Previous issue date: 2001 / Resumo: A clínica com psicóticos, vista sob a perspectiva das leituras de Sigmund Freud e Jacques Lacan e revista a partir de suas particularidades, de uma inserção no dispositivo político-institucional e no discurso da Saúde Mental é o fio condutor deste trabalho. A revalorização do espaço da e para a clínica dentro da Saúde Mental, nas instituições e junto aos agentes responsáveis pela condução do processo de Reforma Psiquiátrica brasileira, é seu objetivo maior. Algumas referências conceituais permitiram discernir este percurso: normatividade, a relação paradigmas/ideologia/legitimação científica, o aparelho psíquico freudiano e a teoria lacaniana sobre as psicoses são as principais. A discussão acerca delas é feita num capítulo específico, mas estes elementos teóricos permeiam todo o trabalho. Neste trajeto, dois ¿encontros¿ foram necessários e precederam a discussão mais própria sobre a questão clínica. Foram eles os encontros com os discursos da Psiquiatria e da Reforma Psiquiátrica. Para falar de ambas houve a necessidade de escolher uma leitura e um recorte diante da extensão e complexidade destes saberes e práticas, que não são propícios à abordagens unificadoras. Sem tais recortes porém, este trabalho ¿ que não visa uma crítica epistemológica nem um levantamento histórico ¿, não teria sido possível. Assim, correndo o risco da parcialização de um enfoque por um lado, e da superficialidade por outro, foi mantido o objetivo anunciado de ¿dar voz à clínica¿. Quanto ¿à¿ psiquiatria, a escolha foi por uma literatura que procurasse explorar a relação de filiação psiquiatria/medicina e suas implicações nas operações intelectuais em jogo no exercício da clínica psiquiátrica. Quanto ¿à¿ Reforma Psiquiátrica, a interrogação de textos de Basaglia e outros acerca do lugar reservado à clínica, ou seja, à particularidade radical da experiência e do discurso do louco, foi o que orientou a pesquisa. Finalmente, para ¿dar voz¿ à clínica, uma exploração sobre a noção de caso clínico em psicanálise e como não poderia deixar de ser, o escrito desses casos que foram atendidos pelo autor/equipe num serviço alternativo de Saúde Mental. A partir disso (com o objetivo de diferenciar-se de um escrito sobre casos), pretendeu-se lançar questões mais genéricas e interpelar o autor, os serviços, as equipes, o discurso da Saúde Mental Pública, a psicanálise e indiretamente, o valor universalizante da política, suas injustiças, claudicações e antinomias / Abstract: The clinic with psychotic people, seen under the Sigmund Freud and Jacques Lacan's reading perspectives and reviewed from those particularities and one insertion in the politic-institutional device and in the speech of Mental Health, is the conduction wire of this work. The revaluation of the space of and for the clinic in the Mental Health, in the institutions e with the agents in charge for the conduction of the Brazilian Psychiatric Reform process, its biggest target.Some conceptual references allowed to discern this course: normativity, the paradigm-ideology-legitimization scientific relation, the freudian psycho device and the lacanian theory about psychosis are the main ones. The discussion about them is made in a specific chapter, but those theoretical elements permeate the entire thesis. During this course, two ¿meetings¿ were needed and preceded the discussion itself about the clinic question. They were the meetings with the psychiatry and psychiatric reform speeches. To be able to say about both ¿meetings¿, there was the necessity of choosing a reading and a psychiatry piece before the extension and complexity of these knowledge and actions, not propitious to a unifier approach. Without these pieces this work (which nor aims an epistemological comments neither a historical putting) wouldn¿t be possible. Thus, taking the risk of partisan of a focus, by a side, and of the superficiality, by another, the announced target of ¿give the clinic its speech¿ was maintained. About ¿the¿ psychiatry, the chosen reading tried exploit the dependency psychiatry/medicine relation and its implications on intellectual operations at stake in the exercise of psychiatric clinic. About ¿the¿ Psychiatric Reform, the Basaglia's text questions and others about the reserved place for the clinic, what means, for the experience's radical particularity and the lunatic speech, were what guided the research. Finally, to ¿give the clinic its speech¿, the exploitation about the conception of clinic case in psychoanalysis and, how it couldn¿t be different, the reports about the clinic cases assisted by the author/staff in one alternative service of mental health, From those cases (looking for making differences between a report about cases), the daring of putting questions more generically and interpellate, the services, the staffs, the public mental health speech, the psychoanalysis and indirectly, the universalizing policy value, those injustices, claudicating and contradiction / Mestrado / Planejamento / Mestre em Medicina Preventiva e Social
