_______________________________________________________________________________________ ABSTRACT / In the Federal District from 2001 to 2006, almost 70.0% of suspected cases of dengue fever have not confirmed this disease and remain without diagnosis. Due to this problem, the study aimed to expand the surveillance of acute febrile syndrome (AFS) among the suspected cases of dengue fever with serology nonreactive. The descriptive study was composed of 144 persons residing in the DF, from January to May 2008. The serology for IgM to dengue fever was collected in the convalescent persons; cases not confirmed were investigated for antibodies to rubella, leptospirosis and hantavirus, selectively. The study was approved by the Committee of Ethics in Research of the SES-DF, in the opinion No. 215/2007. The incidence found was 27.1%. The proportion of dengue fever was 11.8%, rubella 4.0%, and leptospirosis 14.1%. There was no confirmation of hantavirus infection. In the comparison of dengue fever, rubella and leptospirosis with race / color (p = 0.0351) and education (p = 0.0096) a statistical difference was observed. Compared symptom of dyspnea in dengue fever and leptospirosis (p = 0.0124), dengue fever and rubella with rash / petechiae (p = 0.0007), a statistical association was observed too. The implementation of activity in a vacant lot between diagnosed cases of leptospirosis had a relative risk of 3.75 times (p = 0.0135). Dengue fever was present in 34.5% of administrative regions, rubella in 17.1% and leptospirosis in 37.9%. About 85.0% of illnesses occurred during the first quarter, rainy season in the FD. The findings of this case related to dengue fever, leptospirosis and rubella represent a percentage of 3.5%, 6.9% and 170.0% respectively compared to the total of confirmed cases registered in SINAN / DF, during the same period of search. The monitoring of AFS involving suspected cases of dengue fever was important in the elucidation of other diseases and strengthened the epidemiological surveillance.
