Previous issue date: 2014-09-24 / Coordena??o de Aperfei?oamento de Pessoal de N?vel Superior - CAPES / The aim of this study was to verify the effects of the headgear (HG) with low traction and high traction on the upper molars, maxilla, mandible and maxillomandibular anteroposterior relationship, as the first step of treatment of Class II malocclusion. The sample comprised of 110 radiographies of 55 patients, 10.3 mean age, divided into 2 groups: Group 1 (n= 37), HG low traction; Grup 2 (n=18), HG high traction. Each group patients were treated until a Class I molar relationship were reached. The results showed distal displacement of molars in Group1 (P< 0,001), with higher distal inclination in Group 1 (P< 0,001). Significant extrusion in the molars (P< 0,001) with clockwise rotation of palatal plane (P< 0,05) in Group 1. In Group 2 no estatistical differences were showed in the molars position. No difference was observed in the anterior positioning of maxilla in both groups.Mandibular forward growth was observed in both groups but more mandibular forward group between T1 and T2 in Group 1(P<0,001) and wtih significant difference between the groups (P>0,05). Maxillomandibular improvement was not significant. Mandibular clockwise rotation was not significant. Considering results, it is possible to conclude, that both treatment was efficient, with more significant results in Group 1, but also with more dental collateral effects / Introdu??o: o objetivo deste estudo foi avaliar os efeitos do aparelho extrabucal (AEB) usado com tra??o baixa ou alta, sobre os molares superiores, maxila, mand?bula e a rela??o maxilomandibular como primeira fase de tratamento da maloclus?o de Classe II. Metodologia: Foram avaliadas 110 radiografias de 55 indiv?duos, m?dia de idade de 10.3 anos e com maloclus?o de Classe II divididos em dois grupos; Grupo 1 (n=37), com AEB com tra??o baixa; Grupo 2 (n=18), com AEB com tra??o alta. Os indiv?duos foram tratados at? a rela??o molar de Classe I ser atingida. Resultados: os resultados mostraram deslocamento distal dos primeiros molares superiores no grupo Grupo1 (P< 0,001), com inclina??o distal ainda mais acentuada no Grupo 1 (P< 0,001). Houve extrus?o acentuada do primeiro molar superior permanente (P< 0,001) com rota??o hor?ria do plano palatal (P< 0,05) no Grupo 1. O Grupo 2 n?o apresentou diferen?as estat?sticas significantes no posicionamento do primeiro molar superior. N?o houve diferen?a estat?stica no deslocamento anterior da maxila nos Grupos 1 e 2. Foi observado aumento no comprimento mandibular nos dois grupos com maior aumento comprimento mandibular entre T1(in?cio do tratamento) e T2 (final do tratamento), no Grupo 1 (P<0,001) e com diferen?a significativa entre os grupos (P>0,05). A altera??o da rela??o maxilomandibular n?o foi significante estatisticamente. Houve rota??o anti-hor?ria mandibular, embora n?o sendo estatisticamente significativa. Conclus?o: considerando os resultados ? poss?vel concluir que o tratamento com ambos os protocolos foi eficiente e os resultados foram mais significativos no Grupo 1, por?m com mais efeitos dent?rios indesej?veis.
