Resumo: Alterações anatômicas e fisiológicas da gravidez predispõem gestantes a infecções do trato urinário (ITU). O não tratamento ou o tratamento inadequado dessas infecções pode levar a complicações obstétricas e neonatais, como amniorexe prematura, trabalho de parto e parto prematuros. Considerando a relevância das complicações, a possibilidade de ocorrência de bacteriúria assintomática e a demora para obtenção do resultado da urocultura, padrão-ouro para diagnóstico de ITU, muitos profissionais optam por iniciar o tratamento de ITU em gestantes, caso o exame de urina simples mostre-se alterado, mesmo na ausência de sinais clínicos inequívocos. Identificar a acurácia do exame de urina simples para diagnóstico de infecção do trato urinário em gestantes de baixo risco. Material e Método: Foi realizado estudo analítico e transversal no município de Botucatu/SP. A propedêutica de atendimento incluiu a realização do exame de urina simples e urocultura, os dados foram colhidos de outubro de 2006 a março de 2008 nos serviços de atenção básica. Resultados: Foram incluídas no estudo 230 gestantes, com mediana de idade de 25,2 anos (14 - 43), 79,2% casadas ou em união estável. A prevalência de ITU foi de 10%, sendo o microrganismo de maior freqüência a Escherichia coli (47,8%). A sensibilidade foi 95,6% e a especificidade 63,3% do exame de urina simples em relação ao diagnóstico de ITU. A acurácia foi de 66,5%. A análise dos valores preditivos positivo e negativo (VPP e VPN) mostrou que na vigência de exame de urina simples normal, a chance de haver ITU foi pequena (VPN 99,2%), frente ao resultado alterado deste exame, a probabilidade de haver ITU foi baixa (VPP 22,4%). Conclui-se que a acurácia do exame de urina simples como meio diagnóstico de infecção urinária foi baixa; alterações no exame de urina simples, mesmo na presença... Considering complications relevance and the possibility of asymptomatic bacteriuria occurrence and the delay from uroculture results, gold standard for UTI diagnosis, many professionals choose to begin UTI treatment in the pregnant woman, if simple urine examination is altered even in the absence of clear clinical signals. Objective: Identify simple urine examination accuracy to diagnose urinary tract infection in low risk pregnant women. Material and Methods: An analytical and transversal study was performed in Botucatu/SP. The service began with simple urine examination and uroculture, data collected from October 2006 to March 2008 in the basic health service. Results: 230 pregnant women were included in the study with an average age of 25,2 years (14 - 43), 79,2% married or under stable relationship. UTI prevalence was 10%, Escherichia coli as a more frequent organism (47, 8%). Sensitiveness was 95,6% and specificity 63,3% of simple urine examination related to UTI diagnosis. Accuracy was 66,5%. The analysis of predictive positive and negative values (PPV e NPV) showed that during a normal and simple urine examination UTI occurrence was lower (NPV 99,2%), against the altered result of this examination the likely of UTI occurrence was low (PPV 22,4%). Patrones de resistencia antimicrobiana de la familia enterobacteriaceae aisladas de infecciones del tracto urinario de una región alto-andina peruana / Antimicrobial patterns of resistance of the Enterobacteriaceae family isolated from urinary tract infections from a Peruvian high-Andean region

Introducción: La resistencia a antibióticos es considerada la próxima epidemia mundial. Las infecciones del tracto urinario (ITU) son la segunda causa más común de infecciones y la que presenta mayor frecuencia de resistencia. Sin embargo, poco se ha reportado en regiones altos-andinas. Objetivo: Evaluar los patrones de resistencia antimicrobiana de la familia enterobacteriaceae aisladas de infecciones del tracto urinario de pacientes de una región altoandina peruana y sus factores asociados. Métodos: Estudio transversal analítico retrospectivo, a partir de 1717 registros del Servicio de Microbiología de una institución de salud en la región Puno – Perú, entre los años 2014 al 2017. Se estudió la resistencia a antibióticos según uropatógeno en diferentes grupos etarios. Se empleó la prueba de Chi2 de Pearson y un modelo de regresión de Poisson para calcular la razón de prevalencias (RP). En todos los análisis se consideró un valor de p<0.05 como significativo y se estimaron intervalos de confianza al 95%. Resultados: Se presentó una amplia distribución de resistencia en todos los fármacos evaluados, siendo mayor en Escherichia coli y Proteus spp. El grupo etario, adulto mayor, presentó la mayor prevalencia de resistencia bacteriana. A medida que la edad aumentaba, la resistencia a todos los fármacos estudiados también aumentó (p <0.01). Así mismo, los adultos mayores presentaron mayor probabilidad de presentar resistencia bacteriana a penicilinas, cefalosporinas, quinolonas y otros antibióticos. Conclusión: El patrón de resistencia a los antibióticos utilizados en ITUs en la zona altoandina peruana incrementa con la edad. / Introduction : Antibiotic resistance is considered to be the next worldwide epidemic. Urinary tract infections (UTI) are the second most common cause of infection, which also has the highest resistance frequency. Nevertheless, in high Andean regions, little is known about the antibiotic resistance. Objective : Determine the antimicrobial resistance patterns of the enterobacteriaceae family isolated from urinary tract infections of a Peruvian Andean region. Methods: A retrospective cross-sectional review of 1717 records from the microbiology service of a private health institution from Puno - Peru, was done between the years 2014 and 2017. Antibiotic resistance by uropathogenes has been studied among different age groups. Statistical analysis included Chi2 test with a p<0.05 considered to be significant. A Poisson regression has been used to calculate the prevalence ratio (PR) with a 95% confidence interval. Results: There was a wide distribution of antibiotic resistance among all the antibiotics studied, mainly in Escherichia coli and Proteus spp. The elderly age group had the highest prevalence of antibiotic resistance. As age increased, resistance to all drugs studied also increased (p<0.01). Furthermore, the elderly age group had a risk probability of resistance of 1.22, 1.42, 1.20 and 1.32 to penicillins, cephalosporins, quinolones and other antibiotics respectively. Conclusion: The antimicrobial resistance patterns of the Peruvian Andean region were lower than national and international resistance patterns. / Trabajo de investigación

