL films, upon thermal crosslinking of carboxylic acid and amine groups. This is the first report of such films with a polythiophene derivative. LB films of PTAA were obtained without co-spreading materials, but the deposition conditions need to be optimized. To explore the high chemical affinity between PTAA and compounds containing sulfur and heavy metals, some films were used for spectroscopic and electrochemical detection. The UV-Vis and photoluminescence spectra indicated that the metals affect only the excited states, leading to fluorescence quenching after long exposure times and without specificity for the metals. The FTIR spectra pointed to salts in the films. Pb+2 and Hg+2 ions could be detected using cyclic voltammetry, but their simultaneous detection was hampered by the irreversibility of the electrochemical processes which caused broadening of the oxi-reduction peaks.

Avaliação farmacocinética  e farmacodinânica de meropenem e vancomicina em pacientes submetidos à diálise estendida de baixa eficiência (SLED) / Pharmacokinetics and pharmacodynamics of vancomycin and meropenem in critically ill patients submitted to sustained low-efficiency dialysis

INTRODUÇÃO: A combinação de sepse e insuficiência renal com necessidade de diálise é bastante comum nas Unidades de Terapia Intensiva e esta situação tem elevada mortalidade. Um desafio neste cenário é prescrever a dose correta dos antimicrobianos para o tratamento destas infecções. Em pacientes críticos e hemodinamicamente instáveis que necessitam de terapia renal substitutiva, um dos métodos mais utilizados é a diálise contínua, entretanto, recentemente, tem-se utilizado a diálise de baixa eficiência - conhecida como SLED, da abreviação do inglês \"sustained low-efficiency dialysis\". Esta modalidade de terapia renal substitutiva combina características da hemodiálise contínua com a intermitente, utilizando o equipamento da diálise intermitente, com menores fluxos sanguíneos e de dialisato, e com vantagem de menor custo. Apesar do fluxo mais baixo, por ser utilizado tempo mais prolongado, a SLED frequentemente resulta em maior clearance e especula-se que a remoção dos fármacos seria maior. Há escassez de estudos que avaliaram a farmacocinética e farmacodinâmica de antimicrobianos em pacientes submetidos à SLED.OBJETIVOS: Avaliar adequação farmacodinâmica de meropenem e vancomicina em pacientes submetidos a diálise estendida de baixa eficiência. Avaliar a depuração paramêtros farmacocinéticos durante a sessão de SLED. MÉTODOS: Foi realizado estudo prospectivo descritivo observacional com coleta de material biológico julho de 2012 a julho de 2014 HC-FMUSP. Foram incluídos pacientes submetidos à SLED em uso de vancomicina e/ou meropenem. Foram coletadas amostras de sangue seriadas (tempos: imediatamente antes do início da sessão de diálise, 0,5h, 1h, 2h, 4h após o início do tratamento e ao final da sessão). A quantificação dos antimicrobianos foi realizada através dos métodos analíticos de quantificação em Cromatografia Líquida de Alta Eficiência (CLAE). Os parâmetros farmacocinéticos foram calculados apenas durante a sessão de diálise utilizando-se o software WinNonlin. A área sob a curva foi determinada para a vancomicina. Para o meropenem, calculou-se o tempo acima da MIC. Resultados: Foram incluídos 24 pacientes tratados com vancomicina e 21 com meropenem eforam obtidas 170 amostras de plasma. As concentrações médias de vancomicina sérica e meropenem: antes da sessão de SLED foram 24,5 e 28,0 ?g / ml, respectivamente; e após SLED 14 e 6 ?g / ml, respectivamente. A depuração média foi de 41% para a vancomicina e 78% para o meropenem. Para vancomicina, 22 (96%), 19 (83%) e 16 (70%) pacientes teriam atingido o alvo (AUC0-24 > 400) considerando-se MIC 0,5; <= 1mg/l e <= 2 mg/l respectivamente. Para meropenem, 19 (95%), 18 (90%) e 11 (55%) pacientes teriam atingido a meta (70% de tempo acima da CIM) se infectados com isolados com MIC <= 1, <= 4 e <= 8 mg/l, respectivamente. Conclusões: Em pacientes críticos, meropenem evancomicina foram removidas durante o SLED. Entretando, a maioria dos pacientes alcançaria alvo PK-PD, excepto para CIMs mais altas. Sugerimos doses de manutenção de 1g a cada 12 ou 8 horaspara meropenem. Para a vancomicina, deve-se utilizar abordagem mais individualizada com monitorização sérica, uma vez que ensaios comerciais são disponíveis / Background: Antibiotic dosing is a challenge in critically ill patients undergoing renal replacement therapy. Our aim was to evaluate pharmacokinetics and pharmacodynamics of meropenem and vancomycin in patients undergoing SLED.Methods: ICU patients undergoing SLED, receiving meropenem and/or vancomycin, were prospectively evaluated. Blood samples were collected at the start of SLED and 0.5; 1; 2; 4 and 6 hours later. Antimicrobial levels were determined by HPLC. Noncompartimental pharmacokinetic analysis was performed. Area under the curve was determined for vancomycin. For meropenem, time above MIC was calculated. Results: 24 patients receiving vancomycin and 21 receiving meropenem were included; 170 plasma samples were obtained. Median serum vancomycin and meropenem concentrations: before SLED were 24.5 and 28.0 ?g/ml, respectively; and after SLED 14 and 6 ?g/ml, respectively. Mean removal was 41% for vancomycin and 78% for meropenem. For vancomycin, 22 (96%), 19(83%) and 16(70%) patients would have achieved the target (AUC0-24>400) considering MIC 0.5; <= 1mg/l and <= 2 mg/l, respectively. We suggest maintenance doses of 1g tid or bid for meropenem. For vancomycin, more individualized approach using therapeutic drug monitoring should be used, as commercial assays are available
