Estudo Nutricional e Qualidade de Vida em Doentes com Cancro do Pulmão não Pequenas Células

O cancro e o seu tratamento encontram-se associados a uma deterioração do
estado nutricional e da qualidade de vida (QdV), mas não se conhece a
dimensão destas consequências nos doentes com Cancro do Pulmão Não
Pequenas Células (CPNPC). O rastreio deve ser realizado para identificar os
doentes que se encontram em risco de desnutrição. Contudo, a única
ferramenta recomendada para avaliação nutricional de doentes oncológicos, o
Patient Generated-Subjective Global Assessment (PG-SGA), não é adequada
para o rastreio.
Objectivos:
Caracterizar o estado nutricional, identificar o método mais apropriado para o
rastreio da desnutrição e avaliar a associação entre a QdV e o estado
nutricional em doentes com CPNPC.(...) / Introduction: Cancer and its treatment are related to a deterioration of nutritional status and quality of life (QoL), but the extent of these conditions in patients with Non Small Cell Lung Cancer (NSCLC) is not known. Screening must be performed to identify individuals at risk of undernutrition. However, the only tool recommended for nutritional assessment, is the Patient Generated Subjective Global Assessment (PG-SGA), which is not appropriated for nutritional screening. Objectives: To characterize the nutritional status, to identify the most suitable method for screening undernutrition and to evaluate the association between the QoL and nutritional status in patients with NSCLC. Participants and Methods: A cross-sectional study was conducted with 56 outpatients with NSCLC. PG-SGA was used to evaluate the nutritional status.. QoL assessment was performed using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-C30 (EORTC QLQ.C30) and the specific module for patients with Lung Cancer (EORTC QLQ-LC13). The diagnostic values of handgrip strength, of Malnutrition Universal Screening Tool (MUST). and other methods of nutritional screening/ assessment, using the PG-SGA as a reference was evaluated Results: According to PG-SGA, 35.7% of patients are undernourished (1.8% early stages of disease vs 33.9% advanced stages of disease) and according to MUST, 41.1% of patients are at risk of undernutrition (3.6% early stages of disease vs 37.5% advanced stages of disease). MUST, in association with the arm muscle circumference was the best screening tool compared with the PG-SGA. There were no significant differences in the evaluation of QoL between disease stages. Stratifying data for nutritional status, differences for various parameters emerged. (...)
