Previous issue date: 2005-10-05 / Conselho Nacional de Desenvolvimento Científico e Tecnológico / In this study, we intend to articulate the elements which comprehend the meta-psychological basis for the borderline functioning according to Andre Green vis-à-vis the in-home psychotherapeutic clinic by means of illustrating a case study, with specific focus geared towards certain dimensions of the object relations, namely the relations with the absolutely necessary object . Our interest in this study comes from the issues arisen in the in-home psychotherapeutic clinic based on diagnosed problematic and the specifics of the kind of framing. It comprehends a theory development around the primary defense mechanisms, which means, denying mechanisms Verdrangung, Verneinung, Verleugnung and Verwerfung -, created by Freud, and rethought by Andre Green, which he called Denying work . Such mechanisms would be in the principles of the borderline functioning process and would be constituted by the way in which the relations with the primary object were organized in the life history of the subject. According to this author, the object relations undertake an important role in therapeutic relation and from his theory of framing it seems possible to us to introduce the clinic modality of the in-home therapy. We believe that the borderline diagnostic matter and the setting of the in-home therapeutic clinic are similar specially when concerning the uncommon and surprising, altering the standardized norms of the listening as it imposes a rearrangement of the therapeutic pair / Neste estudo, pretendemos articular os elementos que compreendem as bases metapsicológicas do funcionamento borderline, segundo Andre Green, com a clínica do acompanhamento terapêutico, por meio da ilustração de um caso clínico, com um enfoque especial voltado para certas dimensões das relações de objeto, quais sejam, as relações com o objeto absolutamente necessário . Nosso interesse neste estudo parte dos impasses surgidos no atendimento clínico como acompanhante terapêutico, em função da problemática diagnóstica e da especificidade do tipo de enquadramento. Compreende um desenvolvimento teórico acerca dos mecanismos primários de defesa, ou seja, os mecanismos de negação Verdrangung, Verneinung, Verleugnung e Verwerfung - descritos por Freud, e repensados por Andre Green, aos quais denominou de trabalho do negativo. Tais mecanismos estariam nas bases do funcionamento borderline e se constituiriam a partir do modo como se organizaram as relações com o objeto primário na história de vida do sujeito. Para este autor, as relações de objeto assumem um importante papel na relação terapêutica, e a partir de sua teorização sobre o enquadramento, nos pareceu possível introduzir a modalidade clínica do acompanhamento terapêutico. Acreditamos que a questão diagnóstica borderline e o setting do acompanhamento terapêutico se assemelham justamente no que expressam de incomum e surpreendente, alterando as normas padronizadas de escuta ao impor um rearranjo da dupla terapêutica
