Previous issue date: 2006-10-16 / The objective of this research project was to investigate the subjective
implications of breast cancer and mastectomy from a clinical case study
perspective, performed in a hospital. The learning objective of this project was
based upon patient speech in regards to her own experiences of having breast
cancer. The investigators chose to approach this pathology through biomedical
aspects in order to better understand its physiological complexity, as well as its
ethiology, therapeutical and preventive possibilities. Also, the investigators
considered the risk factors in the present time, which have consequently
contributed to the growth of epidemiological indices. A subjective analysis was
done based on how the body is formed as well as in its psychosomatic shape.
The approach used when analyzing the body was through the psychoanalysis
process, considering the erogenous and pulsating body that is formed in alterity.
As a result, we reflected on the mastectomized body and presented a new
vision on breast cancer and its illness symptons through a comprehensive
physician qualitative research. This research was performed at the General
Hospital in Fortaleza from December 2005 through July 2006. We applied the
psychosomatic psychoanalitic perspectives as it offered the possibility of a
differentiated view on the fallen ill body, to comprehend the patient in a
subjective interaction with the illness. Remindnful of the illness and its
implications, family genetics and preivous history of the patient was considered.
The appearance of breast cancer as an indentity mark in patient s family,
emerged as a family emblum . The described clinical case made us think on a
mutilated body, amputated in the breast, acquired subjective directions. When
the subjectivity in the process of the illness was considered, it was verified that
the concepts on health and sickness cannot be regarded in a linear form or
standardized, but subjectively. Therefore, if an individual has a lesion or injury,
their body will resist to it in a peculiar fashion. Finally, we emphasized the
interdisciplinary aspects of this research and its possible benefits to the health
industry in general.
Key words: interdisciplinary, medicine, psychoanalysis, breast cancer, body. / O presente trabalho tem como escopo investigar as implicações subjetivas do câncer de mama e da mastectomia a partir do estudo de um caso clínico realizado em uma instituição hospitalar. Objetiva-se aprender, com o discurso da participante desse estudo, sobre a vivência de se ter câncer de mama. Aborda-se os aspectos biomédicos dessa patologia para se entender sua complexidade fisiológica, bem como sua etiologia, possibilidades terapêuticas e preventivas, considerando, também, os fatores de riscos na atualidade que, conseqüentemente, favorecem o crescimento dos índices epidemiológicos. Analisa-se como o corpo é constituído no campo da subjetividade, bem como em sua organização psicossomática. Aborda-se a visão do corpo para a Psicanálise, considerando o corpo erógeno e pulsional que se constitui na alteridade. Nesse sentido, abre-se espaço para reflexões a cerca do corpo mastectomizado, apresentando uma visão a respeito do câncer de mama e as formas de adoecer a partir de uma pesquisa de cunho clínico-qualitativo realizada no Hospital Geral de Fortaleza, no período de dezembro de 2005 a julho de 2006. Utilizam-se as perspectivas psicanalíticas da psicossomática, por oferecerem a possibilidade de uma escuta diferenciada no tocante ao soma adoecido, procurando apreender o paciente numa interação subjetiva com a sua doença. Ressaltam-se as implicações da doença na família, considerando tanto a herança genética como histórica do sujeito. Observamos o aparecimento do câncer de mama como marca identitária na família da entrevistada, emergindo como um brasão familiar. O caso clínico descrito neste trabalho faz a sua autora pensar sobre um corpo mutilado, amputado em seu seio, mas que adquire sentidos subjetivos. Quando se considera a subjetividade no processo do adoecimento, verifica-se que os conceitos sobre saúde e doença não podem ser havidos de forma linear, normatizados, mas no campo subjetivo, pois cada sujeito se serve de sua lesão e resiste a ela de uma maneira muito peculiar. Destaca-se, por fim, o caráter interdisciplinar desta pesquisa e seus possíveis benefícios às outras áreas da saúde.
