Previous issue date: 2000 / Resumo: o presente trabalho aborda as dificuldades do Sistema de Saúde da cidade de Mauá, situada na região metropolitana de São Paulo, na estruturação da assistência à saúde, apesar de uma longa trajetória de construção e investimento em equipamentos e recursos humanos para o setor. Utiliza uma metodologia com diferentes etapas que inclui entrevistas com usuários, observação do funcionamento das unidades de saúde, entrevistas com dirigentes e funcionários de saúde e entrevistas com pacientes do serviço de urgência-emergência com risco para hipertensão arterial, diabetes mellitus, doença bronco pulmonar obstrutiva crônica, câncer de mama e câncer de colo de útero. Nos resultados foram encontradas 'falas' semelhantes dos diversos atores sociais acerca da falta de vínculo, autonomia, capacidade de resolver os problemas de saúde e um certo 'mal estar' generalizado, que se concretiza na observação do funcionamento das unidades de saúde e nos questionários de risco, onde aparecem dados preocupantes, como 31 % dos pacientes diabéticos e 33% dos hipertensos que não sabiam do risco. Os resultados são analisados diante das chamadas necessidades de saúde trabalhadas por Stotz e mais recentemente por Matsumoto e Cecílio, que são sistematizadas em quatro itens: necessidade de boas condições de vida, necessidade de vínculo com os serviços de saúde, equipes ou profissionais de saúde, necessidade de acesso a todas as tecnologias que melhorem, prolonguem a vida e amenizem o sofrimento e necessidade de autonomia do paciente na relação com os profissionais de saúde. Após confrontar os resultados obtidos com o areabouço coneeptual de análise,
concluiu-se que o Sistema de saúde de Mauá não atende às necessidades de saúde de seus usuários, embora tenha os instrumentos necessários. Finalizou-se com algumas considerações quanto à reformulação necessária, para iniciar o enfrentamento desta dura realidade, que parece ser comum a diversos municípios / Abstract: The present work approaches the difficulties of the System of Hea1th of the city of Mauá, located in the metropolitan area of São Paulo, in the structuring of the attendance to the health, in spite of a long construction path and investmem in equipments and human resources for the section. lt uses a methodology with different stages that it includes interviews with users, observation of the operation of the units of health, interviews with leaders and employees of health anel interviews with patients of the urgency-emergency servics with risk for high blood pressure, diabetes mellitus, chronic obstructive lung discase, mama cancer and cancer of uterus lap. In the results we found 'speeches' similar of the several social actors concerning the entail lack, autonomy, capacity to solve the problems of health and a right 'badlly to be' wiedespread, that is rendered in the observation of the operation of the units of health and in the risk questionnaires, where they appear preoccupying data, as 31 % of the diabetic patients anel 33% of
the hypertension patients that didn't know about the risk. The results are analyzeel before the calls needs of health worked by Statz and more recently for Matsumato and Cecilia, that are systematized in four items: need of good life conditions, entail need with the services of health, teams or professionals of health, access need the all the technologies that get better, prolong the life and soften the suffering, and need of the patient's autonomy in the relationship with the professionals of health. After confronting the results obtained with the conceptual framework of analysis, we concluded that the System of health of Mauá doesn't assist the needs of your users' health, although he/she has the necessary instruments. We concluded with some considerations as the necessary reformation, to begin the act of facing of this reality lasts, that seems to be common to several municipal districts / Mestrado / Mestre em Saude Coletiva
