En el presente trabajo se analizó el consumo de colistina en el Hospital Nacional Edgardo Rebagliati Martins (HNERM) durante el periodo de febrero – agosto 2012, mediante un análisis descriptivo-retrospectivo de las historias clínicas y análisis de las recetas prescritas encontradas en ellas. Se evaluó las condiciones de prescripción de colistina en el HNERM. Los resultados obtenidos muestran que las condiciones de prescripción del antibiótico se basan en su mayoría, en diagnósticos confirmatorios antes de su administración al paciente en 77,88% de un total de 104 pacientes; de estos, se tienen que 40 pacientes presentaron como agente patológico causal-identificado a Pseudomonas aeruginosa MDR y 34 pacientes, para el Acinetobacter baumannii. El 52,50% y 32,35%, respectivamente, del total finalizaron el tratamiento con resultados favorables (efectividad clínica y microbiológica); la duración de las infecciones con mayor incidencia, identificadas en el estudio, varía en general en el rango de 6 a 10 días, así como el tratamiento de bacteriemia, ITU y neumonía; el tratamiento de la infección de partes blandas presenta una duración entre 6 a 15 días. El grado de neurotoxicidad no se evidenció significativamente en el presente estudio; sin embargo, para el grado de nefrotoxicidad se tiene que en 40,38% de los pacientes que usaron el antibiótico fue necesario un ajuste de dosificación por alteración de sus valores de creatinina, úrea y albúmina en sangre fuera de los rangos normales o valores basales. El número total de ampollas consumidas en el periodo de investigación fue de 2600, con un costo que asciende a 913 770,00 nuevos soles, los cuales se reparten entre los servicios de mayor consumo: Unidad de Cuidados Intensivos (UCI), Neurocirugía Vascular y Medicina Interna. multidrogo-resistentes / --- In this paper the use of colistin was studied during the period from February to August 2012 at the National Hospital Edgardo Rebagliati Martins (HNERM), it was made through a descriptive-retrospective analysis of medical records and prescriptions. It was assessed the prescribing conditions of colistin in HNERM. The clinical results about antibiotic prescribing conditions are based mostly on confirmatory diagnosis prior administration to the patient in 77.88% from a total of 104 patients. 40 patients had pathological agent causal-identified to MDR Pseudomonas aeruginosa and 33 patients, to Acinetobacter baumannii. 52,50% and 32,35%, respectively, completed the treatment with favorable outcomes (clinical and microbiological effectiveness); the duration of infections with the highest incidence, identified in the study generally varies in the range of 6-10 days as well as the treatment of bacteremia, ITU and pneumonia; treatment of soft tissue infection has a length between 6 to 15 days. The degree of neurotoxicity was not significantly evident in the present study; however, for nephrotoxicity the 40.38% of patients who used the antibiotic was necessary the adjustment of dosification because of alteration of creatinine values, urea and albumin in blood outside the normal range or baseline. The total number of vials consumed in the study was 2 600, with total cost of 913 770.00 soles, which are distributed among the largest consumer services: Intensive Care Unit (ICU), and Vascular Neurosurgery and Internal Medicine.
