Prevalence of Decompression Sickness and Dysbaric Osteonecrosis in Rescue Divers and Well Workers / 潛水之救難人員與鑿井工程人員健康調查

Taiwan. There are several unique divers who work in special
environments may be associated with high incidence of
decompression sickness. The aims of this study were to checkup
the health status of divers, to examine the diving profiles,
and to investigate the prevalence of decompression sickness and
dysbaric osteonecrosis of these divers. Our survey showed that,
in the past year, 6.7% of these naval rescue divers experienced
limb bends and 38.1% of them had diving-related symptoms other
than bends, whereas 63.6% of the well workers experienced limb
bends and other diving-related symptoms. The risk factors for
decompression sickness among these divers included the habit of
alcohol drinking, full-time diver, and diving without plan.
Subjects received a series of x-ray examinations to evaluate
the necrotic change of long bones. The prevalence of dysbaric
osteonecrosis in the well workers was 69.2%, which is higher
than the other published records in recent years. In contrast,
there was no osteonecrosis in the naval rescue divers. The
neurological examinations revealed that 40 naval divers who
experienced diving-related symptoms had higher vibration
threshold, which implied a neural impairment. This may be a
sequela of decompression sickness. We also found that 39.1% of
the well workers had abnormal level of complement C3, which is
probably a result of repeated exposure to high pressure
environment. In summary, the naval rescue divers, who
relatively have short experience of diving, full training
course, better knowledge of diving safety, and well-supported
medical resources, experienced less decompression sickness and
had no evidence of dysbaric osteonecrosis. In contrast, the
well workers, whoes general characters opposite to the naval
rescue divers, had higher incidence of decompression sickness
and higher prevalence of dysbaric osteonecrosis.
