Previous issue date: 2014 / A saúde, em tempos de globalização, não se atém a fronteiras; e as dificuldades comuns na área da saúde dos países sul-americanos, geram oportunidades na área da cooperação em saúde e, por consequência, no novo campo da diplomacia da saúde. A criação da UNASUL SAÚDE em 2008 e do Instituto Sul-Americano de Governo em Saúde (ISAGS) em 2011 são exemplos do surgimento de novos atores no campo da cooperação internacional em saúde. Neste âmbito, tem sido proposto o conceito de cooperação estruturante em saúde, como orientação programática para a cooperação brasileira em saúde, nos marcos da cooperação sul-sul para o desenvolvimento. Esta dissertação se propõe a discutir se o processo de constituição, organização e funcionamento inicial da UNASUL SAÚDE e do ISAGS configuram essas organizações como instrumentos adequados para a promoção de uma cooperação estruturante em saúde na região. Nota-se que a institucionalidade da UNASUL SAÚDE e do ISAGS buscam incluir funções e estratégias diferentes da OPAS e outros organismos já existentes, disputando a hegemonia do papel na liderança do tema da Saúde nas Américas e projetando suas ideias e valores a nível global. Ainda não está claro, entretanto, qual será o futuro desses jovens atores dado que suas atuações dependem do cenário geopolítico sul americano que é influenciado pelas mudanças de Governos nacionais e de prioridades de política externa de cada um dos 12 países membros da UNASUL. O Brasil nos parece ter um excesso de protagonismo político e técnico dentro da UNASUL SAÚDE, gerando, de certa forma, uma ameaça a proposta de integração regional de cooperação estruturante por desproporcionalidade do Brasil perante os demais países do bloco. / In times of globalization, health issues do not respect frontiers and mutual necessities in the domain of health in the South American countries created new opportunities for health cooperation and in the new field of health diplomacy. The creation of UNASUR Health in 2008 and ISAGS in 2011 are examples of new actors in the international health cooperation field. It has been proposed the concept of structural health cooperation, as a pragmatic orientation for the Brazilian health cooperation, in the boundaries of the south-south development cooperation. This thesis has the intention of discussing if the process of creation, management and organization of UNASR, UNASUR Health and ISAGS allow these new actors to act as adequate instruments in order to promote a structural health cooperation in South America. UNASUR Health and ISAGS use new strategies and functions when compared to PAHO and other health organizations, disputing the role of hegemony on the leadership in the South American health sector as well as trying to project its values and ideas globally. It is not yet clear what the future holds for these new actors since its actions depends on the South American geopolitical scenario, which is influenced by the change of national governments as well as the changes of priorities of the government s foreign policies. Brazil, who in our humble opinion, has an excessive political and technical influence in UNASUR Health which can be taken as a threat to the structural regional integration proposal because of the asymmetries between Brazil and the other member states.
