Substance abuse is an issue that is extremely topical in the society today. There are many factors that is affected by a person having a substance abuse, and the work situation is one of them. Previous research shows that a person that has a substance abuse affects the entire workplace on many levels, and it is important for the working group to have a leader with good leadership skills in such complex situations. The purpose of this study was to examine how leaders in eldercare in a medium sized municipality can handle the situation if someone in their staff have a problem with substance abuse. The theoretical frameworks that has been used to analyze the result are theories about leadership and medicalization theory. The results of the study consists of four interviews. The processing of the material was done by qualitative content analysis and that generated in five categories. The compilation of the material could demonstrate that substance abuse were not that common in the workplaces covered by our study. Something that emerged were that when a substance abuse was discovered on the workplace, the abuse was often very serious. The four leaders that were interviewed for this study could tell that they were well informed and knew how to act if a situation like this would arise within their staff, and insisted on importance of promptly taking action. Something that was prominent during all the interviews was that there were often subtle indications that could indicate that something was wrong, and repeated absence was such a factor. The interviewees also agreed that it was important not to ignore their gut feeling, because they could often get a intuitive feeling when someone in the staff was not feeling well. Something that emerged was that all interviewees considered abuse as a disease condition that can be recovered from.
