Dissertação de Mestrado em Nutrição Clínica apresentada à Faculdade de Ciências da Nutrição e Alimentação da Universidade do Porto / Resumo da tese: O objectivo deste trabalho foi estudar, numa população de doentes em programa regular de hemodiálise, a relação entre alguns indicadores de estado nutricional, factores relacionados com tratamento dialítico e outros dados analíticos bioquímicos com o seu estado: doentes que não foram internados nem morreram (grupo A), doentes que sofreram pelo menos um internamento hospitalar (grupo B) e doentes que morreram (grupo C). Avaliaram-se 62 doentes da Clínica de Hemodiálise Nossa Senhora da Franqueira em Barcelos quanto aos seguintes parâmetros: antropometria (IMC, percentagem do peso seco actual em relação ao peso de referência), albumina, prealbumina, nPCR, creatinina e colesterol total; Kt/V e TRU (taxa de remoção da ureia); ureia pré - diálise, fósforo, transferrina, proteína C - reactiva, TGP/ALT, triglicerídeos e aumento de peso inter-dialítico (percentagem em relação ao peso seco). Consideraram-se significativas diferenças e correlações com %0,05. No início do estudo os doentes apresentaram valores médios dentro das recomendações para todos os parâmetros estudados. No estudo das correlações entre as variáveis estudadas verificou-se que a prealbumina e a albumina foram os parâmetros que maior número de correlações apresentaram com as outras variáveis, seguidos da ureia pré-diálise, nPCR, creatinina e fósforo. Nenhum dos resultados analíticos em estudo apresentou correlação estatisticamente significativa com o aumento de peso inter-dialítico, o que poderá significar que os mesmos não são afectados pelo efeito de hemo-diluição. / Thesis abstract: The objective of this work was to study, in a population of patients in regular program of hemodialysis, the association between some indicators of nutritional status, factors related with dialytic treatment and other analytic biochemical data, with their state: patients that were not hospital admitted neither died (group A), patients that suffered at least one hospital admission (group B) and patients that died (group C). 62 patients of the Clínica de Hemodiálise Nossa Senhora da Franqueira in Barcelos (Portugal) were evaluated as regards the following parameters: anthropometry (BMI and percentage of the current dry weight regarding their reference weight); albumin, prealbumin, nPCR, creatinine and total cholesterol; Kt/V and URR; pre-dialysis urea, phosphorus, transferring, c-reactive protein, TGP/ALT, triglycerides and between dialysis increase of weight (percentage in relation with dry weight). Differences and correlation with %0,05 were considered significant. In the beginning of the study patients presented medium values inside the recommendations for all evaluated parameters. In the study of correlation between the variables evaluated, prealbumin and albumin were the parameters that bigger number of correlation presented with the other variables, followed by the pre-dialysis urea, nPCR, creatinine and phosphorus. None of the analytic results presented significant statistical correlation with the between dialysis increase of weight, what can mean that they are not affected by the effect of hemo-dilution. It was verified that hepatic enzymes (TGP/ALT) were not significant correlated with any analytical parameters, to the contrary of C-reactive protein that was negatively correlated with transferrin, albumin and prealbumin in the beginning of the study and with all of the parameters except total cholesterol and triglycerides in the last analysis.
