Estudo video-polissonografico em recem-nascidos de termo pequenos para a idade gestacional e em recem-nascidos de termo com intercorrencias perinatais leves

Previous issue date: 1999 / Resumo: A vídeo-polissonografia neonatal embora tenha o seu uso consagrado em situações clínicas e laboratoriais como na avaliação da idade conceptual (Nunes & Moura-Ribeiro 1997), na apnéia do recém-nascido (Ferber & Kryger, 1995), nas manifestações epilépticas neonatais(Aicardi, 1994; Mizrahi & Kellaway, 1998); na encefalopatia hipóxico-isquêmica isquêmica - (Lombroso 1993, Lieshout et al 1995, Hellstrom-Westas et al 1995) e no prognóstico do desenvolvimento de recém-nascidos pré-termo (Marret et al 1997), ainda não foi explorada em toda sua potencialidade. Trata-se de exame inócuo, com a vantagem de informar sobre condições funcionais do sistema nervoso. A presente pesquisa tem a finalidade de ampliar o conhecimento sobre o uso deste exame no período neonatal aplicando-o em recém-nascidos de termo pequenos para a idade gestacional , e em recém¬ nascidos de termo com peso adequado para a idade gestacional, com intercorrências leves no período perinatal. Foram analisadas as vídeo-polissonografias de 41 recém-nascidos com 24 horas de vida, com exames clínico, neurológico e ultra-sonográfico cerebral normais. Foram subdivididos em três grupos: Grupo Controle, constituído por 11 recém-nascidos de termo com peso adequado para a idade gestacional e isentos de quaisquer intercorrências perinatais; Grupo de Recém-Nascidos de Termo Pequenos para a Idade Gestacional (RNT¬ PIG), constituído por 15 neonatos; e Grupo de Recém-Nascidos de Termo com peso adequado para a idade gestacional com Intercorrências Perinatais Leves (RNT-IL), constituído por 15 recém-nascidos. No Grupo de Recém-Nascidos de Termo Pequenos para a Idade Gestacional 13 apresentaram algum tipo de alteração na avaliação vídeo-polissonográfica, e no Grupo de Recém-Nascidos de Termo, peso adequado com Intercorrências Leves, 14 / Abstract: The vídeo-polygraphic-EEG study has not yet been well performed in alI its potentiality, though it is welI used in situations such as in conceptual age evaluation (Nunes & Moura-Ribeiro, 1997), at the newborn premature development folIow-ups (Marret et al, 1997), hypoxic-ischemic encephalopathy (Lombroso 1993, Lieshout et al 1995; HelIstrom-Westas et al, 1995) the newborn apnea and in the neonatal seizures (Aicardi, 1994; Mizrahi et ai, 1987 e 1998). It is a non-agressive exam and its aim is to inform us about the neurological system functional disorders. This study has worked in a wider field, using this exam in fulI-term low-birth weight newborns with no intercurrencies, and fulI-term, normal-weight newborns, with light intercurrencies at the perinatal period. We have analysed the video polygraphic EEG study on 24 life-hours of 41 newborns with normal clinical, neurological examinations and ultrassonographic study: results. They were all divided into three groups as follows: the control group, 11 full-term normal-weight, no perinatal injuries newborns, the fulI-term low-birth weight 15 newborns and the fulI-term normal weight 15 newborns with light intercurrencies at the perinatal period. At the second group we found 13 newborns with the video polygraphic exam alterated. The same for the third group, with video polygraphic exams in 14 newborns, showing alteration. We found in the second group, sleep architecture alterations in 11 newborns, of which 5 presented sleeping start in quiet sleep; 4 presented periods of inter-hemispherical asynchonism, and in 2 cases, during the exam period, many changes of states. Also, 1 case of quiet sleep absence and I case of active sleep absence. As far as behavior activities are concerned, we had 10 newborns with behavior alterations of which 8 presented an excessive "startles" number, 2 with a highly motion index and 2 with a low motion one. We have also found excessive respiratory pauses in 3 newborns. From the Iast group of 15 newborns, 14 had exam alterations. 8 newborns had sleep architecture alterations being quiet sleep absence in 2 of them, periods of inter¬ hemispherical asynchronism in 2 and many changes of states in 1. As for behavior, 8 had an excessive "startles" number; low motion index in 1 and high motion index in 1, too. It was also found excessive respiratory pauses in 3 newborns. As to the statistical analysis the Godmann test (1964 and 1965) was applied for contrasts between and withen multinomial populations. The discussions were performed at a level of significant 5%. Statistically significant differences in response rate were observed in the following variables such as: birth delivery types, video poligraphic EEG study alterations presence, sleep-architecture alterations, behavior standard changes, excessive "startles" number, motion index in quiet sleep, and transient multifocal activities / Doutorado / Neurologia / Doutor em Ciências Médicas
