En el presente estudio se evaluó un total de 165 Historias Clínicas y radiografías periapicales de pacientes que tuvieron una extracción de alguna pieza dentaria con endodoncia (Fracaso Endodóntico), en el Servicio de Medicina Oral y Cirugía Máxilo Facial del Hospital Central FAP, durante el período de Enero a Diciembre del 2004, con el fin de clasificar las causas fracasos endodónticos, según el Tiempo y el Estado Pulpar preoperatorio de la pieza dentaria, en cuatro grandes grupos: Fracasos Endodónticos propiamente dichos, Fracasos Endodónticos por Causas Protésicas – restauradoras, Fracasos Endodónticos por Causas periodontales y Fracasos Endodónticos por Caries Recidivante. Los resultados mostraron 95 Fracasos Endodónticos por Caries Recidivante (57.6%), 45 Fracasos Endodónticos por Causas Protésicas – restauradoras (27.3%), 13 Fracasos Endodónticos propiamente dichos (7.9%) y 12 Fracasos Endodónticos por Causas periodontales (7.3%). Según el Tiempo de la pieza dentaria con endodoncia, 19 fracasos fueron menor o igual a 1 año (11.5%), 81 fracasos ocurrieron entre 1 – 5 años (49.1%) y 65 fracasos fueron mayor o igual a 5 años (39.4%), no encontrándose una diferencia estadísticamente significativa. Según el Estado Pulpar preoperatorio de la pieza dentaria con endodoncia, 142 fracasos fueron Vitales (86.1%) y solo 23 fracasos fueron No vitales (13.9%), encontrándose una diferencia estadísticamente significativa. El mayor porcentaje de Fracasos Endodónticos propiamente dichos fueron por Fractura Radicular Vertical (53.8%), y el mayor porcentaje de Fracasos Endodónticos por Causas Protésicas – restauradoras fueron por Restauraciones post Endodónticas deficientes (64.4%), y solo 31.3% fueron por Fractura Radicular Vertical. / In the present study it was evaluated a total of 165 Clinical Histories and x-rays films of patients who had a endodontic treated tooth extracted (Endodontic Failure), attended in the Dental and Oral Surgery Department of the Peruvian Air force Central Hospital, during the period of January to December of 2004; the purpose of this study was to classified the causes of endodontic failures, according to the time after the endodontic treatment and the pulpar condition of the endodontic treated tooth, in four main groups: Endodontic failures, Endodontic failures cause by post Endodontic restoration, Endodontic failures cause by periodontal disease and Endodontic failures cause by Recurrent caries. The results showed 95 Endodontic failures cause by Recurrent caries (57.6%), 45 Endodontic failures cause by post Endodontic restoration (27.3%), 13 Endodontic failures (7.9%) and 12 Endodontic failures cause by periodontal disease (7.3%). According to the Time after the endodontic treatment of the endodontic treated tooth, 19 failures were before 1 year (11.5%), 81 failures were between 1 - 5 years (49.1%) and 65 failures were after 5 years (39.4%), not being a difference statistically significant. The highest percentage of Endodontic Failures was by Vertical Root Fracture (53.8%), and the highest percentage of Endodontic Failures cause by post Endodontic restoration were by deficient post Endodontic restorations (64.4%), and only 31,3% were Vertical Root Fracture.
