Previous issue date: 2013-07-10 / This study aimed to investigate the contributions of psychoanalysis and its specificities in the
field of mental health from the psychiatric reform. The methodology for this research was
based on a theory reflexion (literature review) about Mental Health from the psychiatric
reform by comparing it with psychoanalysis. We investigate the following topics: 1.
regarding to mental health- legislation and the process of psychiatric reform in Brazil,
medical devices and equipment used as an alternative treatment in Instituitions. 2. regarding
psychoanalysis: the notions of subject, psychological distress and therapeutic. 3. regarding
the presence of psychoanalysis and its medical devices in mental health institutions: the place
of the analyst, the discussion of the clinical diagnosis and management. Considering that the
Public Policies in mental health have as goals the achievement of autonomy and psychosocial
rehabilitation of mental distress. We identify goals that guide psychiatric reform, when taken
as a hegemonic character exactly opposite the clinical that should guide treatment strategies
of people in psychological distress. We discussed whether there is a place for the subject, as
envisioned by psychoanalysis subject of the unconscious, instinctual and desiring - the
practices developed in the field of mental health, ratifying the proposed clinic expanded
construction of the case history and concomitant development of a therapeutic project
singular and possibilities of building a place for and by the subject and its vicissitudes.
Keywords: Psychoanalysis. Subject. Mental Health. Psychiatric Reform. Extended Clinic. / Este trabalho teve como objetivo investigar as contribuições e especificidades da psicanálise
no campo da saúde mental a partir do movimento da reforma psiquiátrica. A metodologia
utilizada para a realização desta pesquisa foi revisão bibliográfica acerca da saúde mental a
partir da reforma psiquiátrica em interface com a psicanálise. Foram pesquisadas as seguintes
temáticas: 1. quanto ao campo da saúde mental: legislação e o processo da reforma
psiquiátrica no Brasil, equipamentos e dispositivos clínicos alternativos ao tratamento asilar;
2. quanto à psicanálise: as noções de sujeito, sofrimento psíquico e terapêutica; 3. quanto à
presença da psicanálise e seus dispositivos clínicos nas instituições de saúde mental: o lugar
do analista, o trabalho em equipe, a discussão diagnóstica e o manejo da clínica.
Considerando que as Politicas públicas em saúde mental têm como metas a conquista da
autonomia e a reabilitação psicossocial do portador de sofrimento mental visando à
consolidação da cidadania, com a Psicanálise interrogamos tais objetivos norteadores da
reforma psiquiátrica, quando tomados como hegemônicos frente ao caráter propriamente
clínico que, eticamente, deve nortear as estratégias de tratamento das pessoas em sofrimento
psíquico. Discutimos se há um lugar para o sujeito, como pensado pela psicanálise sujeito
do inconsciente, pulsional e desejante nas práticas desenvolvidas no campo da saúde
mental, ratificando as propostas da clínica ampliada, da construção do caso clínico e
concomitante elaboração de um projeto terapêutico singular como possibilidades de
construção de um lugar para e pelo sujeito e suas vicissitudes.
