Uma vis?o psicanal?tica sobre pacientes com transtornos alimentares / A psychoanalytical view on patients with eating disorders

Previous issue date: 2015-02-24 / Considering that eating disorders are pathologies whose incidence is growing in contemporary society and that they may lead to death or physical and emotional disability, this work aims to understand the suffering of patients with eating disorders, in order to enable theoretical considerations which shed light on the clinical practices in this context. It is qualitative research with psychoanalytic orientation, whose Winnicottian approach is based on the assumption that the eating disorders constitute a defensive organization of double
function, one that reports on a feeble psychosomatic integration while another attests to the hope that the integration process might be resumed. As participants of this study, four adult volunteer patients were invited amidst those who seek the services of a school clinic of a university in the state of S?o Paulo, Brazil. As research procedures, initial interviews were carried out, inspired by Winnicott s therapeutic consultations, aiming to establish a psychoanalytical diagnosis and a therapeutic orientation for each patient, a framework that is already part of the care protocol at the institution where this research was developed, and also psychotherapy sessions with the participants. The interviews and the sessions were registered soon after each encounter with the participants, in the form of narratives, which were examined in the light of Winnicott s psychoanalysis. The clinical material indicates experiences which could not be lived, in a field of unthinkable anxieties, triggering defense mechanisms that threaten the psychosomatic integration of the individual at the same time in
which they seek to reestablish that lost integration. The suffering of the patient s family needs to be addressed, and that is most important to the patient s treament because the family will then become supportive of the treatment. Furthermore, the role of the thin culture in that kind of suffering is emphasized. During the encounters with the participants of this study, one was also able to attest that a therapeutic relation aligned with the emotional needs of the patient with eating disorders is a necessary condition to deal with extreme experiences that are characterized either by excesses or by privation without ever reaching a point of balance or rest. / Considerando que os transtornos alimentares s?o patologias cuja incid?ncia ? crescente no cen?rio contempor?neo e podem levar ? morte ou ? incapacita??o f?sica e emocional, esta pesquisa tem o objetivo de compreender o sofrimento de pacientes com transtorno alimentar, a fim de possibilitar reflex?es te?ricas que iluminem as pr?ticas cl?nicas nesta ?rea. Trata-se de pesquisa qualitativa de orienta??o psicanal?tica, cujo enfoque winnicottiano apoia-se sobre o pressuposto de que o transtorno alimentar constitui-se como
organiza??o defensiva de dupla fun??o, uma que informa sobre uma d?bil integra??o psicossom?tica, enquanto outra atesta a esperan?a de que o processo de integra??o seja retomado. Como participantes deste estudo foram convidados quatro pacientes adultos volunt?rios dentre aqueles que buscam o servi?o de uma cl?nica-escola de uma universidade do interior de S?o Paulo. Como procedimento investigativo foram realizadas entrevistas iniciais inspiradas nas Consultas Terap?uticas de Winnicott, com o objetivo de estabelecer um diagn?stico psicanal?tico e uma orienta??o terap?utica para cada paciente, enquadre que j? faz parte do protocolo de atendimento da institui??o onde foi desenvolvido este estudo, al?m de sess?es de psicoterapia com os participantes. As entrevistas e as sess?es foram registradas logo ap?s o encontro com cada um dos participantes, sob a forma de
narrativas as quais foram analisadas ? luz da psican?lise winnicottiana. O material cl?nico assim obtido aponta para experi?ncias que n?o puderam ser vividas em um campo de
ang?stias indiz?veis, ensejando defesas que amea?am a integra??o psicossom?tica do individuo, ao mesmo tempo que buscam restabeler a integra??o perdida. Destacamos a necessidade de um trabalho de sustenta??o do sofrimento da fam?lia do paciente que, dessa forma, passa a ser coadjuvante de seu tratamento, al?m do papel da cultura da magreza nesse tipo de sofrimento. Pudemos ainda atestar nos encontros com os participantes deste estudo que uma rela??o terap?utica afinada ?s necessidades emocionais do paciente com transtorno alimentar ? condi??o necess?ria para lidar com experi?ncias extremas que s?o marcadas pelo excesso ou pela priva??o sem jamais encontrar um ponto de equil?brio ou de repouso.
