O primeiro artigo teve porfinalidade analisar a teoria salutogênica buscando estabelecer suas interfaces com a promoção da saúde bucal. Realizou-se uma reflexão no plano teórico-conceitual da teoria salutogênica visando analisar suas potencialidades para orientar o desenvolvimento de ações nos cinco campos propostos pela Carta de Otawa. O segundo artigo objetivou analisar as contribuições da teoria salutogênica para o desenvolvimento de ações educativas em saúde bucal voltadas para o empowerment de indivíduos e grupos, tendo como pano de fundo a pedagogia problematizadora. A metodologia desse estudo baseou-se em uma reflexão no plano teórico-conceitual dasalutogênese e suas interfaces com a educação problematizadora, elegendo a categoria empowerment como eixo estruturante dessas relações.O terceiro artigo teve por objetivo gerar evidências da teoria salutogênica nocampo da saúde bucal. O estudo investigou a associação entre o senso de coerência (construto central da salutogênese) e desfechos favoráveis à saúde bucal dos adolescentes. O trabalho considerou enquanto desfechos favoráveis à saúde bucal (1) o fato dos adolescentes com 11-12 anos de idade já terem utilizado o serviço de saúde odontológico e (2) o principal motivo de uso desses serviços ter sido por razões preventivas (e não curativas). Foi utilizada uma amostra de adolescentes de baixo extrato socioeconômico dentro do município de São João de Meriti, RJ. Os resultados dos estudos apontaram que o arcabouço teórico-conceitual da salutogênese mostrou-se consistente, apresentando potencial para subsidiar as ações de promoção da saúde bucal. Ademais, a adoção da perspectiva salutogênica e da educação problematizadora no âmbito da educação para a saúde bucal pode ser capaz de viabilizar o resgate do posicionamento da promoção da saúde em relação ao seu compromisso com a produção da saúde e de sujeitos reflexivos e autônomos favorecendo, dessa forma, a transformação da situação sócio-sanitária. A perspectiva salutogênica sugere que as práticas educativas em saúde bucal voltem-se para a co-investigação dos recursos salutogênicos junto aos indivíduos e grupos, sendo o respeito aos valores morais condição essencial para eficácia dessa estratégia. Os resultados do estudo epidemiológico contribuíram para a própria consolidação da salutogênese no campo da promoção da saúde bucal na medida em que concluiu que o senso de coerência materno esteve associado com a adoção de comportamentos favoráveis à saúde bucal do adolescente, quais sejam: o fato dos adolescentes já terem utilizado o serviço odontológico e na maioria das vezes por razões preventivas. Os resultados apontaram que quando os fatores estruturais são desfavoráveis à saúde, ações de promoção da saúde bucal que fazem uso da abordagem salutogênica junto às mães podem ser capazes de gerar resultados favoráveis em termos de saúde bucal para seus filhos. A tese, como um todo, aponta as potencialidades da teoria salutogênica para renovar as pesquisas e as práticas em saúde bucal. A salutogênese confere importância à distribuição mais igualitária de poder e de recursos no âmbito da sociedade, buscando resgatar o compromisso do Estado, da sociedade e dos trabalhadores em saúde bucal com a vida dos indivíduos e coletividade. Indica que processos de empowerment comprometidos com o aumento da compreensão, coerência e domínio dos indivíduos sobre os aspectos individual, social, econômico e político podem viabilizar que melhores condições de vida (e de saúde bucal) sejam alcançadas. Aponta também os desafios que abordagens dessa natureza representam para a consolidação da saúde bucal coletiva, ao engendrar mudanças nos processos de trabalho e nos sujeitos desses processos. / The present thesis was presented in the format of scientific articles. The purpose of the first article was to analyze the salutogenic theory seeking to establish their interfaces with oral health promotion. There was a discussion on theoretical and conceptual aspects of the salutogenic theory to examine their potential to guide the development of actions in five fields of action proposed by the Ottawa Charter. The second article aimed to analyze the contributions of salutogenic theory for the development of education in oral health towards the empowerment of individuals and groups, considering the problematizing pedagogy as the backdrop. The
methodology of this study was based on the reflection on the theoretical and conceptual
frameworks of the salutogenesis and of their interfaces with problematizing education, considering empowerment the category axis that can structure these relations. The third article aimed to generate evidence of the salutogenic theory in the field of oral health. The study investigated the association between sense of coherence (the central construct of salutogenesis) and favorable outcomes of adolescents’ oral health. The favorable oral health outcomes in adolescents were: (1) the fact of adolescents 11-12 years of age had used oral health services and (2) the main reason for using these services have been for preventive purposes (rather than "curative"). A sample of adolescents from low socioeconomic level was gathered within the São João de Meriti city, RJ. The results of studies showed that the salutogenesis theoretical and conceptual framework proved to be consistent, showing potential to support actions to promote oral health. Adopting the perspective of salutogenesis and problematizing education in oral
health education may be capable to make the rescue of the position of health promotion
in relation to its commitment to subjects’ health production and of reflective and autonomous individuals, which in turn, favors the transformation of the social-health situation. The salutogenesis perspective suggests that oral health education practices should be moved for the co-research of “salutogenic” resources of individuals and groups. The respect for moral values is an essential condition for the effectiveness of this strategy. The results of the epidemiological study contributed to the consolidation of salutogenesis in the field of oral health promotion as it was concluded that maternal sense of coherence was associated with the adoption of favorable adolesccents’ oral
health behaviours, namely: the fact that adolescents had used the dental services and
most of the time for preventive reasons. The results showed that when the structural factors are unfavorable to health, oral health promotion actions using the salutogenic approach with the mothers may be capable to generate favorable oral health outcomes for their adolescents. The thesis highlights the potential of the salutogenic theory to renew the research and practice in oral health. The salutogenesis supports the importance to the more egalitarian distribution of power and resources within the society, seeking to redeem the commitment of the State, society and workers in oral health with the individuals’ life and community. It indicates that processes of empowerment committed to increasing the understanding, coherence and domain of subjects on the individual, social, economic and political aspects can enable better life conditions (and oral health) can be reached. It is also suggested the challenges that such approaches pose to the consolidation of collective oral health and to generate shifts in the work processes and in the subject of those proceedings.
