Previous issue date: 2010 / Technological progress and knowledge obtained in the area of transplants demarcate the development and the consolidation of the important place occupied by lung transplant in the treatment of advances lung diseases. In this development stands out the adjustment of criteria to the selection of candidates for this procedure. According to guidelines established by the Ministry of Health, these candidates, without exception, are carriers of terminal pneumopathology, with severe limitations, for these reasons they are dependents of oxygen, and with a life expectancy of 18 months or less than this. Thus, the situation of these candidates to lung transplant involves a double aspect: on one hand a pathology resolution is opened by the effective performance of the transplant; on the other hand the indication of this procedure assumes a clinical state very severe, in other words, the real risk of death is highlighted. It is understood that a condition of physical pain adds important psychic impact, proving the intrinsic relationship between body and psyche. From psychoanalysis theory, this dissertation aims to understand, from the experience of the candidates to a lung transplant, the complex experience of being in a list to wait for this procedure. Two sections were prepared about the issue: a theoretical and an empirical. The theoretical study, from a review of the psychoanalysis theory about the trauma, offers a relationship with the traumatic situation that involves the waiting for a lung transplant. The empirical section develops a qualitative studying and it aims to investigate the experiences of candidates who are waiting for a lung transplant. Eight patients were interviewed, six men and two women, who are part of the Lung Transplant Group of the Complexo Hospitalar da Santa Casa de Misericórdia de Porto Alegre.Through the technique of the Content Analysis, four final categories were identified and some vignettes were illustrated with candidates statement so called: singularity and complexity: the posture of awaiting of the transplant. Spaces shared on the awaiting: resonance box to the fear and expectations; the historicizing and bounds as resources to build of meaning before the excess and finally, the experience of helplessness and the care and reception through another person. For a discussion of the findings, psychoanalysis theory was used. The aspects found in this dissertation allow a deep understanding about the psychic suffering intrinsic to the traumatic experience of waiting for a lung transplant. The complex effects of this awaiting were highlighted and the severity of the diagnosis that put these patients in the condition of candidate of this procedure. In this context, it is highlighted the benefits that may come with the listening offered through psychoanalysis as a possible tool to build a possibility of assigning meaning to the psychic intensities in which configure a traumatic experience of the waiting of life. / Progressos tecnológicos e conhecimentos obtidos na área de transplantes demarcam o desenvolvimento e a consolidação do importante lugar ocupado pelo transplante pulmonar no tratamento de doenças pulmonares avançadas. Nesse desenvolvimento se destaca o ajustamento de critérios para a seleção de candidatos a tal procedimento. Conforme diretrizes estabelecidas pelo Ministério da Saúde, esses candidatos, sem exceção, são portadores de pneumopatologia terminal, com graves limitações, que os tornam dependentes de oxigênio, e com uma expectativa de vida inferior a 18 meses. Dessa maneira, a situação na qual se encontram os candidatos a transplante pulmonar envolve um duplo aspecto: se por um lado abre-se a expectativa de resolução da patologia por meio da efetiva realização do transplante; por outro, a indicação desse procedimento pressupõe um estado clínico muito grave, ou seja, evidencia o risco real de morte. Entende-se que uma condição de sofrimento físico agrega importantes repercussões psíquicas, comprovando a intrínseca relação entre corpo e psiquismo. A partir dos aportes psicanalíticos, esta dissertação tem o objetivo de compreender, a partir da experiência de pacientes candidatos a transplante de pulmão, a complexa vivência de estar em lista de espera para tal procedimento. Foram elaboradas duas seções sobre o tema: uma teórica e uma empírica. O estudo teórico, a partir de uma revisão dos aportes teóricos da Psicanálise sobre o trauma, propicia uma interlocução com a traumática situação que envolve a espera por um transplante pulmonar. A seção empírica desenvolve um estudo de cunho qualitativo, que tem o objetivo de investigar as vivências dos candidatos que estão em lista de espera por um transplante de pulmão.Foram entrevistados oito pacientes, seis homens e duas mulheres, vinculados ao Grupo de Transplante Pulmonar do Complexo Hospitalar da Santa Casa de Misericórdia de Porto Alegre. Por meio da técnica de Análise de Conteúdo, foram identificadas quatro categorias finais, ilustradas com vinhetas das falas dos participantes, e assim denominadas: Singularidade e complexidade: a postura diante da expectativa do transplante; Espaços compartilhados na espera: caixa de ressonância para temor e expectativas; A historização e os vínculos como recursos para construção de sentido frente ao excesso e, por último, A experiência do desamparo e o acolhimento no cuidado via outro. Para a discussão dos achados, lançou-se mão do referencial psicanalítico. Os aspectos encontrados nesta dissertação permitem uma compreensão aprofundada sobre os padecimentos psíquicos intrínsecos à vivência traumática da espera por transplante pulmonar. São evidenciados os complexos efeitos dessa espera e a gravidade do diagnóstico, que os coloca na condição de candidatos a tal procedimento. Nesse contexto, evidenciam-se os benefícios que poderão advir da qualidade da escuta oferecida pela Psicanálise como instrumento possível de construir a possibilidade de atribuição de sentido às intensidades psíquicas, as quais configuram uma vivência traumática da espera pela vida.
