Influ?ncia da inclina??o do implante distal e da extens?o do cantilever na remodela??o ?ssea periimplantar em pr?teses totais fixas : um estudo metodol?gico em tomografias computadorizadas

Previous issue date: 2014-03-31 / Studies have been shown that for the treatment of the edentulous arches with Branemark protocol the inclination of the most posterior implants with distal cantilever extensions does not appear to have harmful effects on the bone structure. The objectives of this study were; 1) to develop a methodological approach for the evaluation of implant inclination and periimpllant bone remodeling (PBR) through the cone beam computer tomography (CBCT), 2) to investigate the influence of the implant inclination and cantilever extension on the PBR of most distal implants adjacent to prosthetic cantilever in fixed implant-supported complete dentures (FISCD). Twenty distal implants of 10 FISCD were evaluated from 7 patients treated at the clinics of the Faculty of Dentistry of PUCRS. To measure the PBR and inclination of the implants, CBCT scans at the prosthetic delivery phase and 1 year after delivery were used. With the Kodak CS 3D software the central region of the implant was determined and reference lines were traced on coronal and sagittal planes, on the center of the implant and perpendicular to this central line at the apex of the implant in order to measure the periimplant bone level (PBL). To evaluate the mesio-distal and vestibulo-lingual/palatinal implant inclination a reference line was traced on the occlusal plane and the angle between the central line of the implant was measured. The data of mesial, distal, vestibular and lingual/palatinal PBLs were collected in two moments to evaluate PBR in 1 year. The results show that, inclination of the implants adjacent to cantilever presented variations in mesio-distal and vestibulo-lingual/palatinal directions relative to occlusal plane. In general, bone loss occurred on mesial, distal, vestibular and lingual/palatinal faces of the distal implants. Higher bone loss values were obtained on distal and vestibular faces. A direct moderate/strong correlation was detected between cantilever extension and distal PBR in maxilla. This study has demonstrated that, the proposed methodological technique and was reliable and reproducible to evaluate PBR in mesio-distal and vestibulo-lingual/palatinal dimensions. / Estudos t?m mostrado que para o tratamento de rebordo ?sseo desdentado, atrav?s do tratamento tipo protocolo, a inclina??o dos implantes mais posteriores com o uso de cantilever prot?tico parece n?o ter efeitos prejudiciais ? estrutura ?ssea. Assim, os objetivos deste estudo foram: 1) desenvolver uma metodologia para avaliar a inclina??o dos implantes distais e avaliar a remodela??o ?ssea periimplantar com o aux?lio de tomografia computadorizada cone beam (CBCT); e 2) testar a influ?ncia da inclina??o do implante mais distal e da extens?o do cantilever prot?tico sobre a remodela??o ?ssea periimplantar. Foram avaliados 20 implantes distais de 10 Pr?teses Totais Fixas Implantossuportadas (PTFIs) em 7 pacientes atendidos na Faculdade de Odontologia da PUCRS. Para as mensura??es de remodela??o ?ssea de inclina??o dos implantes foram utilizadas aquisi??es atrav?s de CBCT?s no momento da instala??o da pr?tese e ap?s um ano. Com aux?lio de ferramentas do software CS 3D da Kodak, a regi?o mais central do implante foi determinada para que linhas de refer?ncia pudessem ser tra?adas no plano coronal e sagital, na regi?o central e perpendicular a esta no ?pice do implante para mensura??o do n?vel ?sseo periimplantar. Para avaliar a inclina??o dos implantes no sentido m?sio-distal e vest?bulo-lingual/palatino foi tra?ada uma linha no plano oclusal, sendo mensurada a inclina??o com a linha central do implante. Os dados do n?vel ?sseo mesial, distal, vestibular e palatino/lingual foram coletados em dois momentos para avaliar a remodela??o em um ano. Os resultados mostraram que a inclina??o do implante adjacente ao cantilever apresentou varia??es nos sentidos vest?bulo-lingual e m?sio-distal, em rela??o ao plano oclusal. Em m?dia houve perda ?ssea nas faces mesial, distal, vestibular e lingual dos implantes adjacentes ao cantilever, sendo que os maiores valores ocorreram nas faces distal e vestibular. Houve correla??o direta moderada/forte entre a extens?o do cantilever da PTFI e a remodela??o ?ssea distal em maxila. Este trabalho demonstrou que a t?cnica de obten??o e an?lise das imagens tomogr?ficas ? confi?vel e reprodut?vel para avaliar a perda ?ssea periimplantar nas faces mesial, distal, vestibular e lingual/palatina, bem como para avaliar as inclina??es m?sio-distal e vest?bulo-lingual/palatino dos implantes.
