Previous issue date: 2006-01-13 / Oral mucositis is one of the most debilitating complications in patients undergoing bone marrow transplants, occurring 50 to100% in these patients. The oral mucositis treatment and
prevention is not completely defined, although the low intensity laser is widely used. The aim of the current study was to investigate the use of low intensity laser (GaAlAs) in the prevention of oral mucositis induced by conditioning regimen in patients undergoing bone marrow transplants. The study was a clinical trial and a total of 42 subjects undergoing
allogenic and autologus bone marrow transplant at Araujo Jorge Cancer Hospital of the Combact Cancer Association of Goiás were enrolled. All patients received a high dose
chemotherapy conditioning regimen associated or not with total body radiation. The Experimental Group received the hospital protocol of oral hygiene and preventive irradiation
of low level laser (660nm) on day -4 (D-4) through post-transplant day 4 (D+4). The Control Group received just the hospital protocol of oral hygiene. Oral mucositis degree was
evaluated daily according to the World Health Organization Scale (WHO) on day -2 until the wound healing. The Group which low level laser treatment was performed prophylactically, 57,1% of patients were free from mucositis and no one showed mucositis degree III and IV. The difference between both Groups related to the occurrence (p<0,001) and severity (p<0,008) of oral mucositis were statistically meanful. It was concluded that the low level
laser treatment, prophylactically reduces the occurrence and severity of oral mucositis / A mucosite bucal é uma das complicações mais debilitantes dos pacientes submetidos a transplante de medula óssea, com uma ocorrência de 50 a 100% nesses indivíduos. Sua
prevenção e seu tratamento não estão completamente definidos, embora o uso do laser de baixa intensidade seja muito utilizado. O objetivo do presente estudo foi analisar o uso do laser de baixa intensidade do tipo GaAlAs na prevenção de mucosite bucal induzida pelo regime de condicionamento, prévio ao transplante de medula óssea. A amostra foi constituída por 42 pacientes submetidos a transplante de medula óssea autológo e alogênico, admitidos no Hospital Araújo Jorge da Associação de Combate ao Câncer de Goiás (HAJ/ACCG). Todos os pacientes receberam um regime de condicionamento com altas doses de quimioterapia associada ou não à radioterapia corporal total. No Grupo Experimental, a irradiação preventiva com laser de baixa intensidade (660nm) foi realizada nos dias -4 (D-4) ao dia 4 pós-transplante (D+4). O Grupo Controle não recebeu aplicação preventiva com laser de baixa intensidade, sendo que, em ambos os grupos o protocolo de higiene bucal do
HAJ/ACCG foi realizado. O grau de mucosite foi avaliado de acordo com a classificação da Organização Mundial de Saúde (OMS) a partir do D-2, diariamente, até a cicatrização da
mucosite. No Grupo que recebeu o tratamento preventivo com o laser de baixa intensidade, 57,1% dos pacientes ficaram livres de mucosite e nenhum apresentou mucosite Grau III e IV. A diferença entre os Grupos foi estatisticamente significante quanto à ocorrência (p<0,001) e intensidade (p<0,008) da mucosite bucal. Concluiu-se que o laser de baixa intensidade, utilizado preventivamente, reduz a ocorrência e intensidade das lesões de mucosite bucal.de Medula Óssea
