The Study of Concerns and Adjustments of Adolescents with Beta-Thalassemia Major / 重度β型海洋性貧血青少年對其疾病的關注與調適行為之探討

碩士 / 國立臺灣大學 / 護理學研究所 / 87 / The purpose of this study is to investigate the concern and adjustments of adolescents with β-thalassemia major. The study design was a descriptive qualitative approach. The in-depth level interview with the 11 patients was by the semi-structure interview guide, and 40-90 minutes was taken to every patient. The tape-recorded data was transcribed into narrative form of process recordings. By means of content analysis, the concerns and adjustments were identified and analyzed.
After quantifying the qualitative data, 1639 behavior units were inductively coded. There are three major categories of concerns: body image 59.98%, medicine 37.16%, and future 2.87%. The concern of body image contains social function, physical sensation, physical function, and physical structure. The concern of medicine comprises medical life and the aspect of disease. The concern about future includes death, career, and marriage. There are three major categories of behavioral adjustments: cognitive operation 57.72%, emotion response 29.23%, and readjust behavior 13.06%. Cognitive operation contains identifying behavior, evaluating behavior, comparing behavior, detecting behavior, and expecting behavior. Emotion response includes avoidance, acceptance, anxiety, anger, grief, modification of expectation, and optimism. Readjust behavior comprises obeying the treatment process, changing the life style, building up one’s ability, diversion of one’s attention from the disease, explanation, and seeking assistance. The concern and behavioral adjustment of these patients affected by the gender and age.
The finding of this research may act as clinical nursing references. Furthermore, finding may provide nurses a better understanding of the adolescents with β-thalassemia, and provide relevant information and support to the patients in time and properly. To empower the patient’s ability is to adjust with illness and medicine life in more positive way, and it is easier to pass through the adolescent transition.
