2012_GeisaCristinaModestoVilarins.pdf: 2171903 bytes, checksum: 52e3f2c56192d023b974b690be14fad0 (MD5) / A regulação pública no Sistema Único de Saúde (SUS), ao garantir o acesso dos usuários aos serviços públicos, atua sobre a oferta dos mesmos, assume o planejamento das ações e estabelece a adequação desta oferta de acordo com as necessidades identificadas. O estudo tem por objetivo analisar o processo de regulação do acesso à terapia intensiva empregado na Secretaria de Estado de Saúde do Distrito Federal nos anos 2007 a 2011. Como objetivos específicos, busca descrever o modelo vigente de regulação, propor indicadores que permitam o monitoramento e avaliação de dimensões relacionadas à estrutura, ao processo e ao resultado, e verificar a percepção dos médicos reguladores quanto ao sistema regulatório atual. Trata-se de uma pesquisa quali-quantitativa. Foram recomendados 16 indicadores, entretanto, verificou-se que o sistema informacional utilizado não possibilita uma análise gerencial abrangente sobre o modelo regulatório, sendo necessárias melhorias que possam subsidiar a tomada de decisão no processo gestor da regulação. A parte qualitativa foi baseada em entrevistas com 82,5% dos médicos reguladores da central de regulação. Os dados coletados foram analisados por meio do software ALCESTE (Analyse Lexicale par Contexte d’un Ensemble de Segments de Texte) que gerou duas classes temáticas na análise do discurso: uma expressa a predominância do processo de judicialização como entrave no processo de regulação, e a outra, refere-se às dificuldades operacionais inerentes à prática regulatória. A regulação do acesso à terapia intensiva apresenta problemas principalmente em relação ao processo de monitoramento e avaliação, além de se deparar com uma forte influência do Poder Judiciário. Contudo, mostra sua potencialidade como um instrumento importante para a execução das ações em saúde, com vistas a subsidiar os processos avaliativo e gestor tão necessários à qualificação do sistema. _________________________________________________________________________________________________ ABSTRACT / The public regulation in Brazil´s Unified Health System (SUS) exists to ensure users' access to public services and does this by acting on its offer and by planning actions in order to match the services offering with the identified needs. This study aims to analyze the process of regulating the access to the intensive care offered by the Federal District´s Secretary of State for Health between the years 2007 and 2011. Specifically, this research intends to describe the current model of regulation and the standards through whom the regulation´s structure, process and results can be monitored and evaluated. Furthermore, it verifies what is the medical regulators´ perception of the current regulatory system. The research was both qualitative and quantitative. At first, 16 indicators were recommended, however, it was found that the information system used does not allow a comprehensive management analysis on the regulatory model, so improvements are needed to support decision making in the management process of regulation. The qualitative part of the research was based on interviews with 82.5% of the medical regulators of the central regulation office. The collected data were analyzed using the software ALCESTE (Analyse d'um Lexicale pair Contexte Ensemble de Segments de Texte) that generated two classes in the thematic analysis of discourse: one expresses the predominance of judicial decisions interfering in the regulation process, and another the presence of operational difficulties inherent to the regulatory practice. The regulation of the access to intensive care presents problems especially in relation to its monitoring and evaluation and the strong influence of the Judiciary System. However, it shows its potential as an important instrument for the implementation of health initiatives, supporting the evaluation and management processes that are necessary to its improvement.
