Recentemente descrito, o metapneumovirus foi associado a infeccoes respiratorias em criancas, ocorrendo com maior frequencia ate seus cinco anos de idade. Investigacoes recentes o identificaram tambem em adultos e imunocomprometidos. Nesta conjuntura conduziu-se em São Paulo um estudo com coleta sistematizada e consecutiva por tres anos (06/2001 a 09/2003) em tres populacoes distintas (adultos do Pronto Atendimento, transplantados renais e profissionais de Saúde) apresentando sintomas respiratorios. Cento e oitenta e cinco pacientes com resultados negativos para outros virus por outros metodos (influenza A e S, Parainfluenza 1,2 e 3, RSV, adenovirus, coronavirus e enterovirus) foram investigados quanto a presenca de metapneumovirus por uma RT-PCR otimizada em laboratorio. Vinte e quatro pacientes (12,97 por cento) apresentaram amplificacao especifica para o virus, caracterizada pelo fragmento de 347pb amplificado pelos iniciadores correspondentes ao gene F. O virus foi detectado nos meses de junho a setembro dos tres anos investigados. Houve maior frequencia na deteccao em 2003. As frequencias observadas ao final do estudo nos profissionais de Saúde, adultos do Pronto Atendimento e pacientes transplantados renais foram 10,75 por cento (10/92), 18,96 por cento (11/59), e 8,82 por cento (3/34) respectivamente. Verificou-se uma tendencia associativa entre contato com paciente e infeccao por hMPV nos profissionais de Saúde. Foi encontrada associacao entre infeccao e adultos acima de 51 anos (p=0,006). Os pacientes transplantados apresentaram sintomatologia menos expressiva. Nao foi possivel atribuir uma sintomatologia especifica a infeccao por hMPV / Viral respiratory infections are common morbi-mortality causes in adults and in the elderly. Recent advances in viral diagnostics, such as RT-PCR, allowed for the detection of a plethora of new viruses, until then uncharacterized. The recently described human metapneumovirus was associated with respiratory infections by van den Hoogen and coworkers while investigating samples from children without etiologic causes attributable. Researchers worldwide detected the virus in samples from children and recently in adult’s samples as well. Following this approach our study was aimed at adults over eighteen presenting with acute respiratory infections from June/2001 to September/2003. Three groups were enrolled: patients from an out-patient attendance, health care workers and renal transplant recipients. All samples were submitted to routine diagnostics screening through direct fluorescence assay provided by Light Diagnostics for seven different viruses and negative results were set up for RT-PCR with primers selected to hMPV´s F gene. Twenty four out of 185 patients (12.97%) were considered positive after amplification of a 347bp fragment. The virus was detected from June to September in the 2001-02 period, peaks were higher in 2003 and began earlier, as discussed below. Furthermore an association was found between infection and age (p=0,006) for the whole cohort. Out-patient attendance cases exhibited a higher detection frequency (18.92%/11:59), particularly in 2003. Health care workers and ambulatory patients had 10.75% (10:92) and 8.82% of infections attributed to hMPV. A weak tendency was found regarding infection and patient contact in the health care workers population. Transplant infections were less symptomatic than the other groups evaluated.
