Previous issue date: 2000 / Resumo: Cavernomas são malformações vasculares congênitas e consistem em vasos capilares sinusóides de paredes finas, de tamanho variado, sem artérias, veias ou tecido neural dentro da anomalia. O presente trabalho teve por objetivo avaliar uma série de 84 pacientes com esta anomalia, acompanhados no serviço de neurocirurgia do Centro Hospitalar e Universitário da Pitié-Salpetrière em Paris, no período de 1953 a 1995, visando um estudo do diagnóstico clínico e neuro-radiológico, e das propostas de tratamento Comparando a série com os dados da bibliografia, buscamos avaliar a história natural da doença e os resultados, para determinar o tratamento adequado dos diferentes quadros da enfermidade. Usando o método de análise retrospectiva juntamente com um estudo longitudinal retrospectivo em ambulatório, foi-nos possível classificar os pacientes segundo a expressão clínica, e analisar os resultados. O método diagnóstico de escolha foi lRM(Imagem por Ressonância Magnética). Em nossa série, 39 indivíduos apresentavam crises convulsivas, 15 apresentavam hemorragia cerebral ou meníngea evidente, e 28 tiveram evolução tipo pseudo-tumoral após hemorragia, tendo sido agrupados com os hemorrágicos. Na comparação estatística, os dados foram similares com a bibliografia, exceto na precocidade do ressangramento, em 28% dos casos. O total de ressangramentos ficou em 38%, com 28 casos. Mesmo na ausência de elementos de malignidade confirmado pela histopatologia,
devemos considerar o caráter evolutivo da lesão, que associado com os resultados, nos levou à indicação do tratamento cirúrgico precoce, curativo, especialmente em áreas acessíveis. Devemos ressaltar porém, a necessidade de uma avaliação cuidadosa de cada paciente, para a adequada escolha do tratamento / Abstract: Cavernous angiomas are congenital vascular malformations, characterized by enlarged capillary cavities without intervening with arteries, veins or brain parenchyma. The objective for the present study is to analyze a series of 84 patients with this pathology follow at the Centre Hospitalier Pitie-Salpetriere in Paris in the period of 1953 to 1995, for a diagnostic and neuroradiological study and the propositions of treatment. Evaluating this series and comparing with data from the biography, we study the natural history of the disease and the results, looking for the best treatment for the different types of presentations. Using retrospective analysis as method, added with longitudinal retrospective ambulatorial study, it was possible for us to classify the patients by clinical presentation and verify the results of the different treatments. The method of choice for diagnosis was MRI. In this series 39 patients had seizures, 15 had cerebral or subarachnoidea hemorrhage and 28 previously had a hemorrhage with a pseudo tumoral evolution; they have been classified as hemorrhagics. In this statistical comparison the results were similar to those already published; however, the patients have experienced rebleeding far sooner than the documented time in 28% of the cases. The total of rebleedings was 38% with 28 cases. Even in the absence of elements of malignity confirmed by histopathology, we took in consideration the evolutivity of the pathology, associated with the results, to show the tendency to the choice precoce surgical treatment, curative, especially in accessible brain areas.
