Este trabalho tem como objetivo geral a investigação sobre a experiência do \"outro em si\", tal como nos é revelada na clínica psicanalítica e na interlocução com a equipe. Os objetivos específicos são: formalizar teoricamente o processo de inserção e a construção do lugar do psicanalista na equipe, a partir de nossa experiência na Disciplina de Transplante e Cirurgias do Fígado do HCFMUSP, dado que a forma pela qual um psicanalista responde a essas demandas é o que possibilita ou não a sustentação de sua atuação clínica na instituição; analisar situações clínico-institucionais nas quais o psicanalista está inserido, na interlocução com os outros discursos, tomando como referência as diferentes ancoragens éticas, de modo a compreender sua participação no campo das decisões e as conseqüências disso para ele, para o paciente e para a instituição; fazer a diferença do conceito de corpo na Psicanálise e na Medicina para dar subsídios teóricos ao trabalho do psicanalista; formalizar teoricamente o transplante de fígado como experiência de corporeidade e subjetividade, simultaneamente, a partir da clínica psicanalítica com os pacientes, analisando o processo de adoecimento, suas repercussões psíquicas, a incidência do transplante no corpo, a experiência do \"outro em si\" e suas conseqüências. Chamamos de vertente institucional o trabalho do psicanalista na interlocução com os outros discursos, e de vertente clínica o trabalho do psicanalista com os pacientes quesão convocados à experiência do \"outro em si\". Partimos do pressuposto fundamental de que o trabalho do psicanalista na instituição se dá na interface das duas vertentes. As situações clínico-institucionais são colocadas de modo a privilegiar a indissociabilidade das duas vertentes, que nos parece ser o que há de específico do trabalho do psicanalista na instituição de saúde. A experiência do \"outro em si\" é da ordem da estranheza, e só pode ser definida a (cotinua) (continuação)partir da relação que cada sujeito estabelece com sua subjetividade, dado que é a experiência que lhe dá acesso a esse saber. Assim como a entrada do psicanalista na equipe não corresponde à sua inserção, o enxerto do novo órgão na situação do transplante não corresponde à sua incorporação. Em ambos os casos, convém que essa estranheza seja preservada para que possa ser tratada, para que cada sujeito e cada equipe possam lidar com ela a partir de seus próprios recursos. Isso gera conseqüências no campo das decisões na instituição. O psicanalista introduz a clínica do sujeito no serviço de saúde ocupando o lugar do \"outro em si\" da equipe. Sua permanência na equipe implica o resgate da clínica médica propriamente dita, esta que se vê tão ameaçada de extinção pelo casamento da ciência com a bioeconomia tecnológica / The general aim of this thesis is to investigate the experience of the \"other in oneself\' as it is revealed through the psychoanalytic clinic and communication between team members. The specific objectives are: to formalize the insertion process and construction of the position of the psychoanalyst in the team on a theoretical basis, according to our experience in the Discipline of Transplant and Surgery of Liver of the General Hospital of the Medical University of the State of São Paulo (HCFMUSP), Brazil, once the way a psychoanalyst responds to the demands is what makes the support of the clinical work in the institution possible or not; to analyze clinical and institutional situations in which the psychoanalyst is inserted and the communication with other discourses, taking as reference the different ethical support so that it is possible to understand the participation in the area of decisions and its consequences for the analyst, for the patient and for the institution; to differentiate the concept of body in psychoanalysis and in medicine, to give theoretical support to the psychoanalyst work; to formalize the liver transplant as a physical and subjective experience on a theoretical basis and simultaneously, consider the psychoanalytic experience with the patients, analyzing the sickness process, its psychic repercussions, the incidence of the transplant on the body, the experience of the \"other in oneself\'\" and its consequences. The work of the psychoanalyst inthe interface with other discourses is named the institutional current and his work with the patients who are called to experience the \"other in oneself\" is the clinical one. We assume the fundamental presupposition that the work of the psychoanalyst in the health institution is held in both directions. The clinical institutional situations are set in order to privilege the interdependence of the two (continue) (continuation)currents, what seems to be the specific aspect of the psychoanalyst work in the health institution. The experience of the \"other in oneself\" brings a sense of weirdness and can only be defined according to the relation that each subject establishes with his subjectivity since it is the experience that gives access to this subjective knowledge. As well as the psychoanalyst insertion in the team does not mean that it will incorporate him, the insertion of a new organ in the transplant situation does not mean it will be integrated by it. In both cases it is advisable that this weirdness is preserved, so that each subject and each team can handle it with their own resources. It brings consequences to the field of decisions in the institution. The psychoanalyst introduces the clinic of the subject in the health service occupying the position of the \"other in oneself\"\' in the team. Strictly speaking, bis permanence in the team implies the rescue of the medical clinic that has been threatened with the extinction of the science partnership withtechnological bioeconomy