Asociación entre disfunción del suelo pélvico y la ocurrencia de la infección del tracto urinario entre mujeres embarazadas en Lima, Perú / Pelvic floor dysfunction and the occurrence of urinary tract infection among pregnant women in Lima, Peru

Introducción: Al tener una disfunción del suelo pélvico aumentan las probabilidades de desarrollar incontinencia urinaria o prolapso genital. Estas condiciones, a su vez, podrían generar una infección del tracto urinario (ITU). En la población de mujeres embarazadas, la disfunción del suelo pélvico ocurre principalmente por la carga de peso del vientre, el cual podría ser una causa de la ITU. Objetivo: Determinar la asociación entre la disfunción del suelo pélvico y la ocurrencia de la ITU en gestantes. Métodos: Estudio observacional de tipo transversal analítico. La población fueron gestantes entre 18 a 35 años de 5 centros de salud en Lima. La disfunción del suelo pélvico se evaluó con el cuestionario The Pelvic Floor Inventories Leiden (PeLFIs) dividiéndose en 3 componentes: sensación de flacidez, patrón de orina e incontinencia urinaria. La ITU se midió incluyendo una sección en el cuestionario abarcando 3 preguntas. Las variables edad, actividad física, ocupación y peso fueron incluidas en la sección de preguntas generales dentro del cuestionario. Resultados: En total fueron 146 gestantes encuestadas que cumplieron criterios de selección. La prevalencia de ITU durante el embarazo fue de 42.5%. Las gestantes con ITU durante el embarazo tienen un puntaje mayor en los componentes de sensación de flacidez (p=0.036), patrón de orina (p=0.001) e incontinencia urinaria (p=0.011). Conclusión: Luego de ajustar las variables confusoras se determinó que existe asociación entre los 3 componentes de disfunción del suelo pélvico y la ITU, ya que la mayoría de las gestantes tuvieron tratamientos contra la ITU para no perjudicar su embarazo. / Introduction: Upon occurrence of a pelvic floor dysfunction, the chances of developing urinary incontinence or vaginal prolapse increase. Such conditions, in turn, may lead to a urinary tract infection (UTI). In pregnant women, the pelvic floor dysfunction mainly occurs due to the weight of the womb, which could be a cause of the UTI. Objective: To determine the association between the pelvic floor dysfunction and the occurrence of UTI in pregnant women. Methods: Observational research of analytical cross-sectional type. The population included pregnant women between 18 to 35 years old from 5 health centers of Lima. The pelvic floor dysfunction was assessed through the questionnaire “The Pelvic Floor Inventories Leiden” (PelFIs), and it was divided into three components: vaginal swelling, urination pattern, and urinary incontinence. The UTI was measured based on three questions in a section of the questionnaire. Variables such as age, physical activity, occupation, and weight were included in the section of general questions within the questionnaire. Results: A total of 146 pregnant women surveyed met the selection criteria. The prevalence of UTI during pregnancy was 42.5 %. The pregnant women suffering from UTI during the pregnancy period show a higher score in the components of vaginal swelling (p=0.036), urination pattern (p=0.001), and urinary incontinence (p=0.011). Conclusion: After adjusting the confounding variables, it was determined that there is no association between the three components of the pelvic floor dysfunction and the UTI since most of the pregnant women followed treatments against UTI to avoid any harm during pregnancy. / Tesis
