Previous issue date: 2003 / A realizacao deste estudo proporcionou a construcao de uma ficha para registros do atendimento inicial na sala de emergencia clinica e cirurgica, de um hospital escola de porte extra, para ser preenchida pelos enfermeiros. 0 estudo incluiu varias etapas, sendo a primeira construcao da ficha, a segunda a validacao de seu conteudo pelos juizes, e a terceira a aplicacao da ficha na pratica clinica pelos enfermeiros do pronto socorro. Para o procedimento de validacao foram selecionados 5 enfermeiros docentes com titulo de mestre ou doutor, experientes no ensino de emergencia em cursos de graduacao, e 5 juizes enfermeiros que atuam na sala de emergencia por mais de cinco anos. Apos a apreciacao dos juizes, o conteudo da ficha foi readequado, de acordo com as sugestoes propostas e os criterios estabelecidos para o estudo. Em seguida, a ficha foi aplicada na sala de emergencia, no periodo de 04/10 a 02/11/2002 por 18 enfermeiros que atendem as emergencias, para que avaliassem a sua aplicabilidade e a necessidade de acrescentar mais informacoes quando necessario. Foram preenchidas 196 fichas, sendo 146 referentes aos atendimentos cirurgicos e 50 aos atendimentos clinicos. A analise dos dados obtidos levou em consideracao o preenchimento de topicos obrigatorios e nao obrigatorios da ficha. Constatou-se que o conteudo da ficha atendeu as necessidades dos registros das emergencias clinicas e cirurgicas pelo enfermeiro, e que sua aplicabilidade e viavel uma vez que atingiu indice representativo de preenchimento dos topicos. Com este estudo, espera-se enriquecer os registros de enfermagem, fortalecer os registros medicos, contribuir com a continuidade dos tratamentos durante a internacao, servir de respaldo legal e subsidiar cientificamente futuras pesquisas / This study was developed with the aim of building a flow sheet for the initial assessment of patients admitted in the clinical and surgical emergency rooms of a large public school-hospital, to be used for assistance nurses. The two methodological steps were: (1) the validation of the flow sheet contents by ten judges, and (2) the practical use of the flow sheet in the routine work in the clinical and surgical emergency rooms. Five
emergency nursing professors for graduate students, with Master or PhD degrees, and 5 nurses working on emergency rooms for more than five years performed the sheet validation. Their suggestions were appreciated
and contributed to the improvement of the sheet that was applied in the second or experimental phase of the study. Eighteen emergency nurses applied the sheet from 04/10 to 02/11/2002 and they also commented on
the adequacy of the items and suggested alterations. A total of 196 sheets were applied, of which 146 in surgical emergencies and 50 in clinical emergencies. All of the data obtained were analyzed, either of items
considered obligatory or not. This study demonstrated that a registration sheet is a useful tool in emergency rooms, with a large number of items being filled out and causing no disturb to the patient assistance, and that
the experimental sheet contents are convenient for the adequate assessment of emergency data. The routine use of an emergency flow sheet improved the nurse and medical documentation, contributed to the treatment of all patients in the course of their hospitalization period, provided a legal basis for all treatment procedures, and served as a basis for future clinical studies. / BV UNIFESP: Teses e dissertações
