Stroke är en av de stora folksjukdomarna och den tredje största dödsorsaken efter hjärtinfarkt och cancer. Individuell vård och behandling är av största vikt då sjukdomen drabbar patienter olika. Att drabbas av stroke innebär stora förändringar i det dagliga livet. Sjuksköterskan har en viktig roll i att förbereda patienter och anhöriga inför dessa förändringar. Syftet med studien var att få ökad kunskap om hur strokepatienter och anhöriga förbereds inför hemgång samt vilken roll sjuksköterskan har i förberedelserna angående information, stöd och fysisk rehabilitering. Som metod valdes litteraturstudie utifrån elva vetenskapliga artiklar. I artiklarna kunde tre huvudteman identifieras; fysiskt tillfrisknande, psykologiskt och socialt stöd samt information. Resultatet belyser behovet av god förberedelse inför utskrivning där sjuksköterskan har en viktig roll. Målet med fysiskt tillfrisknande är att strokepatienten ska vara så oberoende som möjligt i hemmet. Patienten och sjukvårdspersonalens mål med rehabilitering överensstämmer inte alltid. Strokepatienter och dess anhöriga har ett stort behov av psykologiskt och socialt stöd, men upplever detta som en bristfällig del i rehabiliteringen. Informationsbehovet är stort hos både patienter och dess anhöriga, men upplevs inte vara tillgodosett. Slutsatser som kan dras utifrån resultaten är att omvårdnaden av strokepatienter i möjligaste mån bör individanpassas efter patientens bakgrund, nuvarande situation och framtida mål. För att patienter och dess anhöriga ska känna sig väl förberedda krävs det att de fysiska, psykologiska och sociala delarna i omvårdnaden integreras i större utsträckning. / Stroke is one of the large national diseases and the third largest cause of death after cardiac infarction and cancer. Individual care and treatment is very important as the disease affects the patients differently. To be affected by stroke implies big changes in the daily life. The nurse plays an important role in preparing the patients and their relatives for these changes. The aim of the study was to get an increased knowledge about how stroke patients and their relatives are prepared before getting home and which role the nurse plays in preparations such as information, support and physical rehabilitation. The method of the present study is a literature review from eleven scientific articles. In the articles three main subjects were identified; physical recovery, psychological and social support and information. The result illustrates the need of sufficient preparations before discharging in which procedure the nurse plays an important role. The goal with physical recovery is that the stroke patient will be as independent as possible at home. Patient’s and health professional’s goal with rehabilitation were not always accordable. Stroke patients and their relatives have a big need of psychological and social support and they experience this as an insufficient part of the rehabilitation. Patients’ and their relatives’ need of information is big, but was experienced as unmet. Conclusions that can be drawn from the results are that nursing of stroke patients as far as possible should be individualized according to the patient’s background, present situation and future goals. To make patients and their relatives feel prepared, it requires that the physical, psychological and social parts are integrated to a greater extent.
