Tratamento cirúrgico da síndrome da apnéia e hipopnéia obstrutiva do sono por avanço maxilo-mandibular / Surgical treatment of the osas by maxilomandibular advance

Previous issue date: 2007 / Objetivo: Avaliar se a cirurgia de avanço maxilo-mandibular (AMM) em
pacientes com Síndrome da Apnéia e Hipopnéia Obstrutiva do Sono (SAHOS)
severa é efetiva. Método: Foram inclusos 10 pacientes com índice de apnéia e
hipopnéia superior a 30 eventos por hora de sono, diagnosticados através da
polissonografia. Os pacientes foram submetidos ao exame físico,
polissonográfico, cefalométrico e avaliados subjetivamente através da Escala
de Sonolência de Epworth. Os dados obtidos foram comparados com o mesmo
grupo após seis meses do tratamento cirúrgico de AMM. Resultado: Após o
tratamento cirúrgico de AMM, em relação à avaliação polissonográfica, 20%
dos pacientes tiveram o índice de apnéia-hipopnéia (IAH) normal, 30% dos
pacientes o IAH leve, 10% dos pacientes com IAH moderado e 40% dos
pacientes mantiveram o IAH severo, 60% dos pacientes tiveram diminuição do
IAH de 50% e apenas 10% dos pacientes tiveram o IAH aumentado, após o
AMM. Quanto a média da saturação de oxihemoglobina (SaO2) mínima, no pré-
operatório foi de 63,4%, obtendo melhora significativa após o AMM, sendo
elevada para média de 80,8%, sendo que 70% dos pacientes apresentaram
SaO2 mínima acima de 83%. Em relação a avaliação subjetiva, através da
Escala de Sonolência de Epworth houve melhora de 100% dos pacientes.
Conclusões: Quanto à avaliação objetiva da cirurgia de AMM foi efetiva em
60% dos pacientes com SAHOS severa, sendo que a avaliação subjetiva foi
eficaz em 100%. / Objective: To evaluate the effectiveness of the maxillomandibular
advancement (MMA) surgery in patients suffering from obstructive sleep
apnea/hypopnea syndrome (OSAS) by analysing it objectively and subjectively.
Method: 10 patients suffering from apnea/hypopnea index above 30 events per
sleep hour, diagnosed by polysomnography were examined. The patients
underwent physical, polysomnographic and cephalometric tests and were
subjectively evaluated according to the Epworth Sleepness Scale. The data
obtained was compared to the results of the same group 6 months after the
MMA surgery. Results:After the MMA surgery, as related to an objective
evaluation by polysomnography, the results were the following: 20% of the
patients had a normal apnea/hypopnea index (AHI), 30% of the patients had a
moderate AHI and 40% of the patients kept a severe AHI, 60% of the patients
had a 50% decrease in the AHI and only 10% of the patients had an increase in
the AHI. The average of the lowest oxyhemoglobin saturation (SaO2) in the
preoperative was 63.4%, and a significant improvement was obtained after the
MMA procedure when an average of 80.8%, and 70% of the patients showed a
lowest oxyhemoglobin saturation above 83%. As for the subjective evaluation
using the Epworth Sleepness Scale, there was an improvement in 100% of the
patients. Conclusions: The objective evaluation of the MMA surgery showed
that it was effective in 60% of the patients suffering from severe
apnea/hypopnea index, while the subjective evaluation showed that it was
