Knowledge of Thalassemia, Social Support and Self Care Behavior in Adolescents with Beta-Thalassemia Major / 重型β海洋性貧血青少年疾病知識、社會支持與自我照顧行為之探討

碩士 / 國立臺灣大學 / 護理學系 / 86 / The purpose of this descriptive correlational study was to investigate the
relationships among knowledge of thalassemia, social support and self-care be
havior in adolescents with beta-thalassemia major. A purposive sampling me
thod was used in obtaining the database that consisted of a total of 58 patien
ts aged from 12 to 20 years old who have been diagnosed as beta-thalassemia ma
jor. These patients were selected from the pediatric hematology outpatient dep
artment of one medical center and two quasi-medical centers in the Northern Ta
iwan area. Data were collected by self-design questionnaires. The questionnair
es used in the research include : demographic data , self-care behavior scale
, knowledge of thalassemia scale and social support scale. Data were analyzed
by descriptive statistics , t test , oneway ANOVA , Pearson''s correlation and
stepwise multiple regression. The major findings of this study were as foll
ows : 1.The percentage of scoring in knowledge of thalassemia was 72.1% ; in t
he five categories the best was treatment knowledge , and the worst were the k
nowledge of complications and symptoms. The factors affecting the knowledge of
thalassemia included level of education , length of illness , complications o
f thalassemia and hospitalization due to thalassemic complications.2.The resul
t on social support indicated that family were the major sources of support ,
and that classmates/friends provided the least support for thalassemic adolesc
ents. Among the four types of functional support , emotional support was found
to be higher than others three : apprasial , informational and tangiable supp
ort. The factors affecting the support of adolescents with beta-thalassemia ma
jor included gender, family history, complications of thalassemia, hospitaliza
tion due to thalassemic complications and receiving other treatment.
3.The percentage of scoring in self-care behavior was 77.1% ; in three c
ategories the best was medical and chelation therapy , and the worst was manag
ement of problem and coping. there was only gender that affected the self-care
behavior.4.Overall knowledge of thalassemia , general knowledge of thalassemi
a and treatment knowledge were positively correlated with self-care behavior(p
<0.01) , indicating that thalassemic adolescents who reported better knowledge
of thalassemia also reported better self care.5.Overall social support and so
cial support including family , professional staff , emotional , informational
, tangiable and appraisal support which were positively correlated with self-
care behavior(p<0.05) , indicating that thalassemic adolescents who reported m
ore social support also reported better self care.6.The findings indicated tha
t 50.0% of the variance in self-care behavior can be explained by emotional su
pport from family , general knowledge of thalassemia , treatment knowledge and
appraisal support from family. According to the results of this study , we
can find that better understanding of illness and providing adequate social su
pport will lead thalassemic adolescents to better self care. The results of th
is study could be referential material for clinical care and nursing education
. It also could be the basis for The Thalassemia Association of Taiwan , ROC w
hen it plans for related activities.
