This thesis is submitted in part fulfilment of the requirement for the degree of Doctor of Clinical Psychology (Clin. Psy. D) at the University of Birmingham. It includes a research project and clinical work undertaken during the course of clinical training. Volume 1 of the thesis consists of the research component in the form of two papers. Both papers are written for submission to specific journals, although text and figures have been integrated into the text to aid description. The first paper in volume 1 is a review of the illness representation literature related to the perceptions of family carers and health professionals involved with direct patient care. This paper was prepared for Psychology and Health. The second paper details the development of a questionnaire of the illness representations of care staff working with people with intellectual disabilities who display challenging behaviour illness. This paper was prepared for Research in Development Disabilities. Included in the appendices to Volume 1 is a public domain briefing summarising the research findings. Other details pertinent to the research are also included in the appendices, including ethical approval documentation. Volume 11 contains five clinical practice reports undertaken in the specialties of adult, child, learning disabilities, older adult and community learning disabilities. The reports include a case study of a 50 year old woman who presented with agoraphobia, formulated from a cognitive and psychodynamic perspectives; a case study of 13 year old boy with a simple phobia; a small scale service related project evaluating the implementation of person centred planning approaches in a local learning disability service; a single-case experimental design examining the efficacy of an intervention with a 77 year old woman experiencing anxiety and a summary of an oral presentation outlining a family behavioural intervention. All names and identifying details have been changed to ensure client confidentiality. Volume 11 contains appendices of information relevant to the clinical practice reports.
